Introduction In conjunction with national efforts to increase
the availability of addiction treatment outcomes, the Alcohol and
Drug Abuse Division of the Minnesota Department of Human Services
provides the following measures for the entire state of Minnesota
and for individual treatment providers licensed under state Rule
31. This report will be updated annually. The measures fall into
four categories. (1) The Patient Profile presents background
information about patients obtained at admission to treatment. (2)
Discharge Status describes the situation in which the patient
leaves treatment. (3) National Outcomes Measures (NOMS) are
prescribed by the Substance Abuse and Mental Health Services
Administration (SAMHSA) and provide information at admission and
discharge on patients’ status in six critical domains of living.
SAMHSA initially considered family support as another indicator but
decided not to include it. Because DHS continues to collect the
data, the measure is included in this report. (4) Dimensions of
Addiction are similar to measures developed by the American Society
of Addiction Medicine and provide information at admission and
discharge on six dimensions of addiction. The Patient Profile
presents the percentages of patients in categories of gender, race,
age, primary substance, source of income, and sources of referral
to treatment. These are obviously not measures of performance but
rather provide a picture of the mix of patients seen statewide and
by different providers. Since patients can have two sources of
referral, the percentages for this measure do not sum to 100. The
next measure is Discharge Status. Considerable research
demonstrates that completing treatment is associated with
post-treatment abstinence. The NOMS attempt to capture meaningful,
real life outcomes for people who are striving to attain and
sustain recovery and participate fully in their communities in the
wake of receiving treatment for an active addiction to drugs or
alcohol. They include: rates of homelessness, unemployment, arrest,
alcohol and drug use, involvement in self-help groups, and family
support for recovery. Since these measures are coded so that higher
percentages indicate more people having trouble in that domain,
declines in the percentages from admission to discharge indicate
success. Dimensions of Addiction show the percentages of patients
with serious problems in six dimensions at admission and discharge.
The severity levels within each dimension range from no problem (0)
to extreme problem (4). The dimensions are: 1. Acute
Intoxication/Withdrawal ranges from (0) patients who exhibit no
intoxication or
withdrawal symptoms to (4) those with symptoms so severe that
the patient is a threat to self or others.
2. Biomedical Conditions and Complications range from (0)
patients who are fully functional to (4) those with severe physical
problems or conditions that require immediate medical
intervention.
3. Emotional/Behavioral/Cognitive Problem ranges from (0)
patients with good coping skills and impulse control to (4) those
with such severe emotional or behavioral symptoms that they are
unable to participate in treatment.
4. Resistance to Change ranges from (0) patients who admit
problems and are committed to change to (4) those who are in denial
and unwilling to change.

5. Relapse/Continued Use Potential ranges from (0) patients who
recognize risk and are able to manage potential problems to (4)
those who have no understanding of relapse issues and display high
vulnerability for further substance use disorders.
6. Unsupportive Recovery Environment ranges from (0)
environments that are supportive of recovery to (4) environments
that are antagonistic to recovery.
The numbers shown in the tables are the percentages with (2)
moderate, (3) severe or (4) extreme problems in a dimension.
Declines in the percentages from admission to discharge indicate
success. For both NOMS and Dimensions of Addictions, the tables
show percentages of patients at admission and discharge with
problems. The last column shows the percentage improvement for each
indicator. This is simply 100 times the difference between the
percentage at admission and the percentage at discharge, divided by
the percentage at admission. In other words, this column can be
interpreted as the percentage of the problem that is reduced during
treatment. Because negative values are set to 0, the measures range
from 0 (no improvement) to 100 (all problems substantially
improved). The last row under both NOMS and Dimensions of Addiction
provides a summary measure of improvement. (It results from
dividing the sum of the percentage point improvements on each item
by the sum of the percentages with a problem at admission on each
item and multiplying this ratio by 100). In other words, the
summary measure for NOMS assesses the percentage of the overall
problem captured by NOMS that is ameliorated by treatment, and the
summary measure for Dimensions of Addiction does the same for
Dimensions of Addiction. Negative values indicate that the problem
actually worsened, a value of 0 indicates the problems remained
about the same, and a value of 100 indicates that all problems show
substantial improvement. All data are obtained from The Drug and
Alcohol Abuse Normative Evaluation System (DAANES), which is
administered by the Alcohol and Drug Abuse Division of the
Department of Human Services in order to monitor treatment services
for substance abuse in Minnesota. DAANES data are collected by
providers as clients enter and leave episodes of treatment. All of
these measures are based on episodes of treatment that began in
2015 and are accompanied by discharge data submitted by December
27, 2016. For all tables, data are presented only when there are at
least 10 observations for the measure.
