RoseFellowship in Medical Editing for 2006-2007. This one-year fellowship offers aunique blend of hands-on training inmedical writing and editing, combined withan academic faculty development position ata major university medical center.

The American Academy of FamilyPhysicians originated in the AAFP StaffFellows Program in 1991. The program isdesigned to give graduates of familymedicine residency the opportunity toenhance their skills in a concentrated area ofstudy, as well as work on behalf of thenation's premier organization for familyphysicians. The medical editing fellowship isdesigned to develop medical writing andediting skills and help future leaders formedical editing projects and family medicinefaculty. Additional experiences includeclinical work with underserved populationsin DC through UNITY Health Care, Inc.,and working with health advocacy groupsincluding the DC Medical Society and theDC Hospital Association.

The Department of Medicine at GeorgetownUniversity administers community hospital-based family medicine residency, and offersrequired course during all four years of themedical school curriculum. GERIATRIC MEDICINE

Arrowhead Regional Medical Center Colton, CAThe San Bernardino County's publichospital system has made a commitment toprimary care training since 1969, when SanBernardino County Medical Center gainedaccreditation for a Family PracticeResidency Program. In 1995, recognizing theneeds of San Bernardino County's olderpopulation and the importance of training inolder adult care for the Family Medicineresidents, the Medical Center leadershiprecruited faculty with training in GeriatricMedicine to begin to build clinical programsfor the aged. By 1997, a fellowship trainingProgram in Geriatric Medicine wasestablished and successfully accreditedthrough the ACGME. The purpose of thefellowship was the postgraduate training offamily physicians and internists in state ofthe art care of the aged. A new countyhospital facility was built in 1999 in nearbyColton. It underwent a name change toArrowhead Regional Medical Center(ARMC). It has state -of-the-art earthquake-safe architectural design and state- of-the-artmedical delivery systems. Although thefellowship training has dedicated primaryfaculty, it is a part of the Family MedicineResidency, which is also a fully accreditedprogram affiliated with the University ofCalifornia, Irvine. The 12 Month fellowshipprogram has developed a reputation for astrong clinical training.

ApplicationsThe ACGME-sponsored program isaccredited for 2 residents per year.Application is through the GeriatricMedicine office at 400 North Pepper Avenue,Colton, CA.

The Family and Elder Care CenterThere is one family health center where thegeriatric medicine fellows provide care forolder adults. Based in the ARMC OutpatientCenter, it offers modern state-of-the-artexam and treatment rooms. The suite haseight regular exam rooms, a family conferenceroom, and a minor ambulatory surgery suite.

Inpatient Consultation

Under the supervision of facultygeriatricians, geriatric fellows learn toprovide consultation on all services in thehospital (emergency room, acute units,behavioral medicine unit), thereby honingtheir hospital-based skills in elder care. Interdisciplinary Team Focus A strong emphasis is place on theinterdisciplinary team concept. This conceptis integrated into the fellowship experience.Team meetings are held continuouslyinterspersed with seminars, facultyinstruction in functional assessmenttechniques, and mini-didactics.

Community-Based ExperienceFellows spend a significant part of thetraining providing care in the home and innearby long term care facilities. Teachingfaculty are present in these settings, as areother members of the long term care team,who help to broaden the learning experiencefor fellows.

Case Western Reserve University (MetroHealth)Cleveland, OHMetroHealth Medical Center is an academichospital with a full complement ofresidencies and fellowships affiliated withCase Western Reserve University. Thetraining available in geriatrics is excellent.The MetroHealth program offers a fulldiversity of clinical experiences, two of itsown skilled nursing facilities, a geriatric-specific floor in an accredited rehabilitationhospital, a full consult care experience,hospice/palliative care training, and homecare curriculum. We also offer a PACEexperience and an extensive outpatientassessment and primary care clinic. Fellowsprovide teaching to family practice residents,internal medicine residents, and medicalstudents. Florida Hospital Orlando, FLThe geriatric fellowship program seeks totrain family medicine and /or internalmedicine physicians in all facets of clinicalgeriatrics over a period of twelve (12)months. Both medical and non-medicalaspects of elder care are emphasized,including institutional, non-institutional,

regulatory, economic, psycho-social,pharmacologic, rehabilitation, palliative, andother associated areas. Learning is patient-based in a variety of settings, stressing themulti-disciplinary team approach to thecomprehensive care of the frail, elderlypopulation. Fellows rotate responsibilitiesincluding inpatient geriatric consults,outpatient comprehensive geriatricassessment clinics, hospice care, and twolong-term care facilities. Emphasis is placedon evaluation and management of geriatricsyndromes such as dementia, incontinence,falls, polypharmacy, depression, etc. Fellowshave time for independent study andrecreation in beautiful central Florida asevenings and weekends are free from directpatient care duties. There are opportunitiesfor didactic, scholarly, and instructionalactivities. The goal of this program is toprepare graduates to obtain certification ofadded qualifications in geriatric medicine,and to be providers of excellent geriatric careas practitioners, educators, administrators,or directors of elder care facilities.

Pitt County Memorial Hospital/EastCarolina UniversityGreenville, NCOur program was established in 1984 andwas one of the first geriatric fellowshipprograms in the country. Our mission is toprovide quality geriatric health care to theregion, offer excellent teaching and advancethe science of geriatrics with research.

Fellows are taught by 8 geriatric faculty, andenjoy the extensive resources of theDepartment of Family Medicine, whichincludes 25 physician faculty and 25 facultyin other clinical and non-clinical positions.This includes faculty in educationaldevelopment, pre-doctoral education, andresearch.

Seminars are held weekly during the year tocover 45 geriatric topics. Fellows are involvedin a yearly 36-seminar program in facultydevelopment sponsored by the department,which covers a wide range of topics,including research fundamentals, curriculumdevelopment, small and large group teachingskills, and professional development.

All of our teaching sites are convenientlylocated within a ½ mile radius. Thefellowship year consists of the followingrotations: Nursing home (4 months),ambulatory geriatric clinic (3 months), rehab(1 month), hospice and home health(1month), neurology (1 month), hospitalinpatient service (1 month), and elective (1month). A number of topics are taughtlongitudinally, such as home care, psychiatry,geriatric consults, etc.

Texas Tech University (Amarillo)Amarillo, TXThe division of geriatrics at Texas TechUniversity Health Sciences Center inAmarillo, TX, offers a 12-month accreditedgeriatric fellowship. The patient-orientedtraining program encompasses didactic andbedside instruction, analysis of currentliterature, and the opportunities for bothclinical and laboratory research.

The geriatric training program is devotedprimarily to clinical geriatrics. The geriatricfellow will have the opportunity to care forgeriatric patients in various sites, includingacute inpatient medicine, an outpatientcontinuity care clinic, as well as anoutpatient comprehensive geriatricassessment clinic. Substantial time is spent inlong-term care, including both intermediatelong-term care, as well as short-stayrehabilitation. The program also offers aninpatient geriatric consultation service andhouse-call experiences both in private homes,personal-care homes and assisted living. Thegeriatric fellow will have rotations with bothinpatient and outpatient physical medicine, arotation on an inpatient gero/psych unit, andinpatient/outpatient neurology. It includes aresearch experience and time for electives.The geriatric fellow will also have structuredexperience in hospice and an adult daycarecenter with a PACE program. The programalso includes a 4-week rotation at the

Lubbock campus of the Texas TechUniversity Health Sciences Center, withadditional experience in long-term care, amood-affective disorder outpatient clinic,memory disorders clinic, and balance gaitdisorder clinic.

At the end of the 12-month rotation, thegeriatric fellow will be eligible to sit for theexam to obtain a certificate of addedqualification in geriatric medicine, and will beprepared for either a clinical practice orientedcareer or an academic practice orientedcareer.

UC Irvine Medical CenterOrange, CAThe University of California, Irvine (UCI)Geriatrics Fellowship is a comprehensiveone-year training program, under theauspices of the UCI School of Medicine,Program in Geriatrics. The FellowshipProgram has been accredited since 1991,providing eligibility for a Certificate ofAdded Qualifications (CAQ) in Geriatrics.

Fellows receive training at the UCI MedicalCenter (UCIMC) as well as affiliated sites,including the Veterans AdministrationMedical Center (VAMC), and high qualityalternative community care environments forseniors.

Primary care and the Health AssessmentProgram for Seniors (HAPS) are housed inthe new UCIMC SeniorHealth Center,especially designed for an aging populationand for people aging with disabilities.

Palliative Care Rotation - (1 month inpatientand 2 month outpatient) - During theinpatient month the fellow will admitpalliative care patients and follow them untildeath or discharge. The inpatient rotationemphasizes both pain and symptommanagement in the acute care of hospicepatients. The outpatient experience includesattending palliative care clinic, home visits onhospice patients with physicians, nurses,clergy and social workers.

Nursing Home Medical Directorship Rotation- The fellow will learn the duties of a nursinghome medical director. The rotation includesinstruction on infection control, woundmonitoring and management, qualitycontrol and improvement, behavior,nutrition, and falls assessments. Thisrotation includes an aging simulationexercise in which the fellow spends the nightin the nursing home setting as a patient.

For the period of theAntepartum Rotation fellows will learn toprovide competent prenatal care topregnancies complicated by diabetes,hypertension, twin gestation, thyroid disease,cardiac disease, renal disease, and mentalhealth disorders, stabilize and treat pregnantpatients hospitalized for pre-term labor, pre-eclampsia, diabetes, and placental disorders,perform biophysical profile and limitedobstetric ultrasound, and counsel obstetricpatients regarding genetic risk. Throughoutthe Gyn Surg Rotation fellows will attaincompetency in operative obstetrics, includingcesarean section, instrumented deliveries, tuballigation, and amniocentesis for assessment of

fetal lung maturity. Provide post-operative carefor such patients. And lastly, in the NeonatalElective Rotation, fellows will learn how toresuscitate distressed newborns and provideongoing care to the hospitalized newborn. Thefirst elective will be a focused neonatal care andresuscitation elective.

Each fellow will have zero to three clinics aweek, depending on their rotationassignment. Fellows will care for their ownpanel of patients in a continuity familypractice clinic. University Hospitals of ClevelandCleveland, OHThis is a 12-month fellowship, beginning July1 of each year, carrying salary and benefits ofa PGY4 resident at University Hospitals ofCleveland. A tentative schedule might include: • 9 months on Labor and Delivery, with

graded progression of responsibility froman OB PGY-1 to PGY-2 level, with someOB/Gyn outpatient clinic component.

• 1 month on neonatology, or other electiveexperience.

• 1 month on OB ultrasound, with outpatientwork with genetic counselors.

• 4 weeks of vacation, plus 10 day allotmentfor conference time.

It represents the first in-deptheducational and clinical experiencesinvolving a sports medicine fellowship at aNCAA Division I university. It continues to be an accreditedprogram under the family practice residencyat Sparrow Hospital in Lansing, Michigan.

CurriculumThe Sports Medicine curriculum provides anexcellent experience for the Sports MedicineFellow to achieve the cognitive knowledge,psychomotor skills, interpersonal skills,professional attitude and practicalexperience required in the care of patientswith problems related to sports and exercise.The curriculum is a 12-month program thatis primarily outpatient-based. Fellowsacquire the majority of their experienceworking in the Sports Medicine Clinic withprimary care providers and the orthopedicsurgical faculty involved in the program.Opportunities to work with subspecialiststhat are pertinent to Sports Medicine areutilized.The clinical practice setting is the foundationof this fellowship. Orthopedics, sportsinjuries, human performance andrehabilitation and medical issues related tosports, share the major emphasis. The sportsmedicine fellow sees patients that arerepresentative of all age groups, sexes andlevel of experience in sporting/exerciseinterests or activities in the sports medicineclinic on a regular basis. Athletes at all skilllevels will be represented in the patient mixas well. Under thedirection of the program director, and withassistance from the other sports medicinestaff and the affiliated orthopaedic surgeons,the residents will be supervised in theevaluation and management of medicalillnesses and musculoskeletal injuries whichoccur in athletes and others involved insports and exercise. There will be a strongemphasis placed in producing graduates whowill not only have exemplary clinical skills,but will also have the appropriate training toallow them to pursue scholarly activities suchas research, writing and teaching.

The training program is designed to be 12months in duration. The sports medicinefellows' clinical experiences will takepredominantly place at the sports medicineclinic at the Rothman Institute of ThomasJefferson University Hospital and the FamilyPractice Office at Thomas JeffersonUniversity Hospital. In addition, eachfellow will provide training room coverage atAcademy Park High School, MarpleNewtown High School where they will serveas team physicians. SM fellows will betaught the principles of rehabilitation andconnective tissue healing and will beexpected to oversee the recovery,rehabilitation and return to participation oftheir patients. Instruction in the properperformance and organization of pre-participation physical exams will be givenand the SM fellows will have the opportunityto provide these exams at the high school,collegiate and professional levels.

SM fellows will be guided in recognition andmanagement of those conditions whichrequire consultation and/or treatment fromother specialists, such as orthopaedicsurgeons. There will be the opportunity toobserve and assist in the care of theseconditions including the chance to developskills assisting in the surgical management ofsports related injuries. The SM fellows'clinical experiences will also provide themwith proficiency in the management of awide array of non-skeletal medicalconditions and an understanding of howthey relate to exercise and sport. Examplesof such conditions include eating disorders,reactive airway disease, cardiovasculardisease, menstrual dysfunction andinfections. SM fellows will develop anunderstanding of both the positive andnegative interplay between these conditionsand participation in physical activity. Theywill become highly competent in assistingpatients affected by these conditions andmaximizing both their daily functions andtheir athletic performance.

The training program will also provide theSM fellows with the opportunity to serve asteam physicians at high school, collegiateand professional levels. In this capacity theywill work with coaches and athletic trainersin performing pre-participation exams,covering events, making return to playdecisions, and overseeing the rehabilitationof injured athletes. Appropriate supervisionand consultation by the sports medicine staffwill be provided. The SM fellows in thissports medicine program will also develop akeen understanding of the physiology andbiomechanics of sport, nutritionalprinciples, (including fluid replacement) asthey apply to exercise; the pharmacology oftherapeutic, performance enhancing andrecreational drugs; psychological aspects ofexercise and performance in competition;ethical principles in athletics; and themedical legal aspects to sports participation.

In addition to the goal of promotingclinically proficient residents, the program isdesigned to promote scholarship in theresidents. SM fellows in the program willparticipate in existing courses in researchmethodology and design, statistics andeducational methods with other post-graduate residents in the Department of

Family Medicine. SM fellows will be assistedin preparing an abstract for a nationalmeeting. Regular attendance at journalclubs will help SM fellows prepare to belifelong learners. Graduates of the programshould have the skills to join academicDepartments of Family Medicine.

The fellow will have two family medicinecontinuity clinics a week at the familypractice residency site and two sportscontinuity clinics at the University ofWashington student health center.

We work in state run ICF/MRs (intermediate care facility for the mentally retarded). The individualslive at the center and many have been there for most of their adult years. You would be a full timeemployee of our group and could chose to receive full benefits package. You would be one of four fulltime physicians working at the center. If you are or have worked in private practice/managed careenvironment where you may have felt pressure to see as many patients as possible in order to bill forservices, you will appreciate our positions. The individuals living at the center may not be onindividual Medicare or private insurance so you would not have the hassles associated with billingMedicare, 3rd party insurance, etc. You have the luxury to spend as much time with a patient and notfeel the pressure to do "x" number of units per day. The services you provide are reimbursed viafunding avenues including state and federal funding. In addition to this position being less stressfulthan private practice/managed care, you would also have the luxury to work "normal" hours andcould be on your way home everyday by 4:30.
