4.  MEDICAL CONDITION: HEART ATTACK

Cheney's long histories of cardiovascular disease and periodic need for urgent health care raised

questions of whether he was medically fit to serve in public office. Having smoked up to three or more

packs of cigarettes for nearly 20 years, Cheney sustained the first of five heart attacks in 1978, at age 37.

Subsequent attacks in 1984, 1988, 2000, and 2010 have resulted in moderate contractile dysfunction of 

his left ventricle. He underwent four-vessel coronary artery bypass grafting in 1988, coronary artery

stenting in November 2000, urgent coronary balloon angioplasty in March 2001, and the implantation of 

an implantable cardioverter-defibrillator in June, 2001.

On September 24, 2005, Cheney underwent a six-

hour endo-vascular procedure to repair popliteal

artery aneurysms bilaterally, a catheter treatment

technique used in the artery behind each knee. The

condition was discovered at a regular physical in July,

and was not life-threatening. Cheney was hospitalized

for tests after experiencing shortness of breath five

months later. In late April 2006, an ultrasound revealed

that the clot was smaller.

On March 5, 2007, Cheney was treated for deep-vein

thrombosis in his left leg at George Washington

University Hospital after experiencing pain in his left

calf. Doctors prescribed blood-thinning medication and allowed him to return to work. CBS News

reported that during the morning of November 26, 2007, Cheney was diagnosed with atrial

fibrillation and underwent treatment that afternoon.

On July 12, 2008, Cheney underwent a cardiological exam; doctors reported that his heartbeat was

normal for a 67-year-old man with a history of heart problems. As part of his annual checkup, he was

administered an electrocardiogram and radiological imaging of the stents placed in the arteries behind

his knees in 2005. Doctors said that Cheney had not experienced any recurrence of atrial fibrillation and

that his special pacemaker had neither detected nor treated any arrhythmia. On October 15, 2008,

Cheney returned to the hospital briefly to treat a minor irregularity.

On January 19, 2009, Cheney strained his back "while moving boxes into his new house". As a

consequence, he was in a wheelchair for two days, including his attendance at the 2009 United States

presidential inauguration. 

On February 22, 2010, Cheney was admitted to George Washington

University Hospital after experiencing chest pains. A spokesperson

later said Cheney had experienced a mild heart attack after doctors

had run tests. On June 25, 2010, Cheney was admitted to George

Washington University Hospital after reporting discomfort.

In early July 2010, Cheney was outfitted with a left-ventricular assist

device (LVAD) at Inova Fairfax Heart and Vascular Institute to

compensate for worsening congestive heart failure. The device

pumped blood continuously through his body. He was released from

Inova on August 9, 2010, and had to decide whether to seek a

full heart transplant. This pump wascentrifugal and as a result he

remained alive without a pulse for nearly fifteen months.

On March 24, 2012, Cheney underwent a seven-hour heart transplant procedure at Inova Fairfax

Hospital in Falls Church, Virginia, at the age of 71. He had been on a waiting list for more than 20 months

before receiving the heart from an anonymous donor. Cheney's principal cardiologist, Dr. Jonathan

Reiner, advised his patient that "it would not be unreasonable for an otherwise healthy 71-year-old man

to expect to live another 10 years" with a transplant, saying in a family-authorized interview that he

considered Cheney to be otherwise healthy.

Heart Attack 

If the heart muscle does not have enough blood (and consequently oxygen) it dies and a heart

attack occurs. Another name for a heart attack is myocardial infarction, cardiac

infarction and coronary thrombosis. According to Medilexicon's medical dictionary, a heart

attack is "infarction of a segment of heart muscle, usually due to occlusion of a coronary artery" .

(Infarction = the process whereby an area of dead tissue is caused by a loss of blood supply).

A heart attack usually happens when a blood

clot develops in one of the blood vessels

that lead to the heart muscle (coronary

arteries). The clot, if it is big enough, can

stop the supply of blood to the heart. Blood

supply to the heart can also be undermined

if the artery suddenly narrows, as in a

spasm. 

The most common symptom of heart attack 

is chest pain. The most common

complications are heart failure and

ventricular fibrillation.

Risk factors for atherosclerosis and heart

attack include elevated cholesterol levels,

increased blood pressure, tobacco

use, diabetes, male gender, and a family history of heart attacks at an early age.

Heart attacks are diagnosed with electrocardiograms and measurement of cardiac enzymes in

 blood.

Early reopening of blocked coronary arteries reduces the amount of damage to the heart and

improves the prognosis for a heart attack.

Medical treatment for heart attacks may include antiplatelet, anticoagulant, and clot dissolving

drugs as well as angiotensin converting enzyme (ACE) inhibitors, beta blockers, and oxygen.

Interventional treatment for heart attacks may include coronary angiography with percutaneous

transluminal coronary angioplasty (PTCA), coronary artery stents, and coronary artery bypass

grafting (CABG).

Patients suffering a heart attack are hospitalized for several days to detect heart rhythm

disturbances, shortness of breath, and chest pain.

Further heart attacks can be prevented by aspirin, beta blockers, ACE inhibitors, discontinuing

smoking, weight reduction, exercise, good control of blood pressure and diabetes, following a

low cholesterol and low saturated fat diet that is high in omega-3-fatty acids, takingmultivitamins with an increased amount of folic acid, decreasing LDL cholesterol, and

increasing HDL cholesterol.

1.  MEDICAL CONDITION: EMPHYSEMA

On 23 June 2008, Winehouse's publicist corrected earlier misstatements by Mitch Winehouse that

his daughter had early stage emphysema,

instead claiming she had signs of what could

lead to early-stage emphysema. Mitch

Winehouse had also stated that his

daughter's lungs were operating at 70 percent

capacity and that she had an irregular

heartbeat. Mitch Winehouse said that these

problems had been caused by her chain

smoking crack cocaine. The singer's father also

reported that doctors had warned Winehouse

that, if she continued smoking crack cocaine, she would have to wear an oxygen mask and would

eventually die. In a radio interview, Mitch Winehouse said the singer was responding "fabulously" to

treatment, which included being covered with nicotine patches. British Lung Foundation spokesman

Keith Prowse noted this type of condition could be managed with treatment. Prowse also said the

condition was not normal for a person her age but "heavy smoking and inhaling other substances like

drugs can age the lungs prematurely”. Norman H. Edelman of the American Lung Association explained

that if she stopped smoking, her lung functions would decline

at the rate of a normal person, but continued smoking would

lead to a more rapid decline in lung function. Photographs of 

the singer with a cigarette in her mouth, taken 23 June 2008,

were widely published.

Winehouse was released from The London Clinic 24 hours after returning from a temporary

leave to perform at Nelson Mandela's 90th birthday and at a concert in Glastonbury, and continued

receiving treatment as an outpatient. In July 2008, Winehouse stated that she had been diagnosed with

"some areas of emphysema" and said she is getting herself together by "eating loads of healthy food,

sleeping loads, playing my guitar, making music and writing letters to my husband every day". She also

kept a vertical tanning bed in her apartment. Winehouse began precautionary testing on her lungs and

chest on 25 October 2008 at the London Clinic for what was reported as a chest infection. Winehouse

was in and out of the facility and was granted permission to set her own schedule regarding home

leave. She returned to the hospital on 23 November 2008 for a reported reaction to her medication.

Emphysema

Emphysema is a long-term, progressive disease of the lungs that primarily causes shortness of 

breath. Subcutaneous emphysema is a condition when gas or

air is present in the subcutaneous layer of the skin.

In people with emphysema, the tissues necessary to

support the physical shape and function of the lungs are

destroyed. It is included in a group of diseases called chronic

obstructive pulmonary disease or COPD (pulmonary refers to

the lungs). Emphysema is called an obstructive lung disease

because the destruction of lung tissue around smaller sacs,

called alveoli, makes these air sacs unable to hold their

functional shape upon exhalation. Emphysema is most often

caused by tobacco smoking and long-term exposure to air pollution.

The term "emphysema" is derived from the Greek ἐμφυσᾶν emphysan meaning "inflate" - itself 

composed of ἐν en, meaning "in", and φυσᾶν  physan, meaning "breath, blast ".
