Introduction

Under new mandates NYSDOH is required to report to the Federal Government on progress made by children in the Early Intervention Program related to three specific outcome areas

Introduction

Child outcomes information is collected on children in the sample who are found eligible for the early intervention program

To track progress the Child Outcomes Summary Form (COSF) was developed

The COSF is completed for children who receive an IFSP

Introduction

Evaluators are responsible for participating in the discussion during the IFSP meeting to complete the COSF

To participate in the conversation you will need to provide information about:

The results of your evaluations

The child’s current level of functioning

Evaluator’s Role in Completing the COSF

Inform parents about the new Federal requirement to report child outcome information

Information will be gathered in three broad outcome areas before the child begins to receive EI services

If the child is found eligible (based on state eligibility criteria) the information will be discussed at the IFSP meeting and will be used to determine the child’s CURRENT level of functioning in the three outcome areas

The child’s progress will be measured again when the child exits the EI program

Evaluator’s Role in Completing the COSF

Collect information related to the child outcomes and developmental milestones during the evaluation process

Select assessment(s) and evaluation procedures to be used to assess child’s functioning in the three outcome areas

Evaluator’s Role in Completing the COSF

Complete the Developmental Milestones Checklist - Evaluator Edition or Evaluation Summary - Federal Child Functional Outcome areas

Submit one of the forms to the municipality as an addendum to the child’s multidisciplinary evaluation

Bring the completed form(s) to the meeting to reference during the discussion

If the child is not found eligible for EIP it is not necessary to submit one of these forms

Evaluator’s Role in Completing the COSF

To participate in the conversation you will need to provide information about:

The results of your evaluations

The child’s current level of functioning

If you are unable to attend the meeting, arrangements can be made for your involvement in the meeting
