Clinical Manifestations of Contrast Media Extravasation in the Emergency Department and the Ward응급실과 병동에서 발생한 CT 조영제의 혈관 외 유출의 임상양상

Purpose Contrast media extravasation (CME) is an adverse reaction after administration of contrast media during CT examinations. The purpose of this study was to evaluate the frequen-cy, management, and outcomes of extravasations and to assess the risk factors for CME in the emergency department (ED) and the ward.Materials and Methods This retrospective study was conducted at a single academic urban hospital from January 2013 to December 2015. We analyzed the medical records of all patients who experienced CME after undergoing a CT scan. We compared the patients’ age, sex, under-lying disease, injection site, injection flow rate, time of CT examination, type of CT examination, and severity of injury between those in the ED and the ward.Results CME occurred in 41 (0.36%) of 114767 patients, which included 16 (0.34%) in the ED and 25 (0.37%) in the ward. Both groups were more frequent in those aged older than 60 years and in female. Additionally, the abdominopelvic CT type and 2–3 mL/s as the injection rate were more common in both groups. However, CME was more frequent during the nighttime (10, 62.5%) in the ER, while it was more common in the daytime (14, 56.0%) in the ward. Severe complications were more frequent in the ER (9, 56.3%) compared with the ward (8, 32.8%). There were no significant differences in CME between the ED and the ward. When comparing the clinical manifestations in the mild and severe groups, the antecubital fossa (33.3% and 0%, respectively; p = 0.013) for the injection site and abdominopelvic CT (41.7% and 82.4%, respec-tively; p = 0.012) and CT angiography (41.7% and 5.87%, respectively; p = 0.014) for the CT ex-amination showed significant differences between the mild and severe groups.Conclusion In this study, there were no significant clinical differences in CME between the ED and ward. Thus, prevention is more important than the place of admission. Radiologists and emergency physicians should pay attention to CME in the ED because it frequently occurs at

만 정맥 카테터의 직경에 대한 의무기록이 누락된 경우가 많아서 직경에 따른 발생률의 직접적인
비교는 힘들었다.
그리고 본 연구에서는 조영제 유출량에 대한 의무기록이 미비하여 비교하지 못하였지만 조영제
의 유출량은 임상 증상과 관련이 있으며, 유출량이 많을수록 중증 합병증이 잘 발생한다고 보고되
고 있다(10). 하지만 유출량은 혈관의 위치 및 임상양상에 따라 차이가 나는데, 조영제가 조직 깊
숙이 유출되거나 환자가 증상이 없을 경우 다량(100 mL)의 조영제가 유출될 수 있고, 반대로 소량
의 유출(10 mL)임에도 불구하고 확산이 제한되는 말단혈관에서는 임상 증상이 잘 관찰된다고 보
고하고 있다(4, 16).
조영제 유출 발생 시간대는 응급실, 병동, CT실에서 근무하는 의료진의 근무시간이 고려되었으
며, 이번 연구에서는 병동에 비해 응급실에서 야간에 조영제의 혈관 외 유출이 더 많이 발생하였
다. 이는 첫째로 응급실이 24시간 CT 검사가 이뤄지는 특수부서이기 때문에 야간에도 검사가 많
이 실행되어 상대적으로 발생빈도가 높게 보이는 것으로 사료된다. 둘째로는 CT실 전담 간호사의
유무도 영향을 미쳤을 것으로 사료된다. 본원은 평일 주간에는 CT실에 조영제 투여를 전담하는
숙련된 간호사가 배치되어 있어 조영제를 주입하기 전에 정맥 카테터의 관개통성을 재확인하며
필요 시 카테터를 재삽입한다. 따라서 카테터 기능을 평가하는 간호사의 숙련도가 타 부서의 간호
사와 차이가 나며, 해당부서와 CT실에서 2차례 재확인하므로 조영제의 혈관 외 유출을 예방하는
효과도 있었을 것으로 생각된다. 이와 유사하게 최근 연구에서는 타 부서에 비해 영상의학과 의료
진이 카테터를 삽입할 때 혈관 외 유출이 덜 발생한다고 보고하고 있다(9). 이런 차이는 병동에서
24시간 이상 카테터를 유지하거나, 혈관 확보를 위해 여러 번 삽입을 시도하는 것이 영향을 미쳤
을 것으로 보고 있다. 또한 환자가 CT실로 내려오면서 카테터 위치가 바뀔 가능성 및 해당 부서의
카테터 삽입위치 선호도도 관련이 있을 것으로 보고 있다.
응급실과 병동 모두 복부 CT에서 조영제의 혈관 외 유출이 월등히 많이 발생하였다. 하지만 이
는 복부 CT가 시행된 횟수가 많았기 때문으로 생각되며 CT 종류가 혈관 외 유출에 영향을 준다고
단정하기는 어려웠다. Sistrom 등(17)은 자동 주입기를 사용한 흉부, 복부-골반 CT와 수동 주입을
이용한 뇌, 안구, 척추 CT를 비교하여 전자에서 발생률이 높다고(0.17% vs. 0.09%) 언급하고 있지
만 이는 CT의 종류보다는 자동 주입기를 통한 조영제 주입이 영향을 미치는 것으로 보고하고 있
다. 그리고 본원에서는 CT 혈관조영술에서 상대적으로 경미한 증상이 나타났고 통계적으로 유의
하였는데(p = 0.014), 이는 앞서 언급했듯이 다른 CT에 비해 직경이 큰 18G 정맥 카테터 사용에 따
른 것으로 생각된다. 또한 혈관조영술은 조영제 주입 속도가 다른 검사에 비해 빠르기 때문에 의
료진도 이를 인지하여 더 주의하여 검사를 진행하게 된다. 따라서 혈관조영술 시 조영제의 혈관
외 유출은 발생 빈도에 비해 중증도는 경한 경우가 많은 것으로 생각된다.
중증도를 보면 응급실에서 9건(56.3%), 병동에서 8건(32.8%)으로 응급실에서 중증 비율이 높았
다. 죽상동맥경화증, 당뇨, 레이노드 증후군, 정맥 림프 순환 질환(정맥 혈전증, 림프 부종) 등 말초
혈관에 문제가 있는 경우 조영제의 혈관 외 유출의 발생률은 높지 않지만 피부 괴사, 구획 증후군
과 같은 중증 합병증의 발생과는 관련이 있다고 보고되고 있어(18), 응급실에서도 이 같은 과거력
이 많을 것으로 생각되었지만 본 연구 결과에서는 두 군 간의 유의한 차이는 없어 더 많은 연구가

필요할 것으로 생각된다.
조영제가 혈관 외 유출된 부위는 보존적 치료만으로 충분하며 냉찜질은 급성기 통증과 염증반
응을 줄이고, 온찜질은 조영제를 순환계로 빨리 제거될 수 있도록 한다. 바늘을 이용하여 유출된
조영제를 짜내거나, 히알루론산분해효소(hyaluronidase)의 사용은 아직 효과가 입증되지는 않았
지만 다량의 조영제가 유출된 경우에는 고려해볼 수 있다(3). 수술적 치료는 조영제의 혈관 외 유
출 발생 2~4시간 후에 부종과 통증이 악화되는 경우, 피부색 변성, 감각저하, 피부 괴사, 물집이 호
전 없을 때 즉시 고려되어야 한다고 하였다(2). 본원에서는 조영제의 혈관 외 유출이 발생할 경우
중증도에 따라 조영제의 물리적 제거, 얼음 찜질, 수상 부위 탄력 붕대 압박, 국소 스테로이드 주
입, 상지 거상이 시행되었고, 증상이 호전 없고 악화된 경우 성형외과에서 추후 수술적 치료를 고
려하였다. 수술적 치료를 시행한 경우는 응급실에서 발생한 1건이 있었다. 이는 76세 여자 환자로
복통을 주소로 내원하였고 복부-골반 CT 검사를 위해 우측 손등에 정맥 혈관을 확보하였으며 조
영제 투여 직후 손등 부위의 국소적인 부종이 발생하였다. 보존적인 치료 24시간 후에도 부종의
호전 없고 조영제 유출 부위 주변으로 물집이 발생하였으며 증상 발생 9일째에도 다수의 물집 및
혈종 동반, 손의 감각저하 동반되어 성형외과에서 혈종 제거술을 시행하였고 이후 특별한 합병증
없이 경과 호전되었다.
조영제의 혈관 외 유출을 예방하기 위해서는 상대적으로 세포 독성이 적은 저삼투압, 비이온화
요오드계 조영제 사용을 권고하고 있으며, 환자에게는 조영제 주입 시 어떠한 증상이라도 발생한
다면 즉시 의료진에게 알리도록 교육하도록 하고 있다(2). 또한 조영제 투입 전에 생리식염수를 투
여하여 카테터의 관개통성을 확인할 것을 권고하고 있으며, 24시간 이상 잔존하는 정맥 카테터는
혈관 저항성을 증가시키므로 교체하도록 권고하고 있다. 조영제 주입 속도는 정맥 카테터 직경에
맞는 속도로 투여되어야 한다. 통상적으로 전완의 전주정맥에서 20G 이상 카테터를 사용할 경우 3
mL/s의 속도로 투여할 것을 추천하고, 손등, 손목, 발과 같이 말초 정맥혈관의 경우에는 1.5 mL/s
이상의 속도는 추천하지 않는다(11). 본 연구결과를 바탕으로 조영제의 혈관 외 유출의 예방을 위
해서는 응급실에서 야간에, 팔 오금 부위 혈관을 이용하여, 복부 CT를 시행할 경우, 조영제 투여
전에 카테터 및 정맥 혈관의 기능을 더 주의하여 확인해야 할 것으로 생각된다.
본 연구는 몇 가지 제한점이 있다. 첫째, 후향적 의무기록 분석의 한계로 전적으로 의무기록의
질, 정확도에 따라 선택 오류가 있을 수 있다. 둘째, 전자진료 입원처방 데이터베이스를 이용하여
연구기간 동안 발생한 환자의 모든 기록을 확인하고자 하였으나 의무기록 미비 등으로 연구설계
의 한계를 지닌다. 셋째, 3년간의 짧은 연구 기간과 적은 연구 수로 인해 일반화하기 힘들다.
응급실과 병동에서 시행된 CT에서 발생한 조영제의 혈관 외 유출에 영향을 줄 수 있는 여러 인
자에서 모두 통계학적으로 유의한 차이는 없었다. 하지만 응급실에서는 병동에 비해 야간에 많이
발생하였고 중증인 경우가 많아 상대적으로 주의를 요하였다. 조영제의 혈관 외 유출 및 그에 따
른 중증 합병증을 막기 위해서는 환자가 입원해 있는 장소보다 환자 및 의료진의 교육을 통한 사
전 예방이 더 중요하고, 증상 발생 시에는 의료진의 신속한 대처 및 세심한 추적관찰이 도움이 될
것이다.
