Overseas Domestic Helper Comprehensive Cover for You and Your Family MemberPlatinum Maid Insurance Plan protects you as an employer against liabilities under the Employees' Compensation Ordinance and at Common Law in HKSAR for death, injury or illness sustained by your overseas domestic helper arising out of and in the course of employment. In addition, if unfortunately your family member aged 4 years or below su�ers from bodily injury caused by the intentional, malicious act of your overseas domestic helper, this Plan covers medical expenses for the injured child.
Protection for Your Overseas Domestic HelperThis Plan also covers medical and dental expenses incurred for your overseas domestic helper, including clinical, surgical, hospitalization and bone-setting expenses; as well as providing personal accident protection for your overseas domestic helper during rest days.
Free Extended Bene�tThis Plan covers your overseas domestic helper against medical expenses incurred due to heart disease or cancer.
Comprehensive Protection at Low Costs
You can enjoy one full year comprehensive protection at only HK$600∆ per year. To maximize your bene�ts, you can pay only HK$1,150∆ for a two-year policy.
Local Part Time Domestic Helper Employees’ Compensation Insurance Plan is also available for local part time domestic helper with an annual premium HK$300∆ per year and cover your employer’s liability under the Employees' Compensation Ordinance and at Common Law for death, injury or illness sustained by your domestic helper in the course of employment.
Major exclusion from insurance coverage (including but not limited to):- accidents caused by war- pre-existing conditions, pregnancy and related complications- HIV/AIDS - the in�uence of drink or drugs, venereal disease and suicide- claims for injury sustained or expenses incurred outside HKSAR

Platinum Maid Insurance Plan OCBC Wing Hang Insurance Agency Limited (“OCBC Wing Hang Insurance Agency”) is a wholly-owned subsidiary of OCBC Wing Hang Bank Limited (“OCBC Wing Hang”).
Platinum Maid Insurance Plan (“Plan”) is underwritten by Liberty International Insurance Limited (“Liberty International Insurance”) which is authorized and regulated by the Insurance Authority of the Hong Kong Special Administrative Region (“HKSAR”). Liberty International Insurance will be responsible for providing your insurance coverage and handling claims under your Plan. This Plan is a product of Liberty International Insurance but not OCBC Wing Hang Insurance Agency. 1. I understand that the Platinum Maid Insurance Plan (“this Plan”) is for the proposed domestic helper who is lawfully engaged for domestic duties in Hong Kong only. I agree that Liberty International Insurance Limited (“the Company”) shall not be liable under this Plan for any injury or illness that was su�ered by the proposed overseas domestic helper before the e�ective date of the relevant insurance policy. I understand that the Company shall not under this Plan be liable for the clinical, surgical, hospitalization and dental expenses and loss of service allowance incurred during the �rst 15 days from the e�ective date of the relevant insurance policy.
2. I declare that I have full and complete authority from the insured to sign this proposal form and disclose his/her personal information for the purpose of this insurance application. I understand that this proposal form and declaration will form the basis of the contract between me and the Company. I understand that the Company is responsible for all matters in relation to the insurance coverage and compensation under this Plan.
3. I have been authorized by the insured to allow the Company to obtain medical information from the insured's medical practitioner(s) and I agree to supply any information relevant to the relative insurance policy of this Plan at my own expense.
4. I have not withheld any material facts (i.e. facts relevant to an insurer's decision whether to provide coverage or not) from the Company and that if any material facts shall have been withheld or not truly or fairly stated, this insurance policy shall be null and voided.
5. I have read and understand the contents of this Plan and the insurance coverage, exclusion clauses and other relevant terms and conditions. If there is any inconsistency between the insurance policy and this lea�et (including this Declaration), the contents of the English version insurance policy shall prevail.
6. I hereby acknowledge the insurance agent has clearly explained that this is an appropriate plan with regard to my needs and has explained that the consequences of any fraud, non-disclosure and inaccuracies information provided by me. 7. This application is subject to the approval of the Company which shall, in its absolute discretion, determine whether to accept this application or not.
