Introduction

The New York State Department of Health is collecting information on Early Intervention services

The information collected will be used to:

See how well Early Intervention services work

Improve services for all children with special needs

What information will be collected?

You and people that have evaluated your child, will be asked about how your child does in three areas:

Positive social emotional skills

Playing with others (children and adults)

Expressing emotions and feelings

What information will be collected?

Acquiring and using knowledge and skills, including early language and communication

ImitationProblem-solving skillsAttentionCommunication skillsMemory

Use of appropriate behavior to meet needs (taking appropriate actions to meet needs)

Getting help when neededAdaptive skills (e.g., self-feeding, toileting, sleeping)

How will my child’s abilities be measured?

A meeting will take place to complete a form that will assess how your child is doing in the three areas

The meeting will take place during your child’s IFSP meeting

During the meeting information will be collected from everyone to complete the form

How can I get ready for the meeting?

How is your child doing in the three areas?

Is your child doing what other children his or her age are?

The Parent Edition of the Developmental Milestones Checklist can also be used to help you get ready for the meeting

The Developmental Milestones Checklist

The Developmental Milestones Checklist shows a list of behaviors and the age by which they usually occur

Completing the checklist and bringing it to the meeting will help you be a part of the discussion

Important Information for Parents

Participating in this process will NOT affect whether or not your child can receive Early Intervention services

No individual child’s data will be reported

Do I have to agree to have the form completed?

No, parents are not required to complete the form

However, reporting child outcomes is a federal requirement and the information parents give will help improve Early Intervention services
