Page 1 8/30/18 Create thriving block clubs North Hartf ord Triple Aim Collaborative Creating a Healthier Community Together North Hartford, CT’s designation as a federal Promise Zone (NHPZ) in 2015 mobilized a renewed spirit of collaboration among city leaders, local organizations and citizens on behalf of North Hartford’s 24,000 residents. The North Hartford Triple Aim Collaborative (NHTAC) was launched in 2017 as the Health and Wellbeing work group for the NHPZ. Building on the significant work to date, the Collaborative is currently expanding its reach and impact by harnessing the increasing convergence of partners and resources committed to creating a healthier, more vibrant community for all. Vision: Living our best lives in community. Purpose: Greater alignment and collective action among residents, organizations, government and investors working together to improve health, wellbeing, and value of investment. Guiding Principles 1. Improve outcomes through collaboration 2. Increase equity by strengthening community assets and leadership 3. Coordinate high-impact, mutually reinforcing activities focused on shared outcomes and measures 4. Create support structures that improve effectiveness, efficiency and sustainability 5. CollectiveActionThe10-yearfederalPromiseZonedesignationhascreatedasenseofurgencytoaddresssignificantinequitiesinhealthandwellbeinginNorthHartford.WithdirectionandfinancialsupportfromtheLeadershipCouncil,theCollaborativeisengagingresidents,community-basedorganizations,governmentandfundersto:• Connectresidentsto

BackboneSupportCoordinatingsupport(“backbone”)isco-fundedbySaintFrancisHospital’sWellBeing360initiativeandtheUnitedWayofCentralandNortheasternConnecticutandstaffedbytheNHTACDirector,withadvisorysupportfromWellville.
CreatingValueandSustainabilityWiththeCityofHartfordHealthandHumanServicesasthelead,theCollaborativewasrecentlyselectedasa“referencecommunity”fortheHealthEnhancementCommunity(HEC)Initiative,whichispartofConnecticut’sStateInnovationModel(SIM)aimedathealthcaredeliveryandfinancingreforms.TheCollaborativewillparticipateinthedevelopmentofnewcommunity-centeredmodelsthatencouragemulti-stakeholderpartnershipsacrossthestatetoworktogethertopreventdisease,improvehealth,andlowercosts.Thesenewmodelswillcreatepathstofinancialsustainability,potentiallythrough“sharedsavings”fromhealthinsurersandotherpayersthatrewardspecificoutcomes.

CurrentExamplesofActionPortfolioCollaborativemembersandpartnersarelaunchinganexpandingportfolioofactionstoachievethreecommunity-leveloutcomes:longerlife,greaterwellbeing,andimprovedvalueofinvestment(impactperdollarspent).Afewcurrentinitiativesinclude:
WellBeing360:Throughinvestmentsofmorethan$450,000since2016,thisTrinityHealth/SaintFrancisinitiativehashelpedlaunchtheNorthHartfordTripleAimCollaborative;startadiabetespreventionprogram(DPP)attheYMCAinHartford;improvehealthyfoodaccessthroughtheNorthEndFarmersMarket,Knox’surbanfarminginitiative,andtheCookingMattersnutritiontrainingprogram;reducegunviolencebyexpandingthecrisisresponseteamatHartfordCommunitiesThatCare;andpreventopioiddeathsbypartneringwithGreaterHartfordHarmReductionCoalitiontoincreaseaccesstonaloxone.
Inaddition,SaintFrancisisintegratingasocialdeterminantsofhealthassessmentandreferralprocessinitssafetynetclinicsincollaborationwithCatholicCharitiestohelppatientsimprovesocialfactorsneededforgoodhealthandwellbeing.StrengtheningFamilies:WithfundingfromtheHartfordFoundationforPublicGiving,theOfficeforCommunityChildHealthatConnecticutChildren’sMedicalCenterbegantrainingchildcareprovidersusingastrengths-basedapproachtosupportlocalfamilies.Thegoalistoimproveoutcomesforvulnerablechildrenandfamiliesbystrengtheningthefiveprotectivefactorsknowntoenhancefamilies’capacitytopromotetheirchildren’shealthydevelopment:parentalresilience,socialconnections,concretesupportintimesofneed,parents’knowledgeofchilddevelopment,andchildren’ssocialandemotionalcompetence.ParkerMemorial:Inaddition,theCollaborativehasbeenworkingaspartofacoalitionofresidents,cityagencies,community-basedorganizations,funders,healthsystemsandotherstodeterminehowtheNorthHartford-basedParkerMemorialmaybepositionedasthecoordinatingcenterofanintegrated,trauma-informedsystemtoimprovethelivesoftheentirecommunity.ThecoalitionincludestheCity’sDepartmentofFamilies,Children,YouthandRecreation;HealthandHumanServices;andPromiseZoneAdministrator;alongwithpartnerssuchasConnecticutChildren’sOfficeforCommunityChildHealth,HartfordFoundationforPublicGiving,TheVillageforFamiliesandChildren,andothers.IncludingstrongcommunityrepresentationandleadershipfromtheearlieststageswillensurethatParkerMemorialreflectstheuniqueneeds,strengthsandcultureofthepopulation,andthatthecenteroperateswithanemphasisoninvolvingandempoweringresidentstobuildfamilyresiliencyandwellbeing.

HealthyFoodAccess:Hartfordwasoneof50U.S.citiesselectedtoparticipateinInvestHealth,acollaborativeinitiativeoftheRobertWoodJohnsonFoundationandReinvestmentFund.In2016-2017,amulti-sectorteamledbytheCityofHartfordengagedresidentsandotherstakeholdersinacomprehensiveprocesstoassessthecurrentfoodsystemanddevelopinvestableopportunitiesforimprovement.Retailfoodaccesswasnamedasthetoppriority.Atthesametime,additionaleffortsledbyHartfordCommunityLoanfund(alocalCDFI),HartfordFoodSystem,andothershaveassessedthefeasibilityofananchorsupermarket.Amarketassessmentandresidentfood-purchasingstudyconfirmedtheviabilityofamid-sizedmarketwithafocusonhealthyandaffordablefoods,especiallyqualityfruitsandvegetables,withpricingcompetitiveforadiversepopulationofcityresidents.Withpre-developmentfundingfromtheHartfordFoundationforPublicGiving,continuedworkisnowunderwaytodevelopthe“HealthyHartfordHub”–aproposedmixed-usedevelopmentthataimstoincorporateafull-servicesupermarket,health-promotingservices(suchasaclinic),andpotentialhousingorofficespacetoserveresidentsofHartford,especiallythoseintheNorthEnd,Downtown,andAsylumHillneighborhoods.TheCollaborativewillworkwiththepartnersaboveandwithresidentinputthroughanHHHCommunityAdvisoryTaskForcetoidentifyanddeterminethefeasibilityofvarioushealthpromotingservicesthataimtoimproveresidentwellbeingandbeintegratedwiththenewsupermarket.CurrentExamplesofCapacityBuildingInadditiontotheseandotheractionstoimprovecommunityhealthandwellbeing,theCollaborativeisalsoco-investinginbackbonesupportandotherfunctionalareasneededforsuccess.Forexample:SharedDataandMeasurement:InpartnershipwithDataHaven,HartfordFoundationforPublicGivingandothers,theCollaborativeisdevelopingadataandmeasurementsystemtoestablishsharedgoalsandtotracktheongoingimpactofCollaborativeactionsthroughmetricsthatmattertocommunitymembersandotherstakeholders.DataHavenreceivedgrantsin2015and2018fromtheHartfordFoundationforPublicGiving,SaintFrancis,ConnecticutHealthFoundation,HartfordHealthCare,andotherstofieldthestatewideDataHavenCommunityWellbeingSurvey(in2015and2018).Currentworkisbeingdonetoproduceacomprehensiveregionalcommunityhealthandwell-
beingindexandindicatorsreporttentativelytitledtheGreaterHartfordCommunityWellbeingIndex.Plannedtoincludesignificantstakeholderinput,thecross-sector,collaborativereportisdesignedtohelpmeettheneedsofmultipleorganizationsthroughacoordinatedlocalsource.Stillindevelopment,thevision
