RESEARCH STATEMENT I have focused on neonatal brain injury and neuroprotection for the past 20 years. As a practising neonatologist at UCLH and the research lead of the Neonatal Neuroprotection Laboratory at UCL, I have had the opportunity to ensure our pre-clinical neuroprotection work is translational and relevant to clinical needs. The last 20 years has been an exciting and fast-moving area of neonatal medicine, with therapeutic hypothermia for neonatal encephalopathy being one of the most significant advances for neonatology. I use MRI, MRS and NIRS to understand brain injury mechanisms both in babies in the NICU and in the piglet model in the laboratory. Since 2003 when I was appointed Senior Lecturer at UCL, I have developed the piglet model into a valuable and unique resource to investigate the safety and efficacy of neuroprotective drugs for newborn infants using clinically relevant MR biomarkers, bedside tools such as EEG and immunohistochemistry. As therapeutic hypothermia is only partially effective, we have studied the safety and efficacy of adjunct therapies. Neuroinflammation is known to exacerbate brain injury in NE; we developed an LPS-sensitized model of hypoxia ischemia. Our studies have focused on optimizing cooling itself and adding melatonin, xenon, argon or erythropoietin (Epo) as adjuncts with cooling. We have a recent MRC DPFS award to investigate the safety and efficacy of human umbilical mesenchymal stem cells in the piglet model of hypoxia ischemia. The laboratory work has been mainly funded by the MRC (Neuroscience and Mental Health Board), following my MRC Young Investigator Award in 2006. In October 2020 I took up the position of Professor of Perinatal Neuroscience within Edinburgh Neuroscience & Centre for Clinical Brain Sciences (CCBS) at the University of Edinburgh. The access to world class laboratories here, including the large animal research and imaging facility (LARIF) next to the Roslin Institute https://www.ed.ac.uk/roslin/facilities-resources/larif will facilitate the clinical translation of the pre-clinical neuroprotection research. I am committed to the delivery of optimal clinical neurocritical care as part of a neuroprotective bundle. In the next 5-10 years I am keen to move towards setting up and inspiring clinical trials of the most promising adjunct therapies with cooling, the development of biomarkers to assess inflammation sensitization and tailoring neuroprotection to specific babies. In 2020/2011 I aim to set up a phase I/IIa clinical trial of melatonin as an adjunctive therapy to cooling, based on promising pre-clinical data (MELBA – Melatonin for birth Asphyxia). I was PI of the TOBY Xenon Trial (MRC Experimental Medicine first in man study) which used imaging biomarkers as surrogate endpoints and I am keen to build on this work. My interest in neuroprotection extends to low resource settings in particular sub-Saharan Africa. In 2019 the Bill and Melinda Gates Foundation awarded funding for pre-clinical work in my laboratory on repurposing Azithromycin as a neuroprotective agent. In Uganda, our studies led to an important message of caution regarding the translation of therapeutic hypothermia (a safe and effective therapy in the UK) to low resource settings. I have collaborations set up in Accra, Ghana where we are studying the seizure burden in NE. As the main burden of neonatal encephalopathy is in low resource settings, my aim is to ensure we develop neuroprotective interventions in the laboratory that are economical, practical, safe and translational to both high and low resource settings.
