COVID-19 Vaccine – PreCheck-In

This tip sheet contains instructions to complete their the PreCheck-In from either the LiveWell app on a mobile device or from MyAdvocateAurora on a desktop. IMPORTANT: PreCheck-In is available 3 days prior to your appointment and is required to be completed prior to your appointment. If you don’t complete PreCheck-In, you will need to reschedule your appointment. Tip: During the PreCheck-In process, you will be asked to enter your Password. We recommend that you know your password or have it available. To streamline the process, download the LiveWell app and have a MyAdvocateAurora account set up. • To get the LiveWell with AdvocateAurora app, download it to your smartphone from the App Store or Google Play Store.

• To get a MyAdvocateAurora account, create one now.

• To reset your MyAdvocateAurora password, click Forgot Password? on the sign in screen. If you need

PreCheck-In using the LiveWell App (same steps for Android or Apple)

An email will be sent the email address associated with your MyAdvocateAurora account. If you’re not sure which email that is, follow these steps to find this information:

1. Sign in to LiveWell 2. On the Home screen, select My

Chart 3. On the My Chart screen, select the

3 dots in the upper right 4. Select Account Settings 5. On the Account Settings page, you

will see the Email address that is linked to your MyAdvocateAurora account

1. When PreCheck-In is available, select Upcoming Appointment. Then select PreCheck-In button. Alternatively, an “!” badge will be displayed on the Appointments icon on the My Chart screen. Select Appointments.




2. On the Personal Info screen review the information and if it’s correct select This information is correct. If the information isn’t correct, select Edit to update




3. On the Sign Documents screen, select Review and Sign all required documents Note: There may be multiple documents required to review and sign Tip: For each document, you will be asked to enter your password

4. Once all documents are reviewed and signed, select Next




5. On the Insurance scree review the information and if it’s correct select This information is correct. If you’d like to add an insurance to your file, select Add a Coverage Note: If you have Medicare as your insurance, you may have to answer additional questions regarding your coverage.

You have completed the PreCheck-In process The day of your appointment:

• Be prompt and plan extra time for screening and wayfinding. Visit the vaccine clinic location information at aah.org/vaccine-locations to find the room for your appointment. Some locations may ask you to enter through the team member entrance. Stop by the information desk if you need help finding the vaccine clinic.

• Bring your employee badge with you.

• Wear a mask and practice social distancing.

• Allow for at least 30 minutes for your appointment from start to finish. You’ll need to stay for 15-30 minutes after getting vaccinated.



PreCheck-In using MyAdvocateAurora

An email will be sent the email address associated with to your MyAdvocateAurora account. If you’re not sure which email that is, follow these steps to find this information:

1. Sign in to MyAdvocateAurora 2. Select Profile 3. Select Personal information

On the Personal Information screen, under Contact Information, you will see the Email address that is linked to your MyAdvocateAurora account




1. On the MyAdvocateAurora dashboard screen, select the PreCheck-In action box Tip: PreCheck-In can under the Visits tab under Appointments and Visits

2. On the Appointment Details screen, select PreCheck-In




3. On the Personal Info screen, review the information and if it’s correct check the box, This information is correct, then select Next. If the information isn’t correct, select Edit to update
