日本版敗血症診療ガイドライン 2016
CQ15:体温管理
体温は,全身状態を把握するうえで重要な指標であ
る。低体温あるいは発熱を契機に新たな診療が開始さ
れることは稀ではない 1)。体温は測定部位により正確
性が異なるため,可能な限り信頼度の高い部位で測定
する必要がある。脳・肺・心臓・肝臓・腎臓など主要
臓器の温度が生体活動には重要であり,深部体温測定
が推奨される。現在のところ,深部体温のゴールドス
タンダードは血液温度であるとされているが,血液温
度測定には肺動脈カテーテルなどの挿入が必要であ
り,日常的には測定できない。American College ofCritical Care Medicine と Infectious Diseases Society ofAmerica のガイドラインでは,血液温度・膀胱温度・食道温度・直腸温度が深部温度をより正確に反映する

ることが知られている。35℃以下の低体温では凝固機能が低下し,33℃以下では血小板数の低下が生じ
る 18), 19)。
体温管理の項における 2 つ目の CQ は,“低体温の
敗血症患者を復温させるか?”である。低体温の敗血
症患者に対する復温が,患者予後に与える影響を検討
した研究は未だない。また,低体温患者をそのまま自
然経過に任せる群と積極的に復温させる群に分け,比
較検討するような介入試験は倫理的に実行が困難と考
えられる。低体温からの復温の際には,血圧低下・循
環血液量の相対的減少などにより,循環動態が不安定
化する可能性があることを十分留意する必要がある。
したがって,低体温そのものの副作用と復温の危険性
を考慮して判断する必要がある。復温を試みる際には,
体外循環,受動的保温,ブランケットなどで緩徐に復
温するべきである。
