CHAPTER I

INTRODUCTION

Background of the Study:Witnesses are a crucial support to the dispensation of justice in any civilized

"n the process ho#ever, #itness ay e$perience and suffer fro threats and

harass ents #hich can affect their #ell%bein! and overall health. They e$perience a state

of disbelief and often feel nu b, deeply upset and trau atized. So e of the e$perience

physical sy pto s such as sha&es, sleeplessness and cryin! all the ti e. 'lthou!h so e

of these #itnesses ay not e$perience trau a, ental ba!!a!e occurs as little stresses of

everyday life build up.

While they carry out their s#orn duties to the state, the stress load they e$perience

ay be too uch to ta&e. They can e$perience depression, suffer fro ournin! that

they have never overco e, an$iety, post trau atic stresses, suicidal thou!hts and so e

other proble s that have not been treated. These stresses have the potential to ne!atively

affect an individual s psycholo!ical and physical health ( '*+).

The delayed reactions to these stresses anifest itself in the for of

psychoso atic sy pto s such as headaches, chronic usculos&eletal pain syndro es,

sto ach aches, heart palpitations and breathin! disturbances ( attilio et al, 200-). ivin!

#ith these stresses ay have an i pact on an individual s physical and psycholo!ical

health, affect activities of daily livin! and ay threaten the stability of personal

relationships thus affectin! /uality of life.1

The World *ealth +r!anization (W*+) defines uality of ife as an individual1s

perception of their position in life in the conte$t of the culture and value syste s in #hich

they live and in relation to their !oals, e$pectations, standards and concerns. "t is a broad

ran!in! concept affected in a co ple$ #ay by the person1s physical health, psycholo!ical

state, personal beliefs, social relationships and their relationship to salient features of

their environ ent.

Several studies focused on the /uality of life of people #ith health proble s such

as *" 3'" S, cancer, psychiatric disorder, de entia and any ore. To the best of the

researcher s &no#led!e, no study has been reported on assessin! the /uality of life of

individuals ad itted into the Witness rotection, Security and 4enefit ro!ra in 5e!ion

"6. Thus, the researcher feels the need to assess their /uality of life in relation to their

perspectives on health, current health conditions, and other relevant factors. This study

#ill also enable the readers to better understand the daily challen!es e$perienced by such

#itnesses.

Rev e! of Re"ated # terature

"n 789:, the World *ealth +r!anization defined health as bein! not only the

absence of disease and infir ity but also the presence of physical, ental and social #ell

; bein!. Since then, issues re!ardin! /uality of life beca e of !reat i portance in

research and health care practice. 's there #ere only five (<) articles re!ardin! /uality of

life listed in the =edline database, there had been an increase in the utilization of /uality

of life ( + ) evaluation since 78->.

'ccordin! to a#ton (7887), + is a ulti%level concept reflectin! both acro%

societal and socio%de o!raphic influences and also icro concerns, such as individuals

e$periences, circu stances, health, social #ell%bein!, values, insi!hts, and psycholo!y. "t

therefore enco passes the objective and subjective di ensions of a person or a !roup of

people.

There #ere several literatures revie#ed on /uality of life. +ne of #hich is the

/ualitative study by =insuarez (2008). Si$ty%four (?9) participants #ere recruited fro

the three (>) dialysis units of @a boan!a Aity na ely, the @a boan!a Aity =edical

Aenter (@A=A), Western =indanao =edical Aenter (W==A) and the @a boan!aoctor s *ospital (@ *), usin! convenience sa plin!. The infor ation !athered #ere

su arized into several the es these areB 5eaction to ia!nosis Cno#led!e of the

isease and Treat ent, Dffects of isease and ialysis Treat ent in ife, Aopin!

Strate!ies, Stren!th to Aontinue ivin!, and lastly, the 'ssess ent of their uality of

ife. "t has been concluded that the uality of life of the dialysis patients in the > ajor

dialysis units in @a boan!a Aity are not lo# as confir ed by the + scores and the

results of the in%depth intervie#.

'nother /ualitative study by Te ps&i et. al (2072) ad inistered the convenience

sa plin! ethodolo!y to fifty%si$ (<?) first (7 st) to si$th (? th) year edical students in si$

(?) edical schools fro the different re!ions in 4razil. The lives of the edical students

#ere e$plored utilizin! focus !roup discussions (EG s). The EG s #ere centered ainly

on the factors affectin! their /uality of life durin! edical school as #ell as their dealin!s

#ith hardships durin! their trainin!. 5esults revealed a variety of hardships durin! the

stay in edical school. Eactors reported that have decreased their /uality of life included

co petition, unprepared teachers, e$cessive activities, and edical school schedules that

de anded e$clusive dedication. The lac& of ti e for studyin! due to edical duties,

relationships, leisure and rest #ere considered the ain factors of influence. +n the

contrary, factors that increased /uality of life #ere !ood teachers, active learnin!

approaches, contact #ith patients, and efficient ti e ana!e ent. Students also reported

that eanin!ful relationships #ith fa ily e bers, friends, or teachers increased their

/uality of life.

' /ualitative research done by *anson et. al (2070) revie#ed the e$istin!

literature #hich inspected the functional i pact of cri e victi ization on the /uality of life %related edical, ental health, and e ployer costs, and health care utilization. The

revie# sho#ed that cri e victi ization influenced ultiple do ains such as parentin!

s&ills, reduced occupational functionin!, une ploy ent and inti ate relationships.

'nother research by Silva et. al (2072) done in 4razil ai ed to analyze the /uality

of life and ost%trau atic Stress isorder sy pto s of youn! adult in%patients #ho #ere

victi s of interpersonal firear violence. "nitially, one hundred forty%one (797)

individuals #ere included as respondents but only ninety%five (8<) finished the study.

Si ilar to the researcher s study, the W*+ + %45DE #as used to assess the overall

uality of ife of the respondents. +utco e of the study sho#ed that the trau as due to

interpersonal firear violence ay result in lo# /uality of life scores and hi!h

This study #as conducted at the +ffice of the 5e!ional rosecutor of the

epart ent of Hustice, the i ple entin! a!ency of the W S4 in 5e!ion "6, located at

the *all of Hustice in Sta. 4arbara, @a boan!a Aity.

The epart ent of Hustice ( +H) i ple ented a Witness rotection, Security and

4enefit ro!ra (W S4 ) pursuant to and consistent #ith the provisions of 5epublic 'ct

?8:7 other#ise &no#n as the IWitness rotection, Security and 4enefit 'ctJ created after

the Ddsa 5evolution durin! the presidency of Aorazon A. '/uino and enacted on 'pril

29, 7887. The +H ay call upon any depart ent, bureau, office or any other e$ecutive

a!ency to assist in the i ple entation of the W S4 and shall be under le!al duty and

obli!ation to render such assistance.

The World *ealth +r!anization (W*+) initiated the develop ent of a /uality of

life assess ent due to the need for a !enuinely international easure of /uality of life

and a co it ent to the continued pro otion of a holistic approach to health and health

care. 4y callin! for /uality of life assess ents in health care, attention is focused on this

aspect of health, and resultin! interventions #ill pay increased attention to this aspect of

patients1 #ell%bein! (W*+ 788:).

The World *ealth +r!anization uality of ife ; 45DE (W*+ + %45DE)

/uestionnaire #hich assessed the uality of life of the respondents #as utilized in this

study. The W*+ + %45DE /uestionnaire has t#o (2) parts. The first part of the/uestionnaire contained the socio%de o!raphic profile of the respondents #hile the

second part consisted of a 2?%ite /uestionnaire #hich focused on an individual s o#n

vie# of their #ell%bein!. "t contained t#o (2) ite s fro the +verall uality of life and

General *ealth and t#enty%four (29) ite s distributed a on! four (9) do ainsB physical

(- ite s), psycholo!ical (? ite s), social relationship (> ite s) and environ ent (:

"ntervie# Guide uestionnaireB

Eor the in depth intervie#, the researcher for ulated a set of !uide /uestions in an

open%ended free response for at ('ppendi$ *). This set of intervie# /uestionnaire

consisted of /uestions that focused on ho# the respondents describe their /uality of life

in ter s of their physical health, psycholo!ical health, social relationships and the

environ ent.

the conference roo s in the *all of Hustice.

The /uestionnaires #ere self%ad inistered to respondents #ith sufficient ability

#hile researcher%assisted /uestionnaires #ere ad inistered on respondents #ho had

difficulty understandin! and ans#erin! the listed ite s. uestions #ere read out and the

participants #rote do#n their respective ans#ers on their /uestionnaires. So e

respondents raised so e points for clarifications re!ardin! the survey /uestions, all of

#hich #ere ans#ered by the researcher.

'fter ans#erin! the /uestionnaire, the scores #ere co puted and these #ere

interpreted and classified as poor + , oderate + and !ood + . +f the forty%t#o

(92) #itnesses under the Witness rotection, Security and 4enefit ro!ra in 5e!ion "6,

thirty (>0) a!reed to participate in the survey. Ksin! the purposive sa plin! ethod, ten

(70) out of the thirty (>0) respondents #ere chosen for the in%depth intervie# based on

their + results. 's there #ere only t#o (2) respondents #ith poor + and four (9)

#ith oderate + the researcher decide to include all of the as sa ples. The

re ainin! four (9) #ere ta&en fro the t#enty ; four (29) #ith !ood + usin! lottery

ethod.

The in%depth intervie# #as scheduled dependin! on the respondents availability

and for security reasons the activity #as held privately in a conference roo in the *all

of Hustice. The intervie# #as carried out usin! a for ulated intervie# !uide

intervie#s #ere transcribed and analyzed data #ere referred bac& to the participants for

confir ation to ensure accuracy of the !athered data.

Data Ana"y$ $:

The results of the W*+ + %45DE /uestionnaire provided the uality of ife

scores. The ean score of ite s #ithin each do ain #as used to calculate the do ain

score. =ean scores #ere then ultiplied by 9 in order to a&e do ain scores co parable#ith the scores used in the W*+ + %700. ' ethod for the anual calculation of

individual score is sho#n in 'ppendi$ E.

5a# scores #ere converted after co putin! the do ain scores. The first

transfor ation ethod converts scores to ran!e 9%20, co parable #ith the W*+ + %

700. The second transfor ation ethod converts do ain scores to a 0%700 scale. '

ethod of convertin! ra# scores to transfor ed scores is sho#n in 'ppendi$ G.

The interpretation of + scores #as adapted fro the culturally validated

W*+ + %45DE by de e!a (200<). The cut%off for classifyin! + scores #ereB ::

collection, the entire intervie# #as recorded and transcribed #here patterns of

step #as done by reco!nizin! all data that related to the already classified patterns #hich

#ere e$pounded on. 'll of the tal& that fit under the specific pattern #ere identified and

placed #ith the correspondin! pattern, follo#ed by co binin! and catalo!uin! related

patterns into sub%the es. The es that e er!ed fro the infor ants1 stories #ere pieced

to!ether to for a co prehensive picture of their collective e$perience. Einally, a valid

ar!u ent for choosin! the the es #as built by readin! related literatures #here

infor ation #ere !ained by allo#in! inferences fro the intervie#s.

Ca$e Su++ary ,: Poor %ua" ty of # fe

5ia is a 7?%year%old fe ale #ho is bothered by her apprehensions about any

thin!s. She is the eldest a on! three (>) siblin!s. She has a ten (70) year%old%brother #ho

is a fourth (9 th) !rader and a t#o (2) year% old%sister. She !re# up in a s all house ade

of i$ed aterials in one of the slu areas in the city. *er father is a construction #or&er

#ho earns ( 2<0.00) a day and her other is a ve!etable vendor in the public ar&et in

the street of =a!ay. With her parents li ited inco e, their basic daily needs #ere not

al#ays et. 4ecause of this, she helped by sellin! ice candy in her school and used the

earnin!s for her school re/uire ents and daily allo#ance.

's a second year hi!h school student in a public school, she said that she

so eti es feels asha ed of attendin! classes because she #as often teased by her

class ates as bein! too old to be a sopho ore student. She said that #hen her father had

an accident in the construction site three (>) years a!o, she #as forced to stop !oin! to

school because she needed to ta&e care of her father, her siblin!s and at the sa e ti e

help her other earn a livin! for their fa ily.

't a very youn! a!e of thirteen (7>), 5ia already assu ed the role of an adult. She

did not have ti e to play li&e the rest of the children in their nei!hborhood. These events

in her life ade her sad and vie#ed life as useless and eanin!less. There #ere ti es

#hen she /uestioned God #hy these thin!s had to happen to her fa ily and #hy !ood

she could !o bac& to school, play #ith her friends and be #orry%free. Their life beca e

better co pared to the past years, but her e$periences in the past left a #ound in her heart

that could not easily be healed.

4ut fate is really playfulP just #hen thin!s #ere beco in! !ood for her, anune$pected event happened that ade her hate life a!ain. She #itnessed a cri e and #as

as&ed to testify before the court. 't a very youn!, these events #ere very trau atic for

her. There #ere ti es #hen she had flashbac&s of the cri e she #itnessed and #ould

often have ni!ht ares. She ad itted that it is really difficult to be a #itness since she is

al#ays filled #ith fears. *er o#n safety is al#ays priority that s #hy she avoided !oin!

to places #here the incident happened. She believes that she is no lon!er the sa e person

she used to be since she be!an hidin! hi self fro people.

*er #or&s at school and at ho e #ere also affected. She #as not able to

concentrate any ore and lost focus on #hat she does. She said that there #as one

instance #hen her father as&ed her to buy a lar!e%sized nail but #hen she arrived at ho e,

she realized that she #asn t able to buy #hat she #as told and instead bou!ht a s all%

sized nail. This caused her father to !et ad at her #hich ade her very sad.

5ia also beca e restless. She &ept #al&in! around the house and cannot finish her

#or&. She shared that one ti e #hen she #as doin! laundry her other cau!ht her

she believes that she is too youn! to !rasp all the stresses it brou!ht her. She said that

aybe it #as bad luc& that caused all these or aybe it #as just her #ho cannot handle

such proble s in life.

'naliza, is a >-%year%old public school teacher, arried and #ith &ids. She had

been arried for the past ten (70) years to a ilitary soldier. *er first born is a !irl #ho is

already nine (8) years old, her second child is a boy #ho is five (<) years old and she is at

the o ent three (>) onths pre!nant. *er fa ily rents an apart ent in a subdivision

so e#here in the Dast coast of this city. 5aised by her adoptive parents #ho are both

public teachers as #ell, she felt !rateful #ith the thin!s that she has ; !reat parents,

lovin! husband, #onderful &ids and a decent job.

't the a!e of nineteen (78) she learned that she #as adopted by her aunt and

uncle, #hen her biolo!ical other left her. Knli&e other people #ho beca e an!ry upon

&no#in! that they #ere adopted, 'naliza did not share the sa e senti ent. "nstead she

#as !rateful of her adoptive parents for ta&in! care of her and !ivin! her a !ood life. She

shared that she could not i a!ine life #ithout her nanay and tatay .

adjustin! to arried life #as hard. "t #as only #hen she !ot arried #hen she learned

about the differences she and her husband have. =onths after their #eddin! they startedfi!htin! over petty thin!s, she started to !et jealous over her husband s past lovers and

she &ept na!!in! hi until her husband burns out. 4ut believin! that nothin! in life is

per anent, a!ain, thin!s #ent s ooth for her.

interest in the thin!s that she does.

study provided a /ualitative depiction of the /uality of life of #itnesses under the said

pro!ra . This study focused on assessin! the overall /uality of life of #itnesses by

deter inin! their physical health, psycholo!ical health, social relationships and

interaction #ith the environ ent as #ell as to describe their life e$periences.

The results of the study sho#ed that ajority of the respondents have oderate

uality of ife scores follo#ed by respondents #ith !ood and poor + scores. These

findin!s #ere confir ed by the results of the in%depth intervie#s done a on! the ten (70)selected participants #ho have !ood, oderate and poor overall + scores. Three (>)

case su aries #ere ade to describe the life e$periences of #itnesses fro each of the

cate!ories. "n conclusion, the /uality of life of #itnesses in the W S4 is not lo#

contrary to notions that individuals #ho e$perience stresses that pose threats to life have

a poor /uality of life. The results can be attributed to havin! a !ood e otional and

support syste . "t is reco ended that future studies on the /uality of life should be

assessed every 9 #ee&s after the initial assess ent to deter ine the chan!es in their

uality of ife. *o#ever, future studies involvin! respondents #ith security issues are
