POSITION OVERVIEW The position is responsible for individual support in a safe and secure residential environment for people who have experienced trauma, have multiple complex needs and/or a diagnosed mental health issue or disability. The position is responsible for daily support, crisis intervention, information provision, program development, structured daily activities, administering of medication, household duties along with a range of personal development programs supporting people to assist with their connection back into the community. • Act as an advocate andmediator with other relevant services and professionals in regard to theneedsofthepeoplewesupport.
• Adhere to both internal and external legislative requirements and protocols in relationship toreferral,assessmentanddirectcaretasks.
• Establish and maintain a strong working relationship with family, significant others, communitysupportoptionsappropriatetothepeoplethatwesupportneedsandparticipateoncommitteesandinworkgroupswhicharerelevanttotheworkoftheprogram.
PlanningandReporting
• Maintain case management recording systems including case planning documentation, case notes,reports and data collection in accordance with Ermha procedure, policies and data recordingrequirements.

• Offerprofessionalsupport topeoplethatwesupportwithinacasemanagement framework includingassessment & referral, establishment and implementation of community support and input into thedevelopmentandimplementationofIndividualProgramPlans(IPP).
• Provideinputtotheteam’sworkandotherErmhaProgramsactivelyparticipatinginthemaintenanceofa collaborative and harmonious professional environment, and operate in accordancewith the aims,objectives,philosophyandpoliciesofErmha.

• Otherdutiesasrequired.KEYSELECTIONCRITERIA1. A certificate IV in Disability orMental Health and/or with relevant work experience in the field of
mentalhealth, youthwork, the justice systemand/orworkingwith thosewhoaredisadvantaged inthecommunitywithparticularreferencetopeoplewithcomplexcareneeds.
2. Demonstrated awareness of the full range of issues experienced by people living with significantdisabilityasaresultofaseverementalillness,togetherwithhighlevelpracticeskillinthedeliveryofappropriateservices,strategiesandinterventionsisdesirable.
4. Clear communication skillsbothwrittenandverbal. Theability toprepare clear, thoughtfulwrittenreports,maintain file anddata requirements and the capacity to represent the programat both anagencyandcommunitylevel.
7. Knowledgeof and an ability to access, a rangeof relevant community resources, particularly in theareasofhealth,drugandalcohol,legal,incomesupport,employment,recreation.
8. Demonstrated ability tooperate as a teammember andapreparedness to accept ahighdegreeofaccountabilitytoco-workers,peoplewesupportandmanagement.
