FAQ Answer

Does each user of the MHM Data Registry need to register for his or her own EIDM account? Will each user need to complete all steps for requesting access, or just Practice POCs?

Yes – each person who would like to access the MHM Data Registry will need to register for his or her own account by completing all steps of the process discussed in this webinar. The MH Model team will distribute information on how to request access to the MHM Data Registry via email in the next few weeks. These materials will provide guidance on how to complete all steps necessary for gaining access to the MHM Data Registry.

Will we be able to set up new EIDM accounts for new hires to our organization later in the MH Model?

Yes – you will be able to provide any new members of your organization instructions on how to request access to the MHM Data Registry at any point. Please look out for instructions that the MH Model Team will provide in the next few weeks and save the information for future reference.

If a member of our organization already has an EIDM account, does a new account need to be obtained for this model, or can the existing account be used to access the MHM Data Registry?

If you already have an EIDM account, you may use that same account to request access to the MHM Data Registry through the Innovation Center (IC) portal. Upon receiving further guidance on the access request process from CMS in the coming weeks, feel free to skip the upfront EIDM account registration portion. You will need to complete all subsequent steps associated with setting up your IC account and requesting access to the MHM Data Registry application.

Review of Why and How to Enter Beneficiary Data Throughout the Model

Submission of demographic and clinical data is required for key model activities including:

Beneficiary Enrollment: Inform validation of beneficiary eligibility and alignment.

Ongoing Submission Requirements: Submit beneficiary data annually to CMS.

Based on what works for your organization, you can submit data to the Registry in three ways:

Bulk Upload Using an EHR File

Bulk Upload Using a Data Template

Manual Data Entry

Submit data for one patient at a time by manually typing in each demographic and clinical value collected in an in-person visit.

Submit data for multiple beneficiaries and/or visits by populating a template provided by the Registry with data from your organization’s records.

Upload a CCDA file exported from your EHR that contains data for multiple beneficiaries and/or multiple visits.

Who at our organization is responsible for submitting data to the MH Data Registry?

This decision needs to be made by your organization based on which individuals have access to beneficiary data and feel comfortable navigating the MHM Data Registry. These individuals should have attended or reviewed the recording from the “Introduction to the MHM Data Registry” webinar and complete the “Using the MHM Data Registry in Practice” web-based training.

Are data on beneficiaries we are submitting into the MH Data Registry de-identified?

No, data on enrolled beneficiaries stored in the MH Data Registry will include an individual’s Health Insurance Claim Number (HICN), date of birth, first and last name, and other clinical data. The MH Data Registry is being hosted at a CMS data center and is built to comply with CMS Security standards, which includes meeting all FISMA and CMS security requirements for all PHI and PII administered within the database. These robust security standards ensure that your beneficiary data is kept confidential for the duration of the MH model and beyond.

Which EHRs currently support the EHR data upload to the MHM Data Registry?

The EHR data upload to the MHM Data Registry requires a CCDA format file, which is a standard export file from most EHRs. You can find more information on the file requirements by clicking the EHR upload option on the MHM Data Registry and accessing the data guides and templates at the top of the page in the shaded box. You can also contact the IBOSC Help Desk for CMMI models for more information.

Will the Registry trigger any alerts when it is time for the beneficiary’s next visit?

The MHM Data Registry will list each beneficiary’s Anniversary Date on the Data tab. It will also show an alert next to each beneficiary whose Anniversary Window is currently occurring. You can sort the Anniversary Date column in the Data Tab to show all individuals currently within their Anniversary Window.

Support Beneficiary Enrollment to Meet Requirements and Support Model Success

AlignmentValidationEnrollment

Submit demographic and clinical data collected during in-person visits with your Medicare beneficiaries at any time.

CMS will validate eligibility of the your beneficiaries by comparing the data you submit to Medicare data.

CMS will assign eligible beneficiaries submitted for enrollment to the appropriate participating organization or practitioner for the model.

Adjudication

Optionally dispute the alignment of any enrolled beneficiaries by contacting your MH Project Officer.

Remember these important requirements related to enrollment:

Enroll at least 90% of eligible beneficiaries in the model. CMS will inform you of any eligible beneficiaries you have not submitted for enrollment.

FAQ Answer

How do you suggest we identify eligible beneficiaries? Why doesn't CMS give us a list of attributed beneficiaries ahead of time?

Your participating practitioners and care team should familiarize themselves with the model eligibility criteria listed in your onboarding packet. The MH Model’s enrollment approach is designed to reduce the likelihood that enrollment disrupts your normal practice workflow.

Will participants receive the list of ICD codes the MH Model will use to determine if a beneficiary has had a heart attack, stroke/TIA, and/or ESRD?

Yes, during the week of 12/12, each organization’s MH Model POCs will receive a detailed list of ICD codes the MH Model will use to identify heart attack, stroke/TIA, and ESRD via email from the model team.

What happens if a beneficiary has a heart attack or stroke after enrollment and no longer meets the model’s eligibility criteria?

Your organization should not continue submitting data on any beneficiaries that become ineligible during the lifecycle of the model, including those that have a heart attack or stroke after enrollment.

Will we need a consent form from enrolled beneficiaries in order to collect their clinical or demographic data for this model?

No – you are not required to receive consent from beneficiaries to submit their data.

What are the requirements regarding integration of the MHM Data Registry into our workflow?

It is up to your organization to determine how the MHM Data Registry will best fit into your workflow. CMS does not provide requirements for this integration.

How do we update our contact information?

Please update your organization and provider information using the MHM Data Registry. Organization POCs and individuals with the Practice Administrator role on the MHM Data Registry will be able to access the Administration tab. Access this page to update this information at least once every six months.

Do Control group participant organization members have to attend any in-person meetings?

No. All training events for Control Group organizations will be held virtually.

When is Go-Live?

The MH Model will officially begin in early January, 2017. Please stay tuned for communications from the model team with schedule details. However, the MHM Data Registry is now accessible to participants. You can register for access, log in, and explore the site prior to the official start of the model in January. However, please keep in mind that any data you enter before the official model start date in January will be deleted upon that date.

My organization already participates in another CMMI model. Does this impact our participation in the MH Model?

No, participation in any other CMMI demonstrations does not impact your organization’s participation in the MH Model. Your participating practitioners can enroll beneficiaries in both programs and do not have to address the inclusion priority.

Will beneficiaries enrolled by Control group practices have access to fewer resources to promote their cardiovascular health than the Intervention group? Will this be the case for the full five years or will best practice and resources that prove effective be shared with the control group within the duration of the program?

As a Control Group member, members of your organization are encouraged to visit the Million Hearts® Initiative website (http://millionhearts.hhs.gov/) and utilize publically available resources for practitioners and other care practitioners found on the ‘Tools & Protocols’ page. The Initiative website can also be shared with your beneficiaries, as it also includes many cardiovascular health improvement tips and tools for beneficiary use. Any best practices identified by this model demonstration will be shared after the 5 year model has ended.
