The incident
Whilst moving some pallets of oil, the operator was seriously injured when the forklift rolled onto it’s side.

Move pallets of oil from the top of old pallet racks and relocate some 20 metres to the new storage racks.

The operator carried the loads of drums at height across the yard to save some time.
On the third trip, as the operator swung into the new pallet rack with a load of drums, the forklift overbalanced and toppled over.
The operator was thrown from the forklift seat and was pinned under the forklift by the arm and leg
