Spoke with crew about installing fire hydrants on Donner. The Crew was on Donner last week and only installed 2 hydrants. 2 more hydrants need to be installed still. Approx. 9:30 a.m. While digging with a shovel; to build the receiving pit, contractor’s staff hit the water service line to the home located at 3054 24 th Street. Contractor did not phone any city staff to notify of the water hit. 10:00 a.m. Return to site. Reviewed traffic controls on 24 th . They need some correction; as there is no “Construction Ahead” or “Detour Ahead” signs up. I asked Billy to address this. When I approached the work area I noticed water flowing heavy in the gutter line. I observed 3 holes dug, all were full of water. Billy was in hole dug above the existing main trying to turn the corp off at the main. He was unable to get water shut down. 10:20 a.m. water was being pumped out of the excavation with no filter bags. Boring rig began to bore at home adjacent to the leak; 3056 24 th Street. 11:00 a.m. water was shut off and the crew was able to turn the line off at the main. Boring machine then relocated to 3054 24 th Street and began to bore again. CONTRACTOR ACTIVITY (Please time stamp observations by indicating time at beginning of each new item):

7:40 a.m. arrived on-site. Spoke with crew about installing fire hydrants on Donner. The Crew was on Donner last week and only installed 2 hydrants. 2 more hydrants need to be installed still.

11:15 a.m. As the machine pushed through the soil a load gush of gas blasted up. The bore machine had hit the gas main. I phoned mike Colasanti, Chris Powell and Brett Grant to report the incident.

11:40 a.m. PG&E supervisor; Erik Kurtz (916-386-5373) arrived on-site with a full crew to assess the damage. We started evacuations of the following homes: 3056, 3054 and 3048 24th Street. These homes were within a “blast distance”.

12:15 p.m. After speaking to Erik Kurtz about evacuations, we asked them to phone 911 as standby just in case a critical incident occurred.

12:25 p.m. Fire standby arrived. T&S was asked by PG&E to provide assistance in exposing their main. They made preparations to crimp the line while T&S dug. This crimp was placed on 24th street, between Curtis and Donner. More PG&E crews arrived to assist and relieved the T&S backhoe.

1:40 p.m. PG&E successfully crimped the line and stopped the flow of gas. Gas blew for 2 hours 45 mins which left a large void in the roadway. Many cars and T&S equipment was covered in sand and debris. No residents mentioned any damage to their homes or cars at this point.

1:50 p.m. PG&E requested that T&S assist in cleaning up the area in order for them to expose the damaged gas line. Once this was completed PG&E began to excavate the area to make a repair.

2:40 p.m. While PG&E was digging they hit the water service; 3056 24th, which was previously repaired by T&S this morning. This time the water service was pulled directly from the main. I phoned Mike Shilliday and 311 to report the issue.

3:20 p.m. City utility crew arrived and began to shut off water valves.

3:40 p.m. Water finally shut off. Crew was able to turn off corp stop at the main after dewatering. They then turned on the other valves; restoring all other homes on 24th Street.

3:55 p.m. While digging to expose the line PG&E hit the sewer service for 3056 24th street. They phoned a plumber to repair. His ETA is unknown.

4:30 p.m. PG&E working on cutting their line to make the repair. They stated it would be a long while before they are able to blow the gas line out and said the water service tech. could get in to do his work at this time. The City water tech. stated he could not make the repair and we needed to phone 311 to have another team come out.

5:10 p.m. PG&E informed me they have to remove the mud and water from the inside of the pipe prior to restoring gas service. They have to cut another section out to perform this work.

5:30 p.m. PG&E still not making any repairs. All supervisors have left the job site. 1 journeyman and 2 apprentices remain.

6:00 p.m. PG&E still not performing any corrections. No plumber on-site still to repair the sewer service. I asked the Journeyman for PG&E what the status was. He (Casey) stated it may be around 10pm.

6:10 p.m. Billy with T&S was not comfortable with leaving the resident without City services any longer and chose to make the repairs themselves. T&S then repaired the sewer and water. We then flushed the line and informed the homeowner they could use their utilities again.

6:40 p.m. PG&E still not performing any corrective work. No supervisors on site still. We informed the 3 man crew we had completed the repairs and backfilled our services. T&S provided 2 trench plates for PG&E to use as cover; asking them to not backfill the repaired services so we can ensure they are backfilled with approved material.

7:00 p.m. T&S and I left the site. PG&E still on-site. I asked Billy to come in early and clan the streets and gutter.

Damaged gas main shows the bore machine head penetrated the line.
