Dear Friends,

I had started task of updating the details of the CSOS life members in our records after the last conference. I am happy to inform you all that with the efforts from our end and the support shown by the members themselves, we have updated contact address with mobile no and email ids of 274 members. It is available in our website also.

We have compiled last years annual GBM minutes also in this book because, it is the best way of knowing what is going on in the association and how we can improve it. There are a lot of issues which are being worked, if there is more input from all of you. I hope that in the coming conference you will make it possible by attending the annual GBM.

Every society has a soul as its “Bye Laws” and heart as its ofce bearers so, We have also updated our past ofce bearers in this book with one copy of “Bye Laws”.

It is my privilege to thank to My President Dr. D C Jain Sir for his faith on me. I am trying to deliver my best to the association.

The credit goes to the entire team of CSOS which worked to their level best to complete this endeavor.

Season's greetings

Chhattisgarh State Ophthalmological Society has grown to teen years. We have come a long way since beginning by the vision & efforts of our past stalwarts, leaders and of course our esteemed members. We have been planning to come up with the basic information about the society in print since a long time. I am very pleased that this book is now in your hands. This includes Bylaws of our society, Past ofce bearers, GB minutes of the last 6 years, Life members directory etc. This book has taken a great effort from our honorary general secretary Dr Santosh Singh Patel. I hope that this will be useful for you and you will appreciate the book.

CONSTITUTION AND BYLAWS OF

CHHATTISGARH STATE OPHTHALMOLOGICAL SOCIETY

ARTICLE 1. : NAMEChattisgarh State Ophthalmological Society (CSOS).

ARTICLE 2. : REGISTERED OFFICE The registered ofce of the society will be located at the place or address of the General Secretary till permanent ofce is built. (Room No. 272, Eye Section, Medical College, Raipur.)ARTICLE 3. : WORKING AREA- CHATTISGARH STATEARTICLE 4. : OBJECTSThe objects of the society shall be to cultivate and promote research, and manpower development to render service to the community and to promote social contacts amongst it's members by:Ÿ Organizing, conferences, symposia, seminars, workshops,

refresher courses etc. for promotion and mutual exchange of knowledge.

Ÿ Promoting clinical, experimental, community and operational research in ophthalmology.

Ÿ Instituting awards, orations, prizes, fellowships and research grants.

Ÿ Promoting, teaching, training and continuing education in ophthalmology by establishing and maintaining a reference library, publishing Journals and proceedings of the society and other such materials.

Ÿ Organizing exhibitions of machinery, tools appliances etc. pertaining to ophthalmology with a view to encourage and improve the manufacturers of such instrument and appliances in India and also to promote and advise quality control in the manufacture of the aforesaid instruments and appliances.

Ÿ Developing academic and scientic research funds and activities.

Ÿ Encouraging formation and afliation of city/ district/ divisional/ state ophthalmology societies.

Ÿ Co- operating with other medical societies.Ÿ Working towards promoting the welfare of the society.Ÿ Ensuring ethical practice and professional conduct of its members

as laid down by the society from time to time.Ÿ Maintaining liaison with the Central and State Governments and

various appropriate bodies regarding matters concerning teaching and practice of ophthalmology.

Ÿ Undertaking health education to prevent visual impairment and control of blindness.

ARTICLE 5. : MEMBER

(A) Category of Membership: The member of the society shall consist of –Ÿ Life Membership- Qualied ophthalmologists with allopathic

qualication recognized under schedule I or III of the Indian Medical council Act 1956, who is practicing or working and living in Chhattisgarh shall be eligible for life Membership.

Ÿ I n t e r n a t i o n a l & N a t i o n a l M e m b e r s h i p - Q u a l i e d ophthalmologists working or practicing out of Chhattisgarh or India may become life member. They will not be allowed to contest for any post in the society.

Ÿ Honorary member– Any person who has rendered outstanding service to ophthalmology in general and Indian ophthalmology in particular, may be elected as a Honorary member, provided his name is unanimously recommended by the Executive committee and passed by 2/3rd majority at meeting of the General Body. He shall be entitled to participate in all the activities of the society but shall not be entitled to vote at the meeting for the ofce in the society.

Ÿ The Society will keep a membership register accordingly.

(B) SUBSCRIPTION The life subscription for life members. National and international

members will be as stipulated in the by – laws and decided by the General Body from time to time. (At present life member fee is Rs. 3000/-, for national member fee is Rs. 5000/- and international life member free is $ 100. (with Effect from 31-12-2014)

ARTICLE 6. PROCEDURE AND PRIVILEGES OF MEMBERSHIPŸ All life members shall be entitled to participate in all the activities

of the society including the right to vote and shall also be entitled to receive publications of the society as provided in the by-laws.

Ÿ The application for membership duly proposed and seconded by members of the association shall be recommended by the Executive Committee and ratied by the General Body for admission to the society.

Ÿ Any new member whose application is under consideration is entitled to attend and take part in the deliberations of the conference of society, if it is held prior to his membership being approved by the General Body but will not have the right to vote till his membership is duly ratied by the society.

Ÿ National/ international life members, and honorary member shell be entitled to participate in all the activities of the society, but shall not be entitled to vote at a meeting of the society or hold a position of ofce.

ARTICLE 7. :GENERAL ELIGIBILITY FOR MEMBERSHIPTo become a member of the society, person should be-Ÿ Above 18 years.Ÿ Should be Indian.Ÿ Willingness to follow the rules and regulations of the society. Ÿ Should be of good character and should be non- alcoholic.

ARTICLE 8 . : CESSATION OF MEMBERSHIPIt will take place on-Ÿ Death of member Ÿ InsanityŸ Resignation when acceptedŸ Charges of unethical practice or an act against moral and social

character or nancial misappropriation, which is decided by General Body and the decisions intimated to the members in writing.

Ÿ Cancellation of registration by Indian Medical Council.

ARTICLE 9. :MEMBERSHIP REGISTERA member register will be kept in society's ofce in which following details will be kept.Ÿ Name, address and professional information.Ÿ Date of application for membership, ratication with receipt No. Ÿ Date of cessation of membership.Ÿ Signature of members

ARTICLE 10. : (A) GENERAL BODYŸ Life members shall constitute the General Body of the society.Ÿ It shall govern the affairs of the society through its elected ofce-

bearers and committees. Ÿ It shall hold a General Body meeting during the conference to be

held at least once a year. Ÿ Minimum of 3/5th of total member shall form the quorum.Ÿ In case the quorum is not present at the start of the meeting or

during the meeting of the ordinary General Body meeting, the meeting shall adjourn for at least 30 minutes and shall meet on the same day at the same venue and will not require any quorum.

Ÿ Special General Body meeting will have to be called if minimum 2/3rd of total members request so in writing. The Registrar will have the right to issue directive and give advice in such case.

Ÿ If the General Body meeting is not called by the ofce bearers as prescribed. The registrar may call meeting and x the agenda

(B) Committees : the following shall be the statutory committees elected by the society.Ÿ Executive committee Ÿ Scientic committeeŸ Editorial committee – Journal

ARTICLE 11. : FUNCTION AND POWERS OF GENERAL BODY· To adopt last year Annual report.· Management of society's assets and working · Appointment of an auditor. · To consider all matters presented by the Executive body. · Approval of budget and nancial report of various Committees.

A R T I C L E 1 2 . : C O M P O S I T I O N O F E X E C U T I V E BODY/COMMITTEE.It shall consist of Ÿ PresidentŸ President ElectŸ Vice PresidentŸ General SecretaryŸ TreasurerŸ Joint SecretaryŸ Editor of the JournalŸ Chairman Scientic CommitteeŸ Representative for managing committee of A.I.O.S.-twoŸ Members- Two from each divisional society, one of them must be

the secretary of D.O.S.Ÿ Two nominated members by the presidentŸ Three Ex-ofcio members-Ÿ Immediate past presidentŸ Immediate past SecretaryŸ Immediate past Treasurer.

ARTICLE 13. : TENURE of the EXECUTIVE BODY/COMMITTEE The affair of the society shall be managed by the Executive Committee, which will be elected by the General Body once in three years (except president and president- Elect). The vice president will be elected every year.

ARTICLE 14. : (I) FUNCTIONS AND POWERS of EXECUTIVE BODY/COMMITTEE

Ÿ Executive Committee shall act on behalf of the General Body for running the affairs of the society under its constitution and bye-laws.

Ÿ It shall review the recommendations of the various statutory Committees.

Ÿ It shall carry out the policies and mandate of the society and shall be responsible to it.

Ÿ It shall have power to invest and deal with the money of the society and to borrow such sums of money as it may resolve for the benet of the society.

Ÿ The Executive Committees may constitute as many sub- Committees as it deems t for furtherance of objects of the society. These committees may lay down their own procedures of working.

Ÿ Decisions and recommendations of the Executive Committee shall be subjected to approval of the General Body.

(II) MEETING The members of the Executive committee shall meet as often as required and at least once in every three months annually half of the total number of members of the Executive Committee shall form a quorum.Ÿ The secretary shall circulate the agenda for the meeting at least a

month before the date of the meeting.Ÿ The president shall preside at the meeting of the Executive

committee.Ÿ In his absence, the vice president, will preside. In the absence of

the President, and the Vice President. Vice president, the meeting he may elect its own Chairman, and for the purpose of the meeting he shall exercise all the powers of the president.

Ÿ The President or Chairman, in the event of a tie, have a casting vote.

Ÿ The voting may be by show of hands, or by ballot whichever is considered desirable by the president.

Ÿ Whenever it is found inconvenient to call a meeting of the EXECUTIVE BODY/COMMITTEE and whenever the secretary

in consultation with the president deems it desirable that the opinion of the EXECUTIVE BODY/COMMITTEE is to be obtained he shall do so by a latter through registered post /or by hand/courier .

III. Scientic Committee:Ÿ Commposition :The scientic Committee will be elected ones in three years and shall consist of :Ÿ ChairmanŸ ·Three members, nominated by chairman Ÿ Ex-ofcio members ( entitled to vote)Ÿ PresidentŸ SecretaryŸ Editor JournalŸ Immediate Past-Chairman scientic committee

2. Functions : It shall assist the society in formulating It's scientic programme:Ÿ It shall recommend the subject and name of the convector for the

annual symposium/updates etc.Ÿ It shall undertake other similar tasks assigned to it by the society

from time to time, as screening and regulating the entry of scientic papers.

Ÿ The chairman of the scientic committee will also act as its convener.

iv. Journal of the societyŸ The society will publish a journal.Ÿ An editor will be elected once in three years and shall constitute an

editorial committee which shall comprise of Ÿ EditorŸ Managing EditorŸ Three MembersŸ Ex-Ofcio members (entitled to vote)

Ÿ PresidentŸ SecretaryŸ Chairman – Scientic Committee

Article 15 : ELECTION AND TENURE OF OFFICE BEARERS AND MEMBERS OF THE STATUTORY COMMITTEES The society shall elect the following ofce-bearers :Ÿ The Vice – President shall be elected by the general body once in a

year at its annual conference. He body once the President elect next year.

Ÿ The Secretary/Treasurers. Other ofce bearers, chairman and members of different committees will be elected at the meeting of general body for a period of 3 years or at the end of the 3rd annual conference, whichever it longer, at the end of which period they will be eligible for reelection or not more than one more consecutive term.

Ÿ In case there is an election it will be by secret bellow as per condition and procedures laid down in the byelaws.

Ÿ The President - Elect of the previous year will become president after the presidential insignia is handed over at the inaugural/installation meeting of the next annual conference.

Ÿ Vacancies : The following succession shall be effective – Ÿ President will be succeeded by President – Elect.Ÿ President elect will be succeeded by Vice-President.Ÿ Secretary will be succeeded by Jt. Secretary .· Other ofce bearer and chairman of committee and members will be nominated by the president subject to ratication by the executive committee and shall hold ofce for the residual term.Ÿ Ofce bearers and committee members shall cease to hold ofce.Ÿ When the period of their tenure is over.Ÿ If there a resolution, the general body meeting, calls for the

purpose decides by a three fty majority of the members present that certain ofce bearers be remove from ofce.

Ÿ By resignation.

ARTICLE 16 : FUNCTIONS & POWERS OF HE PRESIDENT : The president will act on behalf of the society and will represent the proceedings of the society and the executive committee. The president shall uphold the constitution and enforce the rules and regulations framed there under. He shall preside over all the functions of the society and meetings of the general body and executive committee.

ARTICLE 17 : FUNCTIONS & POWERS OF VICE-PRESIDENT : In the absence of the president, the vice-president shall ofciate for him.

ARTICLE 18 : FUNCTIONS & POWERS OF THE SECRETARY GENRAL Ÿ The secretary shall manage all correspondence and shall keep the

minutes of the society and committee meeting which will be read and conrmed at the following meetings respectively. He shall have charge of all records of the ofce and keep a register of all papers communicated to him. He shall be responsible to the society for carrying out the resolutions of the society. All records shall be open to inspection by any member of the Society.

Ÿ All properties and documents of the society shall be under the custody of secretary.

Ÿ The secretary may incur an expenditure in unforeseen circumstances not providing in the budget up to Rs. 1000/- with the written permission of the president .

Ÿ The secretary shall receive money due to the society and shall make all payments in accordance with an rules framed by the executive committee and every receipt given shall be signed by him . he will prepare a statement regarding the nancial position of the society joint with president which will be audited by the certied auditors and present the same at the annual meeting of the general body for adoption.

ARTICLE 19 : FUNCTIONS AND POWER OF THE TREASURERThe account of society's income and expenditure will be kept by the treasurer. He will make payments for the expenses approved by

secretary or executive body. He will receive money on behalf of the society and issue receipt.

ARTICLE 20 : Ÿ Bank AccountThe society's account will be kept in nationalized bank the account s h a l l b e o p e r a t e d j o i n t l y b y – a n y t w o o f – President/Secretary/Treasurer one of them shall be Treasurer.Ÿ Finance :Ÿ The nance of the society shall be kept under Ÿ General Account Ÿ Reserve FundŸ Journal Account Ÿ Scientic Committee AccountŸ General Account : It will include establishment account, life membership account and award account . All these accounts shall be maintained by the secretary/Treasurer under separate heads.Ÿ The General account will include all receipt and expenditure by

way of -Ÿ Interest on Life subscription.Ÿ Advertisement charges (other than for journal and scientic

Committee)Ÿ The account may also include any surplus of decit generated

from meeting, conferences, symposium, etc. hosted by Chhattisgarh state ophthalmological society.

Ÿ Sale of any property belong to the society. Ÿ Payments made towards holding of conferences, workshops.

Instruction courses, printing, stationery and postage, etc. for efcient working of the society and ofce.

(b) Reserve fund: I. All life membership subscriptions shall go to the reserve fund, only the interest from this will be transferred to the General fund. ii. Any other income or unconditional donations may be added to this fund at the discretion of the Executive Committee With the

approval of the General Body. iii. Donations towards awards. iv. Afliation fees.

(c) JOURNAL Account This account will be in connection with-Ÿ Donations received from time to time towards the journal.Ÿ Advertisements to the journal. Ÿ 25% Of the interest of life member's subscription. Ÿ 25% Of the share of the Society from trade stall charges. This account shall be maintained and operated by the Editing of the journal & who shall present the A/C to Ex- com General Body meeting.(d) Scientic Committee Account : The account Will be in connecting With – Ÿ Donations as may be received by the committee form time to time

deposited with the secretary for the sciences and any other specic purpose.

Ÿ 25% of the interest of the life member's subscription.Ÿ Advertisements collected by the scientic committee.Ÿ 25% of share of the society from trade stall charges. This account will be maintained and operated by the chairman scientic committee.Ÿ Any person or persons in whom for the time being of the any

funds for money or any property movable or immovable of the society may be vested, shall be answerable for any loss arising in the administration on application of the said fund or sums of money or for any damage to or deterioration shall happen by or through his or their willfull default or neglect as determined by the executive committee or its authorized sub-committee.

Ÿ The persons in charge of the various accounts shall prepare and submit the audit statements of income and expenditure of their respective account.

Ÿ They shall submit the balance sheet and the draft budget to the executive committee/General body and shall be publish in Journal.

ARTICLE 21 : INFORMATION TO REGISTRAR Within 45 days of annual general body meeting, the information of executive body will be led in prescribed format as per section 27. An annual audited statement of accounts will be submitted with due fees as per section 28 of the registration Act.

ARTICLE 22 : CONFERENCESŸ The society shall organize conference/conferences at least ones a

year.Ÿ The venue of the conferences shall be recommended by the

executive committee and approved by General body. In unforeseen situations, the president will take a decision in consultation with the executive committee.

Ÿ At least 2 months before the date of the conference, a preliminary notice will be sent to every member by the secretary.

Ÿ The conference shall be organized on behalf of Chhattisgarh state ophthalmology society by a local committee jointly with the executive committee.

Ÿ The organizers of the conference will pay 50% of the stall charges collected from commercial exhibition to the society.

Ÿ Audited statement of the account of the conference shall be submitted by the organizing secretary to the society within three months from the last day of conference.

ARTICLE 23 : PROPERTY AND ASSETTS of the society will be kept in the name of this society only. Permanent property may only be transferred with permission of the registrar and on payment of prescribed fees.

ARTICLE 24 : BYE-LAWS The society under its constitution shall frame byelaws for smooth functioning of the affairs of the society. If anything in these byelaws is in contravention or in conation with the constitution, the provisions in constitution will have the supremacy .

ARTCLE 25 : AMENDMENT TO THE CONSTITUTION AND BYELAWS Ÿ The executive committee may recommend any amendment,

addition, alteration or detection to the constitution provided they give at least 30 days clear notice with the details of the proposed amendment to the members of the society before the general body meeting called for that purpose.

Ÿ Any member may propose an amendment, addition, alteration or deletion to the byelaws giving a minimum of 60 days clear notice before the special general body meeting. The proposed amendment shall be submitted in writing and duly seconded and signed by ve other members of the society.

Ÿ Quorum for amendment of byelaws shall be 3/5of total members.Ÿ Any amendment, addition, alteration or deletion in the byelaws

shall be made if approved by simple majority of the members present at the general body meeting.

Ÿ The amendments will be done only with the kind consent & permission of registrar along with the appropriate amendment frees chargeable.

ARTICLE 26. : DISSOLUTION OF THE SOCIETY It upon dissolution of the society, there shall remain, after satisfaction of all its debts and liabilities, any property what so ever, the same shall not be paid to or distributed among members of the society or any of them. It shall be given or transferred to society or societies, institution or institutes having objects similar to any of objects of the society to be determined by the votes of not less than three- fths of the members present personally or by proxy in a meeting at or before the time of the dissolution or in default by the Hon' High court. This will be done as per Registrar Act.

ARTICLE 27. : DISPUTESThe president has a right to settle dispute with the permission of General Body. He may refer the matter to the Registrar, who will have the nal verdict.

Inaugural session of Annual Conference Inaugural session will be presided by the President. President elect, Vice President, General Secretary, Treasurer, Jt. Secretary, Chairman Scientic Committee and Editor Journal will be on dais. Organizing Chairman, Organizing Secretary and Chairman Reception Committee will be on dias along with the guests, decided by organizing Committee. The President will make a policy speech, General Secretary will highlight the activities; organizational and nancial matters will not be discussed during inauguring session, when guests are invited. President-Elect will be installed in the session and will deliver a speech.Annual General Body Meeting-To be presided by the President, The Vice president, The General Secretary, Treasurer, Jt. in conict with the constitution of C.G.S.O.S. in the absence of their own constitution and bylaws, the constitution & bylaws of C.G.S.O.S. will apply.

Ÿ Each afliated society shall pay one time afliate fee of Rs. 2000/- for life time.

Ÿ Each afliated society and other societies must have at least 5 members on its rolls who are members of C.S.O.S.

Ÿ These societies shall be autonomous so far as the internal management and working are concerned.

Ÿ Repost of the activities of the societies may be published in the journal of C.G.S.O.S.

ARTICLE 30: ELEGIBILITY FOR VARIOUS OFFICES The prescribed period of membership shall be countable from inception of the society. Initially, all members since inception will be eligible for all posts.Ÿ Past presidents will not contest any election but they may be

nominated or co- opted in any Committee.Ÿ No. member shall hold more than one ofce and one Committee

membership or more than two Committee memberships at the same time

Ÿ Vice President:-Ÿ Must have been a member for a minimum period of 5 years.Ÿ Must have held an organizational position either as an ofce

bearer or a member of Executive Committee for a minimum period of one term and attended at least two such meetings.

Ÿ Should have actively participated in the scientic organizational activities of the society.

ARTICLE 31: CO- OPTION OF COMMITTEE MEMBERS Chairman of various Committees can co- opt more members in consultation with the Executive Committee.

ARTICLE 32: ELECTION PROCEDUREŸ Secretary shall invite nomination for elections for various posts,

three months before the conference date along with preliminary notice of the conference. The proposals must be received on or before a data specied by him/ her.

Ÿ Secretary should keep everything ready so that elections, it necessary, can be completed expeditiously.

Ÿ ·Only life members will be allowed to vote. Ÿ President will constitute an Election Commission consisting of 3

senior members with one of them as chief election ofcer who will also be the returning ofcer. An aggrieved candidate may represent to the election commission. The representation will be considered by the full commission and shall be nal and irrevocable.

Ÿ In case of a tie the casting vote of the President of the meeting will decide the result.
