What is Community Diabetes?Community Diabetes is a 3 hour face to face workshop for people who are newly diagnosed or at risk of Type 2 diabetes.

During this workshop, participants will learn about the small lifestyle changes that can lead to big health improvements and empower them to take action to prevent diabetes or diabetes related complications.

Specifically:

Diabetes and risk factors

Healthy eating and nutrition

How to be more active

Diabetes medications

Annual complications screenings

Workshop size is for 20 participants, including partners, carers and a support person.

The program has been translated into Mandarin, Cantonese and Arabic. Talk to us about other languages you may need.

Facilitator Training

eLearningBy completing Diabetes in Practice for Practice Nurses, participants update their existing knowledge so they can provide even better support to people diagnosed with type 2 diabetes. The eLearning modules cover best-practice and evidence-based guidelines for the management of diabetes. References and sources for further reading are also provided. It is delivered by Diabetes Qualified, a fully owned subsidiary of Diabetes NSW&ACT.

12 hours to complete - Earn 12 CPD hours.

The eLearning course needs to be completed prior to the face to face workshop to ensure participants have the base knowledge required. The face to face facilitator workshop is a full day session delivered by one of our trained Health Care Professionals. This session will ensure your team are comfortable with both the workshop content and the person-centred facilitation methods. If you have 7-10 participants we can run the program in-house for you, alternatively we offer this as a public program twice a year.

locally. The facilitator training involves:

Completion of an eLearning program – Diabetes in Practice for Practice Nurses One day face to face facilitator training

Face to Face Training

Investment: $950 per participant, for both eLearning and Face to Face Training
