Africa is the youngest continent in the world

today which is developing very fast, hosting

about 1 billion people which is 14% of the world

population and very compatible with the modern

smart electronic age.

Today the increasing burden of kidney diseases and

end stage renal disease is a challenge to Africa with

following major risk factors such as Hypertension,

Glomerulonephritis, HIV, Malaria TB, Diabetes

Mellitus etc.

Renal replacement therapies include different

modalities like haemodialysis, peritoneal dialysis,

and renal transplantation. These are options, but

today the most practiced renal replacement therapy

in Africa is still haemodialysis.

Mediheal group of hospitals is establishing

haemodialysis units in every major towns of East

and Central Africa and very soon Mediheal is going

to bring Kidney Transplantation Units in Kenya and

other countries of sub-saharan Africa.

Quality and cost effective health care in super

specialty faculties of medical fraternity is the goal of

Mediheal and that shall lead to technology transfer

from the developed world, high end equipment

installation and ultimately establishment of

destination for medical tourism and travels.

We thank NHIF Kenya and government of Rwanda

for partnering with Mediheal in this noble cause.

Mediheal is already providing haemodialysis

services at Eldoret, Nakuru, Nairobi, Kigali, Addis

Ababa and very soon is going to other towns.

Kidney being one of the vital organs apart from

the heart, lungs, liver, and brain should be taken

seriously for preventive, diagnostic, curative and

prognostic measures of health care.

Dr. Frank Kamunde Mwongera is a well-known Physician and Kidney

specialist with over 35 years of experience. He got educated in Kenya,

United Kingdom and United States.

He served as a Chairman, Dept. of Medicine and Kidney Unit at KNH up to 1994.

He is heavily involved in care of kidney patients including transplantation and novel

therapies in the I.C.U. He is recognized as a Distinguished Fellow of European Dialysis and

Transplantation Society. He is a member of International Society of Nephrology and American

Society of Nephrology.

The expected rise in numbers of patients requiring

dialysis will also require easier access to kidney

transplantation; otherwise the dialysis facilities

will be overwhelmed by the sheer numbers

requiring dialysis.

Private dialysis units will also play an important

role in providing care for those patients who wish

to have this service privately. There are several

dialysis centers scattered all over the country which

have provided the backbone of renal replacement

therapy while these services have been insufficient

if not available.

Mediheal Dialysis Ltd. has recently embarked

on a countrywide expansion program so as to

supplement the dialysis capacity available in

the country. I am proud to announce that I am

specifically associated with this venture. Mediheal

Dialysis Ltd. ultimately aim to provide affordable

and quality dialysis to our citizens and those

• Blood in urine may appear as tea-colored urine

• Protein in urine: appear as frothy urine

• Increased frequency of passing urine, especially at night

• Swelling of legs

Earlier we detect, the higher is the chance of preventing more serious loss of kidney function.

• Pyelonephritis: infection of kidneys

• Glomerulonephritis: immunological damage to the kidneys

• Nephrotic syndrome: excessive loss of proteins in the urine

If kidney disease is not treated, the damage caused can be irreversible.

When your kidneys fail, you need treatment to replace the work normally performed by your kidneys.

When both the kidneys are irreversibly and totally damaged, the only option of survival is by Renal

Replacement Therapy (RRT).

Treatment Options for Renal Replacement Therapy (RRT):

A) Kidney Transplant

B) Dialysis

Kidney Failure

A) KIDNEY TRANSPLANT A kidney from a living donor (related or unrelated) or from a deceased donor (a person who has recently died) is removed and surgically placed into your body. The wait for a deceased donor kidney can be several years as there is a long waiting list. Living related donor kidneys have a better survival. Various factors need to be considered in matching kidneys with potential recipients.

You will need an assessment for fitnesstoundergokidneytransplantation.

• A transplanted kidney works like a normal kidney

• Helps to return you to a state of good health

• You do not need dialysis

• If you do not have a living donor, you may have to wait a considerable time for a deceased donor• It requires a major surgery• Your body may reject the new kidney, so the treatment may not last a lifetime• You will need to take anti-rejection medicine to maintain the transplanted kidney• Very high cost of kidney transplant

B) DIALYSIS

of the functions of the kidney.

There are two forms of dialysis:

• Peritoneal Dialysis

• Haemodialysis

Advantages of Kidney Transplant

Disadvantages of Kidney Transplant

It uses the peritoneal membrane as the filter

for dialysis. The peritoneal membrane lines

the wall of the inner abdomen and covers the

abdominal organs. The dialysis solution, which

isaspeciallypreparedfluid,isdrainedintothe

abdominal cavity through a special tube called

the peritoneal catheter.

Wasteproductsandextrafluidisremovedfrom

the blood into the dialysis solution. When the

dwell is completed, the ‘used’ dialysis solution

can then be drained out of the abdomen and

fresh dialysis solution is instilled.

There are two forms of peritoneal dialysis:

• CAPD (Continuous Ambulatory Peritoneal

Dialysis) is done manually

• APD (Automated Peritoneal Dialysis) is done

using a machine

Disadvantages of Peritoneal Dialysis

• Fourtofiveexchangesperdayareneededfor

CAPD

• A catheter leading from the abdominal cavity

to the outside is permanently present

• High chances of infection due to indwelling

catheter

In view of these disadvantages and high chances of infection, this form of dialysis is no

longer practiced routinely.

How is haemodialysis performed?

Duringhaemodialysisbloodisdrawnoutfromthebodyintotheartificialkidneywherewasteand

excessfluidareremovedandthecleansedbloodisthenreturnedtothebody.

B) HD Catheter:

Ifyouraccess(A_Vfistula)isnotready,youmay require a temporary form of access using a catheter placed into veins in the neck.

A) A_Vfistula:

A_Vfistula:In order to perform haemodialysis,

a surgically created opening between an

arteryandaveinknowasa‘fistula’isneeded.

This allows veins on the surface near the skin

to enlarge and blood can be drawn out and

returned through needles inserted into these

enlarged veins. If your veins are too small, a

graftusingartificialmaterialisneeded.

Blood Access

Advantages

• It is performed 3 times a week for usually 3 to 4 hours each time

• It is performed by nurses and dialysis care technicians in hospitals

• Doing haemodialysis in a dialysis centre or hospital allows for a social contact

• Chances of infection are minimal as compared to peritoneal dialysis

Disadvantages

• Youhaveafixedscheduleforyour treatment

• Youwillbeconfinedtoyourdialysischair for 4 hours

• Medications,fluidrestrictionanddietary control are necessary

machines for haemodialysis

• Sequential dialysis with different modes can be

done here, in which we remove only excess body
