What is EBM?

Evidence-based medicine (EBM) is the integration of best research evidence with clinical expertise and patient values.

by best research evidence we mean clinically relevant research, especially from patient centered clinical research into the accuracy and precision of diagnostic tests (including the clinical examination), the power of prognostic markers, and the efficacy and safety of therapeutic, rehabilitative, and preventive regimens.

Evidence-based medicine (EBM) is the integration of best research evidence with clinical expertise and patient values.

New evidence from clinical research both invalidates previously accepted diagnostic tests and treatments and replaces them with new ones that are more powerful, more accurate, more efficacious, and safer.

• Il ragionamento diagnostico assume così le caratteristiche del riconoscimento di un’impronta digitale: si ricercano i punti di identità fra malattia conosciuta e situazione clinica del paziente e, quando i punti di identità superano un certo numero, si definisce la diagnosi (approccio euristico al problema).

• The American College of Cardiology (ACC) released a list of ‘‘Five Things Physicians and Patients Should Question’’ in cardiology as part of Choosing Wisely which is an initiative of the American Board of Internal Medicine (ABIM) Foundation in partnership with Consumer reports.

• These were among 45 common tests and procedures recommended by nine medical specialty boards that should be performed less often.

The ACC list details the following five recommendations: 1. Don’t perform stress cardiac imaging or advanced non-invasive

imaging in the initial evaluation of patients without cardiac symptoms unless high-risk markers are present.

2. Don’t perform annual stress cardiac imaging or advanced non-invasive imaging as part of routine follow-up in asymptomatic

patients.

3. Don’t perform stress cardiac imaging or advanced non-invasive imaging as a pre-operative assessment in patients scheduled to undergo low-risk non-cardiac surgery.

4. Don’t perform echocardiography as routine follow-up for mild, asymptomatic native valve disease in adult patients with no change in signs or symptoms.

5. Don’t perform stenting of non-culprit lesions during percutaneous coronary intervention (PCI) for uncomplicated hemodynamically stable ST-segment elevation myocardial infarction (STEMI).

The main objection is the perception that ‘Choosing Wisely’ is the first collective effort on the part of professional medical societies to decide how to practise medicine on the basis of cost first and foremost.

The ACC list details the following five recommendations: 1. Don’t perform stress cardiac imaging or advanced non-invasive

imaging in the initial evaluation of patients without cardiac symptoms unless high-risk markers are present.

2. Don’t perform annual stress cardiac imaging or advanced non-invasive imaging as part of routine follow-up in asymptomatic

patients.

3. Don’t perform stress cardiac imaging or advanced non-invasive imaging as a pre-operative assessment in patients scheduled to undergo low-risk non-cardiac surgery.

4. Don’t perform echocardiography as routine follow-up for mild, asymptomatic native valve disease in adult patients with no change in signs or symptoms.

5.Don’t perform stenting of non-culprit lesions during percutaneous coronary intervention (PCI) for uncomplicated hemodynamically stable ST-segment elevation myocardial infarction (STEMI).

Conclusions

• In patients with STEMI and multivessel coronary artery disease undergoing infarct-artery PCI, preventive PCI in noninfarct coronary arteries with major stenoses significantly reduced the risk of adverse cardiovascular events, as compared with PCI limited to the infarct artery.
