President's Desk...
Dear Friends,
I am happy to see the new edition of Critical Care
Communications, congratulations to its editorial team! They have
been coming out with excellent publications in a timely and
consistent manner.
On the academic front ISCCM is doing very well. We have begun
INDICAP again and a large number of ICU’s are participating, thanks
to the senior’s encouragement and support and Dr. Divatia’s
tireless efforts. I am sure that at the end of it we should be able
to come out with a lot of Indian, worthwhile, publishable data
which may ultimately affect our daily practice of critical care.
The research committee is doing very well under the leadership of
Dr. Subhash Todi, many guidelines and ISCCM supported research
project are underway. The Journal is doing extremely well, thanks
to Dr. R. K. Mani and his editorial team. Unfortunately both the
chief editors and publisher are ending their productive terms but I
am sure that it will continue on the upward path.
Elections were held with a lot of excitement and enthusiasm,
under the vigilant eye of CEC Dr. Subhal Dixit and team. I
congratulate the newly elected members and request all those who
could not make it this time to continue their support and
participation in ISCCM activities. “Failure is a stepping stone to
future success (Napoleon Bonaparte)”. I few hiccups were addressed
by the grievance redressal committee. I am going to constitute a
small committee to further strengthen the election process and
bring about a few constitutional changes. World sepsis day is on
13th and we all should try to promote awareness about sepsis in the
general public.
Also ISCCM day is nearing. This year the theme is ‘Clean Hands –
Safe Hands’ practice hand hygiene. All branches should participate
actively to promote this.
Finally 25th Anniversary Mumbai conference preparation are on
full swing. Most of the major requirements are in place and we look
forward to a memorable meeting in ‘Amchi Mumbai’! Long Live
ISCCM.
The natural calamity in Kerala shocked us all.
We stand with our friends in this hour of need.
ISCCM has released INR 6 Lakhs as a token for relief work.
Dr. Rajesh Mishra has worked hard in his capacity to send the
required medicines to Kerala.

Dear ISCCM Members,
Half of this year is gone and we are proceeding fast towards our
Silver Jubilee Conference. In last six months the most important
event which has happened is the ISCCM Election. I am proud to share
that six thousand members voted this year, which is almost double
than that of previous year. This shows that the members involvement
in the society affairs is increasing, which I was trying hard for
last couple of years. I wish this enthusiasm will be
maintained.
Another important event which is coming very soon is ISCCM day
Celebration in the month of October, this time our slogan for
celebration is “Hand Hygiene”. I wish similar enthusiasm with more
and more participation in this ISCCM Day celebration. As a
secretary I will finish my tenure next year. I have always have
dreamed to make our society a fully transparent and democratic. For
which everyone’s involvement is important.
Many a times we have conflict, that means that there are truths
that needs to be addressed on each side, and the best way to
resolve it, is dialogue with more and more discussion to understand
each other. The most important gateway which we have opened for the
members to get involved is the BMS (Branch Management System)
through which every member of the society can connect himself to
the centre anywhere anytime. So, come forward to share your hands,
and make this society as one of the best in our country. We need to
set an example for the entire world.

4th Annual South Zone Conference of ISCCM was held in Mysore
between 30th August to 2nd September. Workshops
were on the first two days, which included Mechanical
ventilation including ECMO, Antibiotic stewardship, Hemodynamic
monitoring, Critical care ultrasound, & Critical care
nursing. There were 120 faculties in total. An overwhelming
response
witnessed about 780 delegates participating actively in the
various sessions. Inauguration was done by Her excellency
Rajmata Pramoda Devi Wadiyar. A souvenir was released in
connection with the meet.Monetory contributions from various
ISCCM branches for flood hit Kerala & Kodagu were handed
over to the respective teams. It was also decided to form a
research committee to encourage & guide critical care
research activities in this region.

MONTHLY MEET ISCCM DELHICritical care medicine is one of the
fastest-growing hospital specialties and, looking back, it is clear
just how far we
have come in such a relatively short period of time. With the
ICU set to occupy an increasingly important place in hospitals
worldwide, we must learn from the past and wisely embrace new
developments in technology, therapeutics, and process,
to ensure that the goals of critical care medicine are met in
the future. Taking care of these goals SCCM Delhi conducts
monthly scientific critical care meet at various hospitals of
Delhi and NCR.
This monthly meet was held in association with Sri Balaji Action
Medical Institute on 28 th July at the conference
hall of their hospital premises. Around 50 doctors from the
various hospitals across Delhi had participated and attended
the sessions. The theme of the scientific meet was to discuss
about the broad aspects and new developments in the field
of critical care medicine. The scientific meeting was
orchestrated by Dr Rajat Gupta HOD Critical care Sri Balaji
Action
Medical Institute and his team. Eminent doctors from various
hospitals joined as faculty, speakers to share their experience
on various topics. The meet included five very important
sessions pertaining to daily practices of critical care.
Dr Tariq Ali, Director critical care, Medanta hospital Gurgaon
shared his elaborative views on Brain death and organ
donation and was appreciated a lot. There were two other major
topics like Antibiotic resistance breakers and role of
CRRT in critical care. Dr Ajay Gupta crical care coordinator
,Max Shalimar Bagh spoke on antibiotic resistance breakers and
highlighted their importance in today's world of deadly
resistent bugs.Dr Rajesh Aggarwal Senior Consultant &
Transplant
coordinator Balaji hospital described the importance of early
CRRT in rhabdomyolysis, tumor lysis syndrome , burns and
patients with early ARDS related to septic shock.Few more topics
were also discussed in the meet by the doctors and
included interesting case of hypokalemia presenting as
respiratory failure and journal scan which comprised the latest
articles. All the sessions were quite interactive and
informative.

Purpose : A rapidly evolving resuscitation science provides more
effective treatments to an aging population with multiple
comorbidities. Concurrently, emergency care has become
patient-centred. This review aims to describe challenges associated
with the application of key principles of bioethics in
resuscitation and post-resuscitation care; propose actions to
address these challenges; and highlight the need for evidence-based
ethics and consensus on ethical principles interpretation.
Methods: Following agreement on the article’s outline, subgroups
of 2–3 authors provided narrative reviews of ethical issues
concerning autonomy and honesty, beneficence/nonmaleficence and
dignity, justice, specific practices /circumstances such as family
presence during resuscitation, and emergency research. Proposals
for addressing ethical challenges were also offered.
Results: Respect for patient autonomy can be realized through
honest provision of information, shared decision making, and
advance directives / care planning. Essential prerequisites
comprise public and specific healthcare professionals’ education,
appropriate regulatory provisions, and allocation of adequate
resources. Regarding beneficence/ nonmaleficence, resuscitation
should benefit patients, while avoiding harm from futile
interventions; pertinent practice should be based on neurological
prognostication and patient/family-reported outcomes. Regarding
dignity,
aggressive life-sustaining treatments against patient’s
preferences should be avoided. Contrary to the principle of
justice, resuscitation quality may be affected by race/income
status, age, ethnicity, comorbidity, and location (urban versus
rural or country-specific/region-specific). Current evidence
supports family presence during resuscitation. Regarding emergency
research, autonomy should be respected without hindering scientific
progress; furthermore, transparency of research conduct should be
promoted and funding increased.
Conclusions: Major ethical challenges in resuscitation science
need to be addressed through complex / resource demanding
interventions. Such actions require support by ongoing/future
research.
Reviewer’s Comments
There are approximately 300,000 sudden death annually in United
States and the numbers are may be much higher in India. Major
ethical challenges (who should receive CPR, when to stop CPR,
special clinical status e.g. traumatic brain injury, legal
implications, access to best quality care, dignity, risk to health
care provider, optimizing end-of life treatment, do not
resuscitate) are present in present era science since the focus has
shifted from doctor centred approach to patient centred approach.
There are also ethical challenges in certain clinical situations or
circumstances e.g. organ donation, health provider safety etc. The
quality of care has improved as there is surge in insurance sector.
The purpose of the current narrative review was to (1) describe
current and emerging challenges associated with ethical principles
in resuscitation and further supportive critical care (2) proposing
actions, and initiatives to address these challenges and (3)
highlighting the need for research evidence-based ethics and
international consensus on the perception of ethical principles in
relation to resuscitation. Ethical issues in health care are
autonomy, honesty, beneficence, non-maleficence, dignity, and
justice. We have to respect/safeguard autonomy of incapacitated
cardiac arrest patient, presenting information to family for
making decision, benefitting without harming, providing high
quality care during and after resuscitation, successful
implementation of advance directives, family in specific clinical
setting or circumstances, and research issues to be discussed in
advance. Possible barrier in ethical issues are culture, region,
country, resource limitations socio-economic status, family
circumstances, disease state, issue of quality of life, conflict of
interest, and legal complications. Advent of ECMO-CPR has also
raised ethical questions and need attention. Proxy decision during
CPR is also a matter of debate. All issues need to be addressed
ethically and professionally. All medical graduates should be
trained in ethical principles in health care. As such there are no
definite guidelines in many countries but transparency should be
maintained during resuscitation. Also, there is insufficient
evidence for or against specific interventions due to the
uncertainty of determining a prognosis and predicting a particular
outcome. Understanding of the ethical principles surrounding
autonomy and decision making must be coupled with the best
information available in current situation. Our efforts should
include all the latest information available in this regard though
some countries have developed No-CPR strategies in terminally sick
patients.

there has been a focus on the early identification and treatment
of sepsis primarily with bundled and goal directed therapy. Despite
these advances, morbidity and mortality has remained high,
prompting investigation into novel therapies. Vitamin C is a
water-soluble vitamin that plays a role in mediating inflammation
through antioxidant activities and is also important in the
synthesis of cortisol, catecholamines, and vasopressin, which are
key mediators in the disease process. Emerging evidence provides
cursory data in support of the administration of vitamin C in
addition to standard therapy to ameliorate the effects of
inflammation and improve hemodynamic stability in patients with
sepsis and septic shock; however, further evidence is needed to
support this practice. This review discusses the physiologic role
of vitamin C as well as the recent literature and evidence for the
use of vitamin C in patients presenting with sepsis.
Reviewer’s Comments
Sepsis is a leading cause of morbidity and mortality in all age
groups and subject can present from sepsis to multiorgan failure
and septic shock. Various pro-inflammatory mediators play crucial
role in sepsis. Mainstay of sepsis treatment includes
identification of infection, source control, antibiotics
administration, hemodynamic stability through fluid and
vasopressors, and support to various organs of body. Microvascular
dysfunction seems to be important cause of mortality in sepsis.
Current sepsis therapy does not target inflammatory and oxidative
stress caused by sepsis and may further lead to worsening due to
further release of inflammatory mediators as a result of standard
therapy. Many sepsis adjuvant therapies like activated protein C,
interleukin receptor antagonist, anti-tumour necrosis factor
monoclonal antibody, toll-like receptor antagonist etc. have been
tried with success and failure but still no ideal adjuvant therapy
or modalities are available. Current rationale is that vitamin C
attenuates the inflammatory and coagulation pathways in sepsis and
occurrence of end-organ dysfunction can be reduced thereby
improving morbidity and mortality. Vitamin C acts as antioxidant,
promotes vasopressor synthesis, implicated in several immune
responses and functions, and
has several physiological functions in human body. The use of
vitamin C in sepsis was pioneered by Dr. Alpha Fowler at Virginia
Commonwealth University. Zabet et al (2016) in a randomized 24
post-surgical patients with septic shock studied role of vitamin C
infusion or placebo; the vitamin-C treated patients had
significantly lower mortality and need for vasopressors. Dr Marik
and his colleagues (Emergency Medicine July 2017) also compared the
outcomes and clinical course for 47 consecutive sepsis or septic
shock patients treated normally with the addition of intravenous
vitamin C (1.5gm IV every 6hrs), hydrocortisone (50mg every 6hrs),
and thiamine (200mg every 12hrs) four days over a seven-month
period and 47 patients as control with similar characteristics who
were treated in the intensive care unit in the seven months prior
and observed a dramatic improvement. Hospital mortality was 4 of 47
(8.5%) in those treated with the cocktail, compared to 19 of 47

Owing to broad spectrum activity β-lactam antibiotics has been
cornerstone of treatment in critical care. Pharmacodynamic efficacy
of antibiotic may be time dependent, or concentration dependent or
concentration dependent with time dependence. β-lactam antibiotics
display concentration-independent killing of bacteria. Septic
patients have altered/low pharmacokinetics (PK) and
pharmacodynamics (PD) due altered renal clearance, increased volume
of distribution, impaired target tissue penetration, altered
protein binding and longer exposure to antibiotics (MIC) may be
helpful in providing better maximum inhibitory concentration.
β-Lactams are the first line of choice in sepsis and their PK/PD
index is disturbed in sepsis. PK/PD indices are best predictor of
antibiotic efficacy. Prolonged infusion of β-lactam antibiotics has
shown improved PK/PD target attainment. An extended infusion (EI)
is defined as an infusion lasting 3 to 4 hours,
whereas continuous infusion (CI) is an uninterrupted infusion
lasting up to 24 hours at a constant rate and intermittent infusion
refers to the administration over a shorter time, usually 30 to 60
minutes. This study demonstrated reduced ventilator days, length of
stay, but no benefit in overall mortality for patients who received
EIM. Findings of present study suggest the benefit of this EIM
protocol is not limited to patients with GN organisms with higher
MICs but may also extend to critically ill patients with altered
PK/PD parameters due to severity of illness. Rebekka J. Dow (2011)
also concluded that prolonged infusion of meropenem and
piperacillin will improve outcome in critically. Clarence Chant
(2013) in systematic review and meta-analysis in pooled results
from small RCTs suggest reduced clinical failure rates and
intensive care unit length-of-stay when using continuous/extended
infusions of antibiotics in critically ill patients. Reduced
mortality rates almost achieved statistical significance when
the results of RCTs were combined with cohort studies. Michael
osthoff et al. in 2016 draw same conclusion. Hui Yang et al in 2016
in systematic review and meta-analysis found that extended or
continuous infusion should be recommended for clinical use in
severely ill patients. Meta-analysis of Individual Patient Data
from Randomized Trials compared with intermittent dosing,
administration of β-lactam antibiotics by continuous infusion in
critically ill patients with severe sepsis is associated with
decreased hospital mortality. In a prospective randomized pilot
study by Hui-Ying Zhao et al. (2017) it was found that continuous
infusion of meropenem provides significantly shorter treatment
duration and a tendency for superior bacteriological efficacy than
intermittent adminis- tration. Thus, extended or continuous
infusion dosing regimen of meropenem has scientific basis in vitro
and vivo in animal and human trials.
