Northeast Georgia Medical Center (NGMC), with input from the community, completed a Community Health Needs Assessment (CHNA) in 2013. NGMC has conducted assessments in the past via Healthy Hall. Now nonprofit hospitals are required by law to complete an assessment, make the results available to the community and develop an implementation plan that the hospital will champion. NGMC's assessment includes a secondary data scan and a community health profile for Hall County. The assessment focuses mainly on the needs of the community's most vulnerable populations, particularly those with low-incomes who are uninsured. Six focus groups were held and nearly 25 one-on-one interviews were conducted with stakeholders. The study culminated in the identification of 10 priority health needs. On the following pages is a spreadsheet of initiatives and activities Northeast Georgia Medical Center is involved with which address those needs. Many activities overlap different priorities, and NGMC’s involvement ranges from providing the activity itself to contributing in some way. The spreadsheet is not an exhaustive list, but highlights many of the organization’s efforts to address identified community health needs. Results of the assessment, including NGMC’s implementation plan, will be posted to www.nghs.com by October 2013. Northeast Georgia Medical Center (NGMC), withinput from the community, completed a CommunityHealth Needs Assessment (CHNA) in 2013. NGMChas conducted assessments in the past via HealthyHall. Now nonprofit hospitals are required by law tocomplete an assessment, make the results availableto the community and develop an implementationplan that the hospital will champion.

NGMC's assessment includes a secondary datascan and a community health profile for Hall County.The assessment focuses mainly on the needs of thecommunity's most vulnerable populations,particularly those with low-incomes who areuninsured. Six focus groups were held and nearly25 one-on-one interviews were conducted withstakeholders. The study culminated in theidentification of 10 priority health needs.

On the following pages is a spreadsheet ofinitiatives and activities Northeast Georgia MedicalCenter is involved with which address those needs.Many activities overlap different priorities, andNGMC’s involvement ranges from providing theactivity itself to contributing in some way. Thespreadsheet is not an exhaustive list,but highlights many of the organization’s efforts toaddress identified community health needs.
