3. Definitions and Notes

All references herein to “Specialist Contractor” shall where applicable be deemed to include all Contractors,Subcontractors, Trade or Works Contractors, their employees or others under their control who have access tosite.

All references to Principal Contractor refer to the Principal Contractor as defined in the Construction (Design &Management) Regulations. The Principal Contractor for each contract will be nominated by the client and mayor may not be Carillion

4. Standard

4.1. Introduction

Carillion is committed to achieving the highest standards in Health, Safety, Quality, Sustainability andEnvironment. We demand that Specialist Contractors share our commitment to achieving these higheststandards and to continuous improvement in all aspects of our work.

You will, in addition to complying with your legal duties, adopt the standards detailed within this documentas a minimum requirement and ensure that they are planned for in your submissions.

Failure to comply will jeopardise your continuing to work with Carillion.

It is the responsibility of each Specialist Contractor to effectively communicate both the context and thecontent of this document to all personnel working under their direction and to ensure that it is understoodand strictly followed.

This document will be incorporated into the pre-start meeting and is deemed to be accepted by theSpecialist Contractor, however, nothing in this document in any way relieves the Specialist Contractor ofits legal and contractual obligations to Carillion under the contract agreement or order, be they expressedor implied.

4.2. Vision and Values

4.3. The Carillion Culture

‘Target Zero’, Carillion’s safety campaign, was introduced to create a behaviour-based and employee-ledculture of ‘Everybody, Everywhere, Everyday’. The campaign has a target of zero accidentsand an uncompromising message:

“If it’s not safe - don’t do it”

Whilst our safety performance continues to improve, there is no room for complacency andsenior management routinely visit all our sites to check if they are safe. If not, specific activitiesor the whole site will be stopped immediately and will not be allowed to restart until the problems arecorrected. A number of activities have already been stopped on sites and if this involves you, it willseriously jeopardise your future work prospects with Carillion. Where consistently high levels of safetyperformance are demonstrated, this will raise the team profile and be recognised at the highest levelswithin Carillion.

Target Zero has a specific vision – to protect all of our employees and those working with us from the riskof occupational injury or ill health, through safe operating practices and behaviour.

All Specialist Contractors will be required to engage their workforce in active participation in our TargetZero programmes and initiatives.

4.4. Carillion Health and Safety Policy Statement

4.5. Don’t Walk By

· Don’t Walk By is about creating an environment of awareness and openness – one in which peoplehave the confidence to identify and challenge unsafe acts and conditions. It is about reaching thehearts and minds of people, changing attitudes and behaviour by ensuring that we all understand andare committed to the undisputed benefits of a safe working environment. It is also about the activepromotion of a process that is working particularly well and is contributing positively to our Target Zeroobjective.

· Achieving this demands the involvement of EVERYONE. That is why the Don’t Walk By Take ActionNow philosophy runs from the boardroom right through to our operatives, into our Specialist

Contractors and through every stage and function of our construction projects from tender to handover.

· We require that everyone working on a Carillion project including Specialist Contractors, who seeanything that they think is either unacceptable or could be improved, should raise it with the projectteam verbally or by using the Don’t Walk By card. The Don’t Walk By theme is not limited to Healthand Safety issues it can be used for any concerns including those relating to the Environment andQuality.

· Carillion will ensure that everyone raising a Don’t Walk By receives appropriate feedback and theproject will confirm how this will be done.

· The message you must give to give to your employees is that for every Don’t Walk By raised andactioned a potential accident is eliminated.

4.6. Communication and Consultation

Pre-Start Engagement (PSE) replaces the traditional project induction

All Specialist Contractor personnel are to attend a Carillion PSE before starting any work. Prior to attendingthe Carillion PSE, the Specialist Contractor shall make arrangements for all personnel to have attended asite-specific training/ briefing on their own Method Statement at which the hazards and risks associated withthe work and the control measures to be implemented are effectively communicated. Evidence that thistraining has been provided shall be given to Carillion. This includes all visitors, delivery drivers, plantmechanics etc who will require a PSE commensurate with their duties prior to entering the site.

The specialist contractor’s supervisors will be required to attend a specific “Supervisor’s Working withCarillion training”, to be delivered by either the project team or Carillion’s external provider. This briefing willensure that Carillion’s expectations and the roles and responsibilities of supervisors are understood by all.

In addition to the project specific PSE all mobile plant operators are required to attend the Carillion plantoperator’s specific induction.

A completed PSE Start Sheet, which includes any relevant Health issues, must be submitted to the inductorprior to receiving the Carillion PSE. Specialist contractors are responsible for ensuring that Carillion isadvised of any operative with a health issue that may impact on health & safety including diabetes, epilepsy,hearing impediment or taking prescribed drugs.

Point of Work Risk Assessments

Specialist Contractors will carry out a Point of Work Risk Assessment for every activity, everyday andwhenever the task or environment changes. This does not replace the activity risk assessment but is anassessment to confirm the task and environment do not present any unexpected hazards. These will berecorded on either Carillion’s format or a format agreed with Carillion.

In Carillion Rail completed assessments will be reviewed by a nominated person

Worker EngagementCarillion’s safety record continues to improve as a result of us being able to remove hazards from theworkplace, much of this improvement has been through the positive contributions from operatives to ourDon’t Walk By culture.

Specialist contractors are to encourage a high level of engagement with operatives in order that they workwith us to remove those hazards. Operatives have first hand experience of site conditions and are often thefirst to identify potential problems or safer methods of carrying out the work. Their involvement andparticipation in identifying and controlling hazards and risks is crucial to reducing the high accident rateassociated with construction work and in helping us achieve Target Zero.

Worker Engagement is about directly and proactively engaging with site operatives and using their skills andknowledge for this purpose.

Carillion is already considered a leader in recognising the benefits of engagement through Don’t Walk By.The Worker Engagement initiative takes this to the next logical level and provides operational staff with thetraining and support necessary to undertake effective engagement and make the best use of lessons learnt.

We will undertake engagement in the following ways:

· Ad hoc engagement with site operatives during unplanned events, such as site tours. This form ofengagement will allow us to determine facts and an understanding about contributory factors that affectthe behaviour of individuals.

· Planned engagement with new inductees to our projects and those who have been on our sites for aconsiderable time or who are departing. Learning from these groups and their broad range ofexperience will enable us to understand what more we can do to create better and safer workingenvironments.

· Group engagement, every other month we will select a topic, this could be a focus area, high risk areaor a subject highlighted through event reporting, and canvas feedback from all projects. This allows usto get a snap shot of the views across Carillion and to understand the issues relating to these topics andwhat local actions have been taken.

Carillion will ensure that where lessons are learnt, feedback on our actions and response is logged andcommunicated back to operatives, through the same process’s used for communicating feedback on Don’tWalk Bys.

Where wider benefits for the business are identified, the learning will be cascaded through our network ofSafety Action Groups for review at all projects and feedback communicated to the workforce through ourEngagement Action Bulletins.

As one of our Specialist Contractors we require you, as part of the risk assessment and method statementcommunication process, to ensure all your site personnel understand the concept of Worker Engagementand that an increased level of engagement will, when approached honestly and constructively, deliver realhealth and safety benefits to everyone on site.

Behaving Safely Strategy

Our approach to behavioural safety is to create a Health and Safety culture by influencing everyone so thatthe decisions they make and the actions they take are the safe ones.

We will achieve this by winning the hearts and minds of those we wish to influence, through continual andInspirational Leadership and the successful implementation of our of behavioural related processes.

The Behaving Safely strategy sets out 12 behavioural ‘standards’; four which should be displayed byeveryone, four more which should be displayed by supervisors and a further four to be displayed bymanagers. To support this we select a standard periodically and ask people on all our projects to answerquestions about the way they and other people behave, as part of a “Work Stoppage” exercise. It alsoprovides an opportunity for people to reflect on the safety of their working environment and the impact theyare having on others working adjacent.

Safety Action Groups (SAG)

All Carillion projects hold regular SAG meetings. Specialist Contractors are required to have representationand active participation from their employees at these meetings. The SAG will discuss and review siteissues with the prime objective of improving health and safety performance. It also provides a two waycommunication mechanism with the senior management of Carillion.

4.7. Leadership

Directors’ Leadership Tours

Carillion conducts Directors’ Leadership Tours that provide an important opportunity for senior managementto meet project staff with senior management from our Specialist Contractors and to engage in constructivedialogue

Influencing Behaviours

Through positive and proactive leadership, Carillion is determined to promote a positive Safety, Health,Welfare and Environmental culture to everyone on site and operate procedures that will provide a qualityproduct whilst recognising and rewarding good practice, as well as clearly identifying unacceptablebehaviour that will lead to penalties being imposed.

The Specialist Contractor must take a lead in changing behaviours and cooperate fully with Carillion in orderto ensure that everyone working on site is made aware of the behavioural expectations and the associatedreward and discipline procedures adopted for the project.

4.8. High Risk Areas

Carillion have defined our Top 8 High Risk Areas. These are the areas where accidents have the greatestpotential to cause significant harm. And any accident within one of the following categories is consideredhigh risk and will be investigated accordingly:

· All incidents in which somebody is injured by falling from height (any height other than slips/tripson the same level or off a kerb, etc)

· All incidents in which somebody is struck and injured by an object falling from height

· All incidents in which somebody is struck and injured by a moving vehicle/item of plant

· All incidents in which somebody is injured through contact with electricity

· All incidents in which somebody is injured through fire/explosion

· All incidents in which somebody is injured through being crushed or trapped by something

· All incidents in which somebody is asphyxiated/overcome by or suffers some other injury fromfumes, chemicals or lack of oxygen

· All incidents in which somebody suffers a penetrating injury or burn to the eye

Plus any incidents in which somebody is injured by some other event not listed above that wedetermine to have been a high risk event.

4.9. Drug and Alcohol Testing

Carillion recognises the negative impact drugs, either recreational, prescription or over the counter, oralcohol can have on an individuals performance and as such reserve the right to carryout drug and alcoholtesting in line with our procedures at the following times:

· Pre-commencement / commencement – prior to commencing work on site

· Post Incident – following accidents, incidents

· On Suspicion Testing – testing undertaken when there is suspicion someone is under theinfluence of drugs or alcohol.

· Random – as part of our commitment to randomly test 5% of our workforce annually.

Failure to comply with the testing regime will be considered a positive test result

4.10. Site Security and Access Arrangements

Carillion will provide access/security systems e.g. turnstiles, access registers, the Specialist Contractor andhis employees will comply at all times with the specific project procedure for access control. Randomsearch of persons/vehicles entering or leaving the site may be instigated at the discretion of the ProjectManager.

Data collated from the security access system may be used for fatigue management and/or theverification/support of contractual/commercial issues.

4.11. Sustainability and the Environment

It is Carillion’s policy to develop a culture of continualimprovement in our sustainability performance and thatkeeps us at the leading edge of sustainable development.The three pillars of sustainability; Social, Environment andEconomic, form an integral part of our 2020 SustainabilityStrategy. For Carillion, becoming a more sustainablebusiness goes hand in hand with becoming a moresuccessful business.

Carillion’s 2020 Sustainability Strategy is based around sixpositive outcomes of: Enabling Low Carbon Economies,Protecting the Environment, Supporting SustainableCommunities, Providing Better Prospects for our People,Leading the Way in our Sector and Building a SuccessfulBusiness.

We recognise that all of our activities have a direct impact on people, communities and the natural and builtenvironments. We seek to reduce and mitigate our negative impacts whilst increasing our positive impacts,therefore having a positive net benefit that leaves an overall lasting improvement. Our success in achievingthis is dependent on the co-operation of our Specialist Contractors who undertake a significant part of ourbusiness and we seek the active co-operation of all Employers, Suppliers, Subcontractors and all personnelto ensure this.

A Specialist Contractors working with Carillion must therefore demonstrate its commitment to providing theskills and resources needed to develop and deliver more sustainable solutions, specifically in the followingareas:

4.11.1 Enabling Low Carbon Economies

Contractors should:

· Understand and contribute to the initiatives and targets embedded into the project specific CarbonReduction Plan.

· Reduce project carbon emissions (both mains and generator use) as part of our Carbon Policyand the Carbon Reduction Commitment (CRC)

· Reduce miles travelled (both for deliveries and personnel travelling to work) investigating smarterways of working and travelling, recording those mile travelled in order to calculate a carbonfootprint.

· Reduce the embodied carbon of the materials that we use and where required report within anembodied carbon calculator.

4.11.2 Protecting the Environment

4.11.2.1 Environmental Impacts

· Carillion operates an Environmental Management System compliant with ISO14001 and issupported through an Environmental Policy statement. Specialist contractors are required todevelop and implement systems that support these systems/policies and the specificenvironmental measures outlined within the Contract Management Plan (or specificEnvironmental Management Plan).

· Specialist contractors should implement and contribute to the project’s Environmental Aspectsand Impacts Register, highlighting specific specialist risks associated with planned activities, anddetailing mitigation and control measures, and agreeing and managing these controls throughoutoperations.

The specialist contractor will:

· Prevent ground pollution through appropriate fuel/oil storage, use of well maintained plant andequipment, use of drip trays/nappies, refuelling regimes and controlling water runoff.

· Minimise air pollution through management of noise, dust and light.

· Protect natural features and wildlife. Recognise the local biodiversity and relevant BiodiversityAction Plans that are in place on the project, including supporting restoration measures.

· Minimise water consumption through use of water efficient processes and water savingtechnologies, and contribute to the project’s Water Reduction Plan.

4.11.2.2 Resource Use

· The specialist contractor will take measures and provide solutions that support the core objectiveof diverting all non-hazardous waste from landfill. Where this is considered unachievable,Carillion should be informed immediately together with an explanation of the measures proposedto maximise recovery as far as is possible.

· Contractors will recognise the waste hierarchy and demonstrate improvements to reduce theamount of waste generated in the first place. They are required to support measures to increasethe amount of waste reused, or if not possible recycled or recovered, and finding end-destinationswith high recovery rates.

The specialist contractor will:

· Comply with the project’s Site Waste Management Plan, and fulfil their ‘Duty of Care’requirements with regard to waste legislation.

· Provide waste predictions prior to commencement (including packaging waste), and disposalrecords throughout their operations. These records will detail waste stream (with EWC code),volume and destinations, and include reuse/recycling/recovery/landfill or other rates followingdisposal. (Volumes reused/recycled should also include whether this has occurred on or off site.)

· Follow as a minimum the site strategy around segregation of waste.

· Inform Carillion of any Hazardous substances which it intends to bring on to site and comply withthe Hazardous Waste Regulations for disposal.

· Identify opportunities to increase the recycled content of the materials used and provide quantitiesfor recycled content in major materials to be supplied, and offer for prior agreement alternativeswhere technically and/or commercially viable.

· Support the project in designing out waste through tools such as the WRAP toolkits (Waste andResource Action Programme).

4.11.2.3 Sustainable Procurement

· Contractors are required to be compliant with the Sustainable Timber Procurement Policy and theSustainable Procurement Policy which describe Carillion’s strategic commitment to sustainableprocurement, including how this should be identified and controlled by the subcontractor.

· Contractors will support Carillion where possible in progressing within the SustainableProcurement Task Force Flexible Framework

· All materials should be ethically and responsibly sourced.

4.11.3 Supporting Sustainable Communities

The specialist contractor will:

· Recognise the stakeholders and relevant concerns/issues and support Carillion’s engagementwith the local community in an appropriate manner as detailed in the Community Needs Plan.

· Contribute to the Project’s success in the Considerate Constructors Scheme by beingconsiderate, environmentally aware, having a clean and tidy work area, being a good neighbour,respectful, safe and responsible.

· Contribute to placing Carillion Apprentices within our supply chain

· Recognise and contribute to the local employment targets on the contract where possible andagreed.

· Contribute to the Business Action on Homelessness programme by supporting placements andwhere possible providing or assisting with employment opportunities.

On some projects, other sustainability requirements and specific targets set by the project client ordeveloper may also apply.

4.12. Specialist Contractor’s General Arrangements

The Specialist Contractor will ensure that Carillion has received the following information:

· The Specialist Contractor’s health & safety, environmental and quality policies· The name of the Specialist Contractor’s appointed director(s) with responsibility for health &

safety, environment and quality matters· The name and CV of the Specialist Contractor’s person(s) appointed to assist in complying with

health & safety, environment and quality matters· The name and CV of proposed key management personnel agreed.

A pre-commencement meeting will be held with every Specialist Contractor at which Project Specificrequirements including attendances will be discussed and agreed. The meeting will also review thedetailed arrangements to ensure that the Specialist Contractor complies with the requirements of thisdocument.

Specialist Contractors will co-operate with Carillion in discharging its responsibilities and hence they will:

1) Comply at all times with the Site Rules, Emergency Plan and procedures in force as set out in theCarillion Contract Management Plan (CMP)

2) Ensure that Carillion is provided with any information needed for the Health & Safety File and O&MManuals in an agreed format

3) Provide detailed responses relating to hazards and risks highlighted by Carillion at tender stage and inparticular, those identified in the Hazard assessment within the CMP

4) Prior to commencing on site and when changes are made or additional information is available,provide Carillion with:

a) An agreed General method statement with activity risk assessments which should include theoutputs of the following:i) Identify all occupational health and safety hazards, environmental aspects and quality issues

associated with its activities. This should be based on both the contractor’s specialistknowledge and the Hazard Matrix contained in the Carillion Contract Management Plan

ii) Carry out an initial assessment of the above items to identify those which are considered to beof significant risk, based on the likelihood of an incident occurring and its severity should itoccur

iii) Document formal risk assessments for all significant risks identified and the control measuresincluding monitoring procedures to be implemented to minimize the risk of an incidentoccurring

iv) Identify all occupational health and safety hazards, environmental aspects and quality issuesassociated with its activities. This should be based on both the contractor’s specialistknowledge and the Hazard Matrix contained in the Carillion Contract Management Plan

vii) Carry out site specific CoSHH assessments (product data sheets alone are not acceptable),maintain records of information, instruction and training to operatives and provide details toCarillion prior to using products on site

viii) Ensure a specific Point of Work Risk Assessment (POWRA) is undertaken for every task,every day or whenever there is a change to the task or environment.

b) A list of activities to be sub-let and details of all proposed sub-tier contractorsc) Supervisor’s competence pro-forma duly completedd) A Quality Plan and/or inspection & test plans duly developede) Start on site pro-forma, completed for all contractor’s personnelf) Schedule of Toolbox talks

5) Ensure that where Young Persons (person under the age of 18 years) are employed, either directly orthrough subcontractors you comply with your legal requirements for protecting the Health and Safetyof those Young Persons whilst at work. The specialist contractor must undertake a specific YoungPersons risk assessment, provide an appropriate level of supervision and agree the arrangementswith Carillion prior to bringing any young person onto the project

6) Ensure that where non-English speaking personnel are employed by them or their subcontractors theyprovide a competent interpreter in order that clear instruction can be given and effective consultationcan take place, this must include delivery of the Pre-Start Engagement . This person must be able toprovide direct competent communication with the non-English speaking workers at all times. Asuitable ratio agreed at tender stage of not more than 8 non-English speaking personnel to oneinterpreter, is to be provided

7) Provide suitable and sufficient site supervision to Carillion’s accepted standard, to control the works,apply, monitor and enforce the site rules with respect to Health and Safety, Quality and Environmentalstandards

8) Maintain a competent person manager/supervisor on site who is available to receive and implementsafety instructions from Carillion at all times when work is being undertaken by the SpecialistContractor. The manager/supervisor is to attend the informal daily coordination briefing (daily 4CsMeetings) and will also be expected to attend the Carillion Supervisors Induction as requested by theContract team.

9) Arrange for a professional safety adviser to conduct a safety inspection at agreed regular intervals. Acopy of the report is to be submitted to Carillion immediately following the inspection. When SpecialistContractors are visited by their in-house safety advisor or external safety consultant we ask, in thetrue spirit of Don’t Walk By, that they review safety across the site rather than restrict their attentionsto the particular areas where their activities are taking place. This will ensure best use of theprofessional safety advice visiting the project.

10) Have in place suitable arrangements to undertake health surveillance of all personnel at risk,appropriate to their work activities; eg Noise, Hand Arm Vibration Syndrome, Asbestos, Lead,Radiation

11) Have in place suitable arrangements to monitor the working hours of its personnel

12) Be responsible for the removal and safe disposal of its waste and surplus materials from the workarea and the maintenance of a clean and tidy work area. Should Carillion deem it necessary tointervene to maintain clean and tidy work areas, then the costs of this will be borne by the SpecialistContractor

13) Use designated access routes for vehicles and pedestrians and adhere to the security arrangementsimposed by Carillion.

14) Ensure the use of mobile phones on construction sites or in other hazardous environments isrestricted to emergencies and essential work related calls only. Persons must go to a safe refuge,away from work activities or plant/traffic and remain in that location whilst receiving or making calls

15) Where Specialist Contractors are responsible for the provision of temporary accommodation units,such units must be subject to a fire risk assessment and comply with the Joint Code of Practice – Fireprevention on Construction Sites.

16) Where specialist contractors employ people to prepare food on the Carillion site then they mustcomply with the Food Information for Consumers regulations.

5. Quality and Inspection & Test Plans

The Specialist Contractor is required to operate a Quality Assurance System that complies with therequirements of Carillion’s Quality Assurance System. This will include the development of inspection and testplans (I&TP) to ensure that the work is compliant with health, safety, environmental and quality requirements.

Plans must identify:

· all the significant items required by the specification, drawings and main contractor’s requirementsto make the work compliant

· what is being inspected

· the method of inspection

· acceptance criteria

· how often it is to be inspected

· who needs to inspect

· the record of inspection and where it is retained

The plan must be a separate document.

Based on the inspection and test plan, the specialist contractor shall prepare check sheet(s) on which ALLthe inspections required by the I&TP are included. These sheets shall be completed as the items towhich they relate are inspected, with the person confirming the item is acceptable by name or initials.

Carillion will be given access to audit the works and the test and inspection records as required throughthe duration of the contracted works

6. Safe Systems of Work

Inspections

Carillion requires that specialist contractors carry out and record regular safety inspections of both theirworkplaces and the plant/equipment used within the workplace. Evidence of these inspections is to beprovided to Carillion management and a visual record of inspections to be located at the workplace for safetycritical plant and equipment (e.g. mobile towers, MEWPs, scaffolds, harnesses etc). Plant and equipment is tobe clearly labelled to indicate who is responsible for managing the item in good order.

Underground Services

A particular and all too frequent event occurring on our projects is that of buried cables being struck by steelpins driven into the ground for setting out purposes (e.g. for kerb alignment, road barriers etc). This practice isbanned and steel setting out pins must not be driven into the ground, suitable alternatives are available forexample GRP pins or other solutions.

Pinsafe

Pinsafe offers a safe and innovative solution, whicheliminates the risks involved with the use of existingequipment.It can be used on any degree of ground, e.g.camber of road and unmade ground.

Steel pins cast in a concrete haunch, set intoexcavated ground and backfilled around

The only exception to this is where the pins form a fixed part of a proprietary system (e.g. the Varioguardtraffic safety barrier).

Where such products are to be used it must only be with the written consent of the Carillion OperationsDirector or equivalent and following a specific risk assessment detailing how identification and avoidance ofunderground services will be managed.

This will require more than usual services detection measures e.g. CAT scan etc. The assessment mustprovide total assurance that no services are in the area; for example by prior excavation under controlledconditions or raising ground levels by a suitable distancewith a ‘clear’ substrate.

Activities requiring Authorisation

The Specialist Contractor will notify Carillion and include within its method statements, all permit andauthorisation requirements associated with the works and will comply fully with the requirements.

Carillion will appoint a “Permit Coordinator” who will be a senior member of the Project Team. The followinglist gives examples of authorisations frequently required:

· Hot Works Permit· Permit to Disturb Ground· Confined Space, Permit to Enter· Permit to access high level works· Authorisation to pump fluids and/or discharge· Permit to Isolate, (electrically/mechanically live area)· Temporary Works, see below· Authorisation to work from ladders/step ladders

The authorisation to work from ladders/step ladders must demonstrate that other forms of access have beenconsidered and that ladders/step ladders are the most suitable type of equipment in the particularcircumstances when used under the control measures set out in the authorisation.

The permit holder must wear a permit holder vest and remain in attendance at all times whilst theactivity/operation covered by the permit is undertaken.

The example list is not exhaustive and there may be other “Specific” permits or authorisations that arerequired as a result of risk assessments.

Temporary Works

The Specialist Contractor shall advise all temporary work requirements to Carillion’s Temporary WorksCoordinator, for entry onto the Carillion Temporary Works Register.

Examples of temporary works:

• Excavations • Propping and shoring

• Tower crane and hoist installations • Foundation underpinning

• Fencing, hoardings & signs • Scaffolding

• Formwork • False work

• Temporary Glazing retention – temporary stitching of curtain wall glazing

Specialist Contractors must comply with the Carillion Group Temporary Works Policy and the two supportingdocuments, the Temporary Works Standard and the Temporary Works Process. These identify how tomanage the risks and hazards associated with Temporary Works and how they are identified, classified andcontrolled. All temporary works must be identified and notified to Carillion.

Other than Class C temporary works, as identified in the documents above, the design of all temporary worksmust be submitted to Carillion for checking. No temporary works may be loaded, accessed or used untilformal consent in writing has been issued by Carillion. All proposals will therefore need to be submitted ingood time.

Work at Height

The Specialist Contractors will identify within their method statements how they will justify and comply with thehierarchy of protective measures and of the controls it will implement with regard to all working at height.When establishing the safe system of work Carillion’s Work at height Equipment selector must be used.Controls are to include permit to access high level works where appropriate.

Where work is carried out within 2 metres of a leading edge where there are not specific measures in place toprevent materials or equipment from falling, all hand tools must have a lanyard fitted. Lanyards will be eithersecured to the operative or, in the case of larger items, be secured to an effective fixing point

Where mobile towers are used for access to work at height there must be a minimum of one PASMA orequivalent trained person as one of the people using the tower. It is not acceptable to have e.g. one PASMAtrained person overseeing use of multiple towers in use at the same time.

Manufacturer’s instructions must be available on site. Single width towers only allowed following Carillionagreement

Mobile Elevated Work Platforms – Should have anti-crush over the controls and “alarms” if obstructions arepresent

Ladders / Steps to be used for short duration one man jobs minimum tools and materials – should beconsidered as a last “resort” – must be covered by issue of a ladder authorisation / Tag / Checklist – must be“Class 1” for industrial use and under inspection / checks – work must be short term (no more than 10minutes)

Whilst not a principal means of controlling work at height risk, the Specialist Contractors must establish andmanage exclusion zones where there is a residual risk of falling objects as a result of their activities on site.Exclusion zones must be suitably robust, fenced, with adequate warning signage, have strictly controlledaccess (e.g. tally boards) and be fully maintained by the contractor.

When employed in work at height and where personal fall restraint or arrest equipment is used, it is essentialthat an effective rescue plan has been developed and that personnel receive training in rescue techniques andFirst Aid.

Specialist Contractors are to include within their method statements the arrangements for the safe unloadingof materials from delivery vehicles. Where there is a risk thatmaterials may fall from the sides of the delivery vehicle or thatunloading operations may knock or shift materials from thedelivery vehicle, control measures must be adopted including:

Vertical restraints such as ‘King Posts’ must be fitted to thedelivery vehicle (see photograph)

Each individual layer of loaded material must be independentlysecured to the delivery vehicle, so that lower materials are notdislodged whilst upper ones are removed.

Where a Specialist Contractor provides or arranges for the provision of any of the following they must complywith the Carillion standards:

Plant Pedestrian Segregation

The Specialist Contractor will comply with Carillion’s hierarchy with regard to plant pedestrian segregation:

1. Barrier all plant to prevent pedestrian access

2. Minimum segregation of plant/vehicles and pedestrians by 3 metres (3m Rule)

3. All vehicles are to be fitted with devices to give all around vision or be guided by a suitably qualifiedperson if there is a need to reverse.

4. Banksmen control of a single machine at all times- static and in transit – all pedestrians excluded fromwork area

Notwithstanding the above, whatever approach is in place plant operators and pedestrians must comply withCarillion’s “Thumbs Up” initiative

“Thumbs Up” – Our Approach to Approaching Plant

In addition to segregation/proximity warning systems, Carillion requires that pedestrians wishing to approachplant and Plant the Operator, to adopt its ‘Thumbs Up” initiative.

“Thumbs Up” requires anyone wishing to approach the working area or risk zone of plant to make theirintentions clear by attracting the driver’s attention and giving them a “Thumbs Up” gesture. This may beengineers, Supervisors or Managers entering the area for example; but also includes anyone wishing to workon the machine; for example to fit or remove lifting equipment.

Pedestrians must not approach further until the machine is in a safe condition and this is confirmed by theoperator returning a “Thumbs Up” gesture. Where the item of plant being approached includes a cabarrangement or restricted visibility pedestrians must only approach from the cab/visible side.

‘Safe condition’ requires the Plant Operator to:

· place moving elements of the plant into a neutral safe position where those approaching simplyrequire entry into the area for example excavator arms must be lowered to the ground

· engage the deadman handle or isolate the machine from movement by e.g. turning it off

· acknowledge that this action is complete by returning a “Thumbs Up” gesture

· Only take plant out of the safe condition once parties have discussed working arrangements andcontrols are in place.

Or

· place the machine into a suitable position where those approaching wish to work on the plant, forexample to fit or remove lifting equipment

· engage the deadman handle or isolate the machine from movement e.g. by turning it off

· acknowledge that this action is complete by returning a “Thumbs Up” gesture

· only take plant out of the safe condition once pedestrians have left the risk zone of the machine.

In both situations the operation for pedestrians leaving the working area/risk zone should reverse the process– by letting plant operators know their intention by signalling a “Thumbs Up” gesture and the operator returningthe signal.

7. Events (Accidents and incidents)

The Specialist Contractor will report all accidents, incidents, near misses, environmental and quality incidentshowever trivial to Carillion site management as soon as they are aware of them. In the case of accidents withthe potential to result in lost time, updates shall be given as required until the IP is fit for work.

In addition, all reportable accidents or dangerous occurrences as defined in RIDDOR, must be reported to theappropriate enforcing authority (HSE or ORR) by the Specialist Contractor. When reporting an incident, thereis a requirement to explain what actions have been taken to prevent a recurrence. It is important that carefulconsideration is given to this response in order to demonstrate that the situation is being properly managed.

In the event of a death you must notify the Health and Safety Executive without delay.

The Specialist Contractor will investigate all accidents and incidents; in particular those accidents resulting inlost time and those which occur in one of the Top 8 High Risk Areas, and ensure a report is submitted to theCarillion site management as soon as possible detailing the actions taken to prevent a recurrence.

8. Competence and Training

Carillion is committed to achieving a fully certified workforce on all its sites. This includes managers,supervisors, plant operators and site operatives. Carillion supports the Construction Skills Certification

Scheme (CSCS) including its affiliated schemes and ALLMI training in relation to lorry loaders and Hiabs.Carillion requires that Specialist Contractors who wish to work with us commit to providing a workforce whichis fully and appropriately carded prior to any order being placed.

It is the Specialist Contractor’s responsibility to ensure that all their employees have received suitable trainingso that they can carry out their work safely, with due regard for the environment and to the desired quality. Itmust also agree to release employees to attend Carillion site training sessions as and when required.

Specialist Contractors whose activities involve disturbing ground are to ensure that the permit receiver isdemonstrably competent ((eg to NVQ Level 3 Gold Skilled Worker CSCS Card for Building, Energy and UtilitySkill Registration Scheme (EUSR) card for CNS and the Appropriate CSCS card for Civils and Rail). Thefollowing groups of personnel must also have attended the Carillion training module for underground services:

· supervisor controlling works of disturbing ground· holders of a “Permit to Disturb Ground”· individuals charged with operating a Cable Avoidance Tool (CAT) and Generator

Carillion will only allow plant to be operated by a driver holding an appropriate card covered by theConstruction Plant Certification Scheme (CPCS) or affiliated scheme, with the exception of HIAB/lorry loaderswhere ALLMI competence cards will be accepted.

The Specialist Contractor will provide evidence of the training and competence of each of its employees,appropriate to the activities and tasks being undertaken. Failure to provide evidence of appropriate trainingwill result in access to site being denied. Where this training is in-house Carillion reserves the right to attendand monitor this training.

A director of the specialist contractor will be required to endorse and return to Carillion, a CarillionCompetence of Specialist Contractors Supervisors form for each supervisor employed directly or for 2nd andsubsequent tier Specialist Contractors. Carillion reserves the right to withhold consent to the deployment of anindividual supervisor if their competence to fulfil their managerial or supervisory role is not adequatelydemonstrated. All such supervisors must have attended Carillion’s Supervisor Training and meet therequirements outlined in Appendix A of this document. Supervisors will normally be identifiable by wearing ablack helmet and on Rail contracts by a reflective arm band.

Carillion Training Services is accredited to deliver craft training programmes within the Government’s ModernApprentice Framework. Specialist Contractors are to commit to providing local work experience opportunitieswhere appropriate as part of the apprentice scheme.

9. Tools, plant and equipment

All tools, plant and lifting equipment used by Specialist Contractors shall comply with statutory requirements.Specialist Contractors must satisfy themselves that the tools, plant or equipment are fit for purpose, that theyare maintained and in satisfactory condition and that Test / Examination Certificates, insurance documentationand registers are available on site. The procedure for the procurement of mobile plant must comply withCarillion’s Plant Safety Assessment System.

All plant and equipment used by the Specialist Contractor must only be operated by competent persons withan appropriate CPCS card or approved affiliated equivalent and who have also received familiarisation trainingfor the specific item of plant/equipment being used. Within 3 years of first qualifying and at intervals notexceeding 3 years thereafter, forklift/telehandler operators must attend a one day refresher training with anapproved training provider. All mobile plant operators are required to attend the additional plant operatorinduction on each Carillion site where they are to operate plant. It is a mandatory requirement for all steelerectors and net riggers working on our sites to hold an IPAF PAL+ qualification.

Specifically for Quick-Hitch attachments, user manuals must be available in the cab of the vehicle and theoperators must have received specific training on the Quick-Hitch system being used. This information mustbe documented and records maintained on site.

The preferred type of quick hitch mechanism to be used on Carillion sites is the fully automatic type that locksonto both bucket pins. Alternative types of quick hitch mechanism will be accepted provided their use hasbeen justified and adequate controls are in place to prevent attachments from becoming unintentionally

detached. In the case of semi-automatic quick hitches, these must be fitted with a permanently retained ornon-removable safety pin such that when it is disengaged it is clearly visible to both the operator of themachine and those working in the vicinity. Manually operated hitch attachments are also acceptable.

The following requirements apply whatever type of quick hitch mechanism is used:

· there must be a secondary mechanical interlock to prevent the latch from being accidentallyreleased

· quick hitch activation controls must be designed to prevent unintentional activation e.g. bysimultaneous activation of two controls/switches or the activation of a control/switch with a safetyfeature that requires a continuous positive action to override e.g. a locking safety switch

· all quick hitch attachments must be compatible with each other.

The plant operator must also sign the Plant Operator’s Quick Hitch Safety Declaration form to confirm theyhave received training on the specific quick hitch assembly fitted to his machine, commits to complying withthe inspections and operational requirements in line with the training received and to stop work should anyoneencroach into the swing radius of the machine.

Any mobile plant which when used has the potential to overturn will be fitted with a roll over protection system(ROPS). If such plant has the potential for persons being carried to be thrown from their seat then seatrestraints will be fitted and their use enforced.

All plant and equipment brought on site by Specialist Contractors must be such that the emission of noise shallbe reduced to the lowest possible levels.

All equipment brought on to site by Specialist Contractors where there is a risk from vibration to those using it,must be accompanied by a suitable and sufficient risk assessment. The assessment must cover as aminimum, control measures to mitigate the risk, health surveillance for employees and what information,instruction and training has been provided.

Articulated tipping road vehicles are not to be brought onto site without written agreement from the relevantOperations Director. Articulated dump trucks are permitted subject to agreement with Carillion regarding asafe system of work.

Plant is to have visibility aids such as mirrors, CCTV etc to ensure compliance with the requirements for allround visibility.

Where a MEWP is being used, there must be an attendant operative equipped with a dry powder fireextinguisher at ground level to react in an emergency.

Oxy-acetylene bottles are to be moved on a proprietary trolley, which will have an appropriate fire extinguisherheld on it. Acetylene bottles are to be removed from site when they are no longer required. All fuel andoxygen bottles are to be fitted with appropriate flashback arrestors. All flexible gas lines are to meet therelevant BS.

Fire extinguishers are to be serviced at not greater than annual intervals and visually checked weekly

Full screening to arc welding is required where controlled access to the welding area is not possible byrestricting access.

Barriers are required to be placed around pipe threading machines, bench mounted wood and metal workingmachine areas and band saws.

Site hoardings or fences provided by the contractor around any external works should comply with the Carillioncorporate image and be provided whenever possible to prevent access by unauthorised persons. This fenceis to be at least 2m high and be such that it is not easily climbed i.e. close boarded or mesh not exceeding30mm in size. The design for such fencing must comply with Carillion’s Temporary Works Procedure andhave a design check completed.

All temporary protection supplied by the Specialist Contractor is to be in accordance with Loss PreventionStandard 1207 i.e. floor protection, window and wall coverings are to be to the appropriate fire protectionstandard.

In areas where there is any risk of a person striking or falling onto reinforcement starter bars or similar, thesemust be protected with suitable caps or boxing. Where the risk is of a person falling on to the bars theprotection must be suitable to withstand the weight/force e.g. reinforced caps or timber boxing.

Exposed fixed blade craft knives such as a Stanley knife are not permitted on Carillion sites; all tools of thatnature must be the self retracting safety utility knife type.

Brickwork and blockwork contractors shall use safety wall ties unless otherwise specified by the designer.

Toolboxes/chests must be safe for use. The uncontrolled closing of lids must be prevented by the use ofhydraulic /pneumatic (gas) stays with mechanical lock open capability to prevent injury in the event of a failure.

Specific measures must be in place for delivery drivers to be able to demonstrate at the site that there is all thenecessary legal inspection documents (12 months lifting equipment, 6 months for lifting accessories) availableand that the driver has the necessary competence certification to operate any on board unloading equipmentor devices.

All mobile plant and vehicles will be operated in compliance with Carillion’s Reversing Policy i.e. for all vehiclesother than cars or car derived vans all reversing must be controlled by a vehicle marshal, unless it has beenspecifically risk assessed out and alternative controls put in place.

All deliveries will be arranged in compliance with the requirements and restrictions of Carillion’s ProjectLogistics Plan

10. Lighting and Temporary Electrics

All services installed underground, whether temporary or part of the permanent works must be identified withsuitable marker tape above the service along the entire length. Marking must be added during initial backfilloperations.

Only competent and authorised persons are to adapt or move the temporary electrical installation and generalarea safety lighting. Specialist Contractors are responsible for providing and maintaining their own tasklighting to a sufficient level that the operation can be carried out safely and to the required standard ofworkmanship.

Halogen lights are not to be used unless properly mounted on a rigid structure away from people andpotentially flammable materials.

All temporary electrical cabling shall be routed to avoid creating a trip hazard and where practicable the cableswill be suspended at high level.

Portable or semi-portable electric tools and temporary lighting must not be in excess of 110v, without specificpermission of site management. Dual voltage generators are not permitted on site.

All electrical tools, leads and equipment will have an auditable visual inspection tagging system showing thedate of test and when the next test is due. The arrangements for Portable Appliance Testing (PAT) and otherregular inspection must be risk assessed and included within the activity method statement.

11. Personal protective equipment & Dress Code

The Specialist Contractor will supply its employees and manage the wearing of the following items of PersonalProtective Equipment that are to be maintained to a good standard at all times:

Protective Footwear – appropriate footwear with toe and mid-sole protection.

Head Protection – suitable and appropriate for the work being carried out and the workplace environment.Chin straps to be worn where there is a risk of helmets falling or being blown off.

Headwear such as ‘beanie’ hats or hoods are not allowed under head protection. To tackle colder conditionsonly proprietary helmet liners are permitted.

Hand protection – the wearing of hand protection is mandatory on site. Gloves must be appropriate for thetask or tasks being carried.

High Visibility Top - either vest or jacket as appropriate must be worn at all times as an outer garment.

Eye protection - the wearing of light eye protection on site is mandatory. In addition the Specialist Contractormust assess the specific needs of persons who wear prescription glasses. Over-glasses may not be the mostappropriate solution and prescription safety glasses must be considered as the preferred option. Where therisk assessment process identifies the need for a higher level of protection, it is the responsibility of theSpecialist Contractor to supply suitable eye protection for the identified risk and ensure that it is used correctly.

Hearing protection - where people are exposed to noise where levels cannot be reduced below the thresholdlevels, then suitable hearing protection must be provided and worn.

Respiratory protection – as identified by Risk / CoSHH assessment where workers are exposed topotentially harmful airborne contaminants (smoke, fumes, vapours or dusts). Where the risk is of dustinhalation then a face fit test must be carried out for the specific type and manufacture of respirator worn(minimum accepted classification FFP3).

Safety harnesses - all safety harnesses are to be full body type. Lanyards used in conjunction withharnesses will, in the first instance, be of a type that positions the wearer and prevents the fall rather thanarresting a person following a fall. Inertia reels used in conjunction with lanyards shall only be used inaccordance with the manufacturer’s recommendations. In particular they should only be used in the verticalplane unless the equipment is designed for use in the horizontal plane. Where harnesses are used as theprimary means of fall protection and it is necessary to disconnect lanyards to facilitate movement, then adouble lanyard system will be used.

Winter Conditions –Cold, wet or icy conditions require specific considerations for all PPE be it for waterprooffootwear and clothing; insulating layers, helmet liners etc. Where operatives will be exposed to suchconditions it is expected risk assessment will determine any adjustments to or special requirements of PPEnecessary.

Flame Retardant PPE

Fire Retardant Clothing may be required if working in the chemical or petro-chemical environments or wherework entails hot works or a risk of striking underground apparatus (based on risk assessment / job hazardanalysis). The material from which the garments are made shall pass either of the two tests outlined below:

The Box Arc Test (EN IEC 61482-1-2) - mainly used in Europe to measure the protective performanceof fabrics and clothing against the thermal effects of an electric arc.

The Open Arc Test (EN IEC 61482-1 or ASTM 1959-F) - mainly used in North America but is becomingmore frequently used in Europe.

In both case a fabric passes the test if the heat being transferred behind the fabric would not cause 2nddegree burns to the wearer.

It is important that the fire retardant clothing remains properly fastened at all times during the work; for thatreason clothing with only Velcro as fasteners shall not be used. Zip fastenings offer much better protectionagainst the clothing accidentally bursting open during exertion by the wearer.

The Specialist Contractor shall ensure that all PPE is maintained in line with manufacturer’s instructions, in agood condition, does not display inappropriate branding and is tested where appropriate.

Carillion operates a minimum dress code for all persons whilst in the operational areas of the site.

You must ensure that the minimum standards followed are:

Shirts/T-Shirts - no sleeveless tops will be permitted; shoulders and upper arms must be protected at alltimes

Long Trousers - no shorts, regardless of length, will be permitted. Only full length trousers may be worn.

The policy is designed to reduce the number of accidents on our sites and reduce the risk of skin cancer tothose working outdoors.

12. Scaffolding & edge protection expectations

Carillion maintains robust policies and guidance including scaffolding, edge protection, high rise constructionetc. Specialist contractors are to familiarise themselves with these documents insofar as they impact on thework activities of the specialist contractor.

Where the Specialist Contractor engages the services of a scaffolding contractor, the following Carillionconditions shall apply:

· Only scaffold contractors on the Carillion’s My Register may be employed to undertake scaffoldingrequirements on Carillion projects

· Only one of the nominated “Preferred Scaffolding Contractors” will be used on a particular Project.The contractor will operate to the Carillion Group Standard for scaffolding.

· Only one of the nominated “Preferred Edge Protection Contractors” will be used on a particularProject. The Contractor will operate to the Carillion “Edge Protection Requirements” standard.

· A proper design brief is to be agreed between parties so requirements are clearly understood.· A design is produced which meets the design brief and the practical issues are clarified.· The design is checked and approved to suit procedures of both parties.· The design information is passed on to the erection crew together with key issues clarified where

reference must be made back to the design.· The erection crew signs off that the work had been completed to the design.· A nominated competent person inspects the completed work and hands over a signed and dated

copy, which makes specific reference to any variations from design and limitations imposed.· Where applicable, reference should be made to preferred hook-on points for fall arrest equipment.· Note should be made of any specific requirement in striking the scaffold/edge protection and any

limitation on intermediate loading points.

13. Lifting operations including use of 'Flexible Intermediate Bulk Containers (FIBCs)

All lifting operations must be notified to the Carillion lifting coordinator. Hired lifting equipment should only besourced from approved, accredited suppliers. Specialist contractors must only use cranes supplied byCarillion approved suppliers. Contractor owned cranes cannot be used unless the contractor is an approvedcrane supplier.

Lifting Operation plans must be drawn up by a competent person (Appointed Person) and include details ofindividual items to be lifted and any special lifting arrangements required by the Specialist Contractor. This

information must be supplied to and reviewed by the Carillion lifting coordinator prior to the event to ensurethat a safe system of work has been agreed as part of the overall lifting plan

Only competent persons are to carry out duties associated with lifting operations and where the specialistcontractor provides the Appointed Person, confirmation of training and competence must be presented to ourproject management. Specialist Contractors engaged in lifting operations on site must have a CraneSupervisor authorised via the Project Lifting Plan. The Specialist Contractor’s Crane Supervisor must beapproved by and report on lifting matters to the Project Appointed Person.

FIBCs, (sometimes called bulk bags or One Trip Bags) are commonly used in the construction industry for thetransportation of bulk granular materials such as sand or aggregates. They must only be utilised for the singletrip as intended and never used to transport steel reinforcement links, construction waste or other materialswith sharp edges such as rebar and scaffold tube off cuts that can cut the fabric. The Carillion guidance onthe use of FIBCs must be complied with at all times.

14. Welfare and First Aid

Where the Specialist Contractor provides welfare facilities, they must meet the Carillion standard and bemaintained to a high standard.

Specialist Contractors are to provide sufficient first aid facilities and the appropriate number of qualified firstaiders to comply with the requirements of the Approved Code of Practice and Guidance for The Health andSafety (First Aid) Regulations, having duly assessed their workforce and work activities. Carillion is to be keptinformed of the current provisions in place.

15. HealthSpecialist Contractor will be required to demonstrate compliance with Carillion’s Occupational Health Strategy“Health Like Safety” in particular where they employ Safety Critical Workers they will be required todemonstrate that they are fit for work before they start commence work on site.

Specialist Contractor will be required to demonstrate that they have in place suitable procedures to ensure thehealth of their workforce. As part of those procedures, they will need to show that appropriate healthsurveillance measures are in place to identify any harmful effects at the earliest possible opportunity. Thesemeasures should include, but not be restricted to the following:

· Noise· Manual Handling· Musculoskeletal disorders· Hand arm vibration syndrome (HAVS)· Full body vibration· CoSHH· Respiratory disease prevention· Display Screen Equipment Assessments.

16. Fatigue

When working on Carillion Rail projects sub contractors are to comply with NR/L2/ERG/003 - Management ofFatigue: Control of Working Hours for Staff undertaking Safety Critical Work and NR/GN/INI/001 - Guidance on themanagement of door to door work and travel time – the key requirements being

· No more than twelve hours to be worked per period of duty/shift.· No more than seventy two hours to be worked in a seven day period.· A minimum of twelve hours rest between booking off from a period of duty/shift to booking on for the next

period of duty/shift.· No more than thirteen periods of duty to be worked in any fourteen day period.
