A. EFFECTIVE DATE : July 1, 2018 B. PURPOSE : To establish uniform standards for the use of the Chronic Opioid Therapy

Pain Management Agreement with patients receiving opioid prescriptions for chronic pain for 3 months or longer.

C. POLICY : 1. All patients prescribed opioids for treatment of chronic pain for 3 months or longer will be required to participate in a Medication Agreement.

2. Patients will have been assessed for risk of future addiction. 3. Agreements are valid for 1 year and must be renewed if treatment

continues. 4. Practitioners will adhere to all requirements of state law including

checking CPMRS prior to the initial prescription and at 3 month intervals.

5. UConn Health Practitioners will not provide long term opioid prescriptions to patients who do not adhere to the terms of the agreement including random urine testing, pill counts, and adjunctive pharmacologic and non-pharmacologic therapies (physiotherapy).

D. SCOPE : Applies to all UConn Health Providers who prescribe opioids for chronic pain.

with Chronic Non-malignant Pain

A. EFFECTIVE DATE: July 1, 2018

B. PURPOSE : To Operationalize the Chronic Opioid Therapy Pain Management

Agreement

C. POLICY : Policy Chronic Opioid Therapy Pain Management Agreement [Link]

D. SCOPE : Applies to all UConn Health patients receiving opioid prescriptions for

the treatment for Chronic Non-Malignant Pain (CNMP) with the

exception of patients with Sickle Cell Disease

E. DEFINITIONS : 1. Chronic Opioid Therapy – Any treatment in which an opioid is

prescribed for 3 months or longer

2. CNMP – Chronic Non-Malignant Pain

3. CPMRS- state controlled substance prescription monitoring system

4. EHR – Electronic Health Record

5. LEP – Limited English Proficiency

6. ORT – Opioid Risk Tool

7. VRI – Video Remote Interpreter

F. MATERIALS NEEDED : Chronic Opioid Therapy Pain Management Agreement

G. PROCEDURE

(WORKFLOW) :

1. Prescribing practitioner determines the need to prescribe an opioid

for relief of CNMP when alternate measures to relieve pain have

been unsuccessful.

2. Prescribing Practitioner completes the risk assessment (the ORT

can be done in under 1 minute) to determine the risk of future

addiction.

3. Practitioner informs the patient of the need to initiate a

medication agreement.

4. The agreement is reviewed with the patient by a member of the

care team.

a. For patients who are deaf or hearing impaired, an

interpreter must be provided either via the VRI or with a

live interpreter.

b. For patients with LEP, an interpreter must be provided

either via the telephone interpretation service or a live

interpreter.

5. The patient or legal guardian signs the agreement.

6. Clinical staff assure that the agreement is dated with the date the

agreement is signed and the date the agreement expires (one year

from date of signing).

7. Clinical Staff facilitate scanning of the agreement into the EHR

where it will be viewable by all other practitioners involved in the

patient’s care.

8. Clinical Staff provide the patient with a copy of the agreement.

9. Clinical Staff perform initial qualitative urine toxicology screen.

Toxicology screen will be repeated every 6 months and at random

intervals as determined by the practitioner with a screen for the

prescribed opioid.

10. The Practitioner consults the CPMRS prior to the initial prescription

of a controlled substance and at 3-month intervals.

11. Clinical staff will perform random “pill counts” at the direction of

the Practitioner. This can be used for monitoring and safety

reasons for patients with inconsistent urine results, as a secondary

method of measuring adherence, and for dialysis patients.

Patients are to come in within that business day. If this is

burdensome due to work or life responsibilities, staff can call

randomly sometime in the following two weeks. Patient must

come in one of the two times called.

12. Practitioner may discontinue prescribing opioids if there is a

substantive change in the patient’s medical condition including the

onset of pregnancy, failure to obtain benefit, or serious adverse

effect.

13. Medication Agreements will be considered void

a. If the patient violates the term of the agreement.

b. If the patient’s qualitative urine toxicology screen has

other opioids present or prescribed substance is absent.

c. Patient fails to keep quarterly follow up appointments.

Chronic Opioid Therapy – Provider Initial Checklist

Initial Checklist: � Determined that chronic opioid is the best medical option for treating the patient’s chronic pain

� Patient is participating in ALL aspects of a multidisciplinary care program as indicated including: o Mental health care, Physical therapy, compliance with other medications, specialist

appointments, PCP office appointments

� Identify what pain is being treated with the opioid

� Identify a patient goal for treatment

� Completed Opioid Risk Tool (ORT) reviewed and documented

� Review and sign medication agreement (copy to patient and to scanning for EMR)

Chronic Opioid Therapy – Provider Follow-up Checklist

Follow up Checklist: � Patient is attending Q3 month visits for opioid check in (mandated by the DEA)

� Patient is participating in ALL aspects of a multidisciplinary care program as indicated including:

o Mental health care, Physical therapy, compliance with other medications, specialist

appointments, PCP office appointments, random utox or pill counts

� Considered alternative and/or adjunctive chronic pain medications (Duloxetine, Gabapentin,

Pregabalin, TCA, Cyclobenzaprine)

� Checked CTPMP (must be done at least every 3 months as per CT State Statute) and it appeared

as though the patient was in compliance with conditions of the opioid agreement

� Reviewed prior utox (perform at least every 6 months), perform new utox if needed

� Medication agreement is on file and less than 1 year old, if more than 1 year old, new

agreement signed today

� I have discussed intranasal Narcan with the patient, prescribed it, and provided the patient with

a list of pharmacies that dispense it

� Patient has been recommended a stimulant laxative (Senna) if needed for constipation

Chronic Opioid Therapy - Patient Introduction UConn Health is committed to exceptional, high quality patient care and safety. Your provider has recommended a medication to treat your pain with the goal of improving your functional status. These medications can be helpful; however, can also be dangerous if not taken as directed by your provider. We are committed to your safety while working with you to get better. This document highlights our institutional rules and expectations which must be followed for your safety. Safety monitoring is for everyone regardless of age, race, diagnosis, ethnicity, or social class. Every patient receiving opioids from UConn Health is monitored for their safety. Our Commitment to You:

• WE will help you understand the risks and benefits of taking these beneficial yet potentially dangerous medications.

• WE will monitor your safety which may include: random urine testing for the presence of the medication and other drugs; routinely reviewing your medical, pharmacy and refill records; random pill counting; monitoring refills through the Connecticut Prescription Monitoring Program.

• WE will use these medications in combination with other helpful treatments (physical therapy, massage, acupuncture, cognitive behavioral therapy, counseling, etc).

• WE will ask for help when we need it and may refer you to a specialist (Neurologist, Orthopedist, or Psychiatrist) if indicated.

• WE will help you get the care you need, including treatment for addiction and mental health problems if necessary. Treatment program information and phone numbers will be provided when indicated or requested without judgement or bias.

• WE will update you with new opioid and non-opioid pain medications and treatments. • WE will continue to treat your pain in other ways if we decide that controlled substances are no

longer safe or working for you and need to be discontinued. • WE will NOT continue using opioids or sedatives if we don’t think they are safe or beneficial

anymore.

UConn Health Chronic Opioid Therapy Pain Management Agreement

I understand that opioid pain medications can be addicting. I know that it is a crime to give

away or sell these medications. I understand that my doctor does not have to prescribe these

medicines for me and that there are other options to treat pain. I can die from taking too much

opioids. The risks and benefits of opioid pain medications have been discussed with me.

Because these medications are dangerous, there are many rules for getting regular opioid

prescriptions from my doctor’s office, which are described in detail below. I understand and

agree to follow all of these rules.

Chronic Opioid Therapy Informed Consent

POSSIBLE RISKS: 1. Overdose: You can die from an overdose of opioid medications. The risk of dangerous overdose

is higher if you take opioids combined with alcohol or other medications that can make you

sleepy (See Appendix I). Most overdoses are unintentional and can happen to anyone taking

opioid medication.

a. There is a medicine called Naloxone (Narcan®) that can save your life. It is available at

some pharmacies and does not require a prescription. We recommend you get it, but it

is not a requirement of this agreement.

2. Side Effects: Such as constipation, nausea, vomiting, sedation, drowsiness, slowed breathing,

slowed reaction time, confusion, difficulty thinking, depression, headaches and sexual

dysfunction can occur at any time. It may be unsafe for you to operate machinery, drive a car, or

be responsible for taking care of other people while using this medication.

a. For men: Chronic opioid use can cause low testosterone levels in men. This can

decrease your stamina, mood, libido and physical and sexual performance.

b. For women: If you use opioid medications while pregnant, the baby can be born

addicted to opioids. I will tell my doctor right away if I am or I become pregnant.

3. Addiction: Opioid medications are addictive.

Tolerance: Your body may get used to the opioid medications and their effect on your pain

could decrease over time. If this happens, we may need to stop the medicine.

Physical Dependence: You may have withdrawal symptoms such as abdominal cramping,

diarrhea, anxiety when you stop the medicine.

4. Theft: People may try to steal these medications from you. You should not tell other people you

have these medications. You are responsible for keeping these medications safe, for example,

keeping them hidden in a lock-box.

5. CT Law: Connecticut state law (CGS § 14-227a) prohibits driving while under the influence of

alcohol or drugs. If your driving is impaired and you are taking opioid medications, you could get

a DUI (Driving Under the Influence) or a DWI (Driving While Intoxicated) offense.

POSSIBLE BENEFITS: 1. Opioids may help improve your function at home and work

2. You may have improvement of your pain, but will not make you pain free, and the

improvement can decrease over time

3. By decreasing pain, you may have improvement in sleep

4. If pain is better controlled, you may be able to take better care of yourself such as exercising,

working, and taking care of your home

5. You and your doctor may be able to focus more on your other medical problems once your pain

is better controlled

6. Overall, taking opioids may improve your quality of life

7. Setting a treatment goal helps measure how opioids are helping you with your pain

ALTERNATIVES: There are many alternative options for treatment of chronic pain other than opioids. Opioids

are not the recommended treatment for chronic pain that is not due to cancer. Opioids are not

recommended on their own to treat chronic pain. Alternative options will be recommended in

addition to opioid medication (See Appendix II).

Pain Management Agreement

AGREEMENT: 1. I understand that treatment with opioids is started as a trial and will only be continued if it is

helping.

2. I understand that the goal of treatment is to function better and opioids will not make me pain

free.

3. I understand that medication is only one part of my treatment plan and I agree to participate in

all recommended treatments and follow up.

4. I agree to have regular visits specifically for the condition I am receiving opioid therapy to treat

as well as set yearly clinic visit to re-sign a medication agreement. At a minimum, I will be seen

in the office every three months for ongoing care and evaluation.

5. I agree to take the medication only as prescribed. It is my responsibility to call the office no less

than 7 days in advance when I need a refill. I understand that refills will never be provided at

night or over the weekends. I understand that changes to my prescriptions, including dose

adjustments and new medications, will be made only during scheduled office visits and not over

the phone or during unscheduled visits.

6. I agree to random drug testing and random pill counts, which may be requested at any time. I

understand that the medication will be stopped if I am unreachable or unavailable for this

monitoring within 24 hours when requested.

7. I agree to fill all opioid prescriptions at the same pharmacy every time.

8. I understand that providers in CT are limited to a 7 day supply for a new prescription and a 30

day supply script without refills after that.

9. I understand that lost or stolen medications will not be refilled early. It is my responsibility to

protect and secure my medications. This includes keeping them out of reach of children.

10. I will not abuse alcohol or use any illegal drugs while taking this medication.

11. I will not take any opioid medications from another provider. I will not take any sedating

medications or other controlled substances without prior approval from my provider. Controlled

substances can include prescriptions from other doctors, medications borrowed or accepted

from family or friends and any illicit or street drugs.

12. I understand that pharmacies report all opioid prescriptions to a database that my prescribers

are required by CT law to check.

13. I understand that if my prescriber is unavailable, a covering provider within my UConn Health

primary care office may refill my medication.

14. If an unavoidable emergency situation occurs requiring a prescription for sedative or opioid pain

medication, I will contact my prescribing provider within 72 hours and discuss with the office.

15. I agree to allow my providers at the UConn Health to speak with other providers (within and

outside UConn Health) and pharmacists regarding my use of controlled substances, mental

health and pain management as deemed clinically necessary. I agree to have the electronic

medical record contain information about the opioid agreement so the other providers at

UConn Health are informed.

16. In addition to all of the above, I understand and accept the right of UConn Health to terminate

this agreement for the following reasons:

a. If I give, sell, or in any way distribute prescribed medications to any other person(s).

b. If I, in any way, attempt to forge or alter a prescription.

c. If I have violent or aggressive behavior at the clinic or over the phone.

d. If my medical condition declines to the point at which, in my doctor’s judgment,

continuing opioid medications is dangerous for my well-being or safety.

e. If there is evidence that I am no longer receiving a reasonable benefit from the

medication or my doctor determines that I am no longer a good candidate to continue

the medication.

f. If I don’t follow all of my physician’s recommendations.

g. If I don’t follow all of the conditions of this agreement.

UConn SDM Clinical Guidelines: Management of Acute Orofacial Pain – developed by the SDM Opioid Working Group led by Dr. Elias M. Chatah – June 2019

A. EFFECTIVEDATE

06/19/2019

B. PURPOSE To establish uniform standards for the management of acute orofacial and postoperative pain.

C. SCOPE Applies to all UConn School of Dental Medicine providers. D. GUIDELINE Children and adolescents: refer to the American Academy of Pediatric Dentistry (AAPD)

Policy on Acute Pediatric Dental Pain Management (2017).

Adults: 1. Consider the use of long-acting local anesthetics such as bupivacaine to reduce the

severity of post-operative pain.2. In the absence of a contraindication, Non-Steroidal Anti-Inflammatory Drugs

(NSAIDs) should be considered pre-emptively and first-line analgesic therapy tohelp reduce the severity of postoperative pain.

3. The sequencing of NSAIDs and APAP administration is evidence-based to besynergistic and should be considered.

4. If opioids are indicated:a. it is recommended that the provider screen patients for the risks of

developing substance use disorders using a validated screening tool,namely, the Opioid Risk Tool (see below).

b. it is recommended that the provider, or their delegate, check the CTPrescription Monitoring and Reporting System (CPMRS) for any concernsrelated to substance use disorders and potential opioid misuse.

Long-acting or extended-release opioids are not evidence-based for managing acute and post-operative pain.

5. Patient identified as “not at risk” of developing substance use disorders or opioidmisuse, and in the absence of 1. an opioid contraindication, 2. a past and familyhistory of substance use disorders, 3. benzodiazepine and centrally-acting sedativetherapy, and 4. drug interactions, a short-acting opioid with the lowest potencymay be prescribed for no longer than 3 days.

6. Patients identified as “at risk” of developing substance use disorders or opioidmisuse:

a. must be informed of their risk screening resultb. must be re-evaluated for the need for opioids. The provider must consider

a trial of opioid alternatives (NSAID + APAP) prior to prescribing an opioidc. if opioid alternatives are not effective, they may be prescribed a short-

acting opioid with the lowest potency for no longer than 3 days7. Consent for opioid therapy must take place by informing the patient of possible

Opioid Tapering General Principles Opioid tapering can be a scary time for patients. Prescribers may be worried about their patient turning to illegal means for pain control or abandoning them in their pain management treatment, but tapering can reduce the development of tolerance and offer the opportunity to assess continued need of a high risk medicine. Additionally, reasons to taper are varied and range from alleviating unacceptable side effects, reducing potential for diversion of medications, address the failure to reach functional goal(s), and meet the desire for less risky medication use.

Tapering should begin by decreasing dosing milligrams first followed by frequency of dosing once at the lowest milligram option. If this is not tolerated, decrease to the lowest acceptable dose and then decrease frequency if able.

Published evidence illustrates success for patients on high daily doses and a willingness to taper.

Many medications used to alleviate withdrawal symptoms, like opioids, need to be used with caution in patients >65.

Naloxone Prescribing Information Sheet Naloxone kits should be prescribed to high-risk patients:

• Received emergency care for opioid intoxication or

overdose

• Have suspected substance abuse or nonmedical

opioid use

• Are taking >100mg morphine equivalents/day

• Are taking opioids but have unreliable access to

emergency medical services

• Have been recently incarcerated/released from

prison

• Have resumed opioid use after a period of

abstinence

• Patient or family requesting naloxone kit

• Are receiving an opioid prescription for pain PLUS:

o A prescription for buprenorphine or

methadone

o A history of poorly controlled respiratory

disease or infection

o A history of renal dysfunction, hepatic

disease, or cardiac comorbidities

o Known or excessive alcohol use or

dependency

o Concurrent use of benzodiazepines,

antihistamines, muscle relaxants,

barbiturates, or alcohol

o Suspected poorly controlled depression

Provider liability: • No civil or criminal liabilities for prescribers prescribing naloxone kits (CT §17a-714a (2012))

• Providers do not need to be affiliated with an opioid overdose prevention program
