Green Nail Syndrome Is Not Easy to Treat DEAR DR. ROACH: For the past four months, I have had an infection around the cuticle of my right index finger. There is green coloration under the nail near the cuticle, and the nail grew rather deformed. I am a gardener and a homemaker, so I tend to wash my hands often. The urgent-care doctor I saw prescribed clindamycin and mupirocin cream. I also was told to soak my finger twice a day. My family doctor took a culture and, because she did not know if I had a fungus or an infection, prescribed Lamisil and levofloxacin. She also recommended saltwater or peroxide soaks twice daily. The culture showed pseudomonas as the cause of the infection. With no improvement, I saw an orthopedic doctor. He told me to continue the soaking and the mupirocin cream. On my second visit, he lanced the area between the cuticle and the nail bed. There was improvement, but the symptoms soon return. The orthopedic doctor spoke of removing the nail as a possible solution to clearing up the infection. Is this my next step, or is there something else I can do? -- P.B. ANSWER: It sounds as though you have the aptly named “green nail syndrome,” which is caused by the bacteria you mention, Pseudomonas aeruginosa, and which often is complicated by a paronychia, an infection along the side of the nail. Your orthopedic surgeon is quite right that sometimes the nail needs to be removed; however, many people can avoid that unpleasant procedure (I had to perform it once) with ciprofloxacin (the best oral antibiotic against pseudomonas), antiseptic soaks and by keeping the hands meticulously dry, using gloves and even using a hairdryer to dry the nails after soaking or cleaning. *** DEAR DR. ROACH: I am a 67-year-old male who has had mild to moderate psoriasis for the past seven years or so. I have been avoiding the very effective biological drugs because of the scary possible side effects. Instead, I use anti-itch topical creams and lotions, some with cortisone. In researching the topic, I ran across a treatment called Fumaderm that is available in Germany, but I have never heard of it being used in the U.S. -- C.S. ANSWER: Psoriasis is an inflammatory skin condition that can range from annoying to life-threatening in severity. Psoriasis and its common complication, psoriatic arthritis, together are a highly destructive arthritis that often requires powerful treatment: biologic agents, like adalimumab (Humira) or etanercept (Enbrel). These do have the possibility of serious side effects, such as reactivation of tuberculosis. Fumaderm has been used in Germany for 19 years with a generally good safety record. There have been reports of a disease, PML, caused by brain virus, associated with Fumaderm, although it isn’t clear whether the Fumaderm predisposed the individuals to develop this infection. No medicine is free of the possibility of side effects, and treatment for psoriasis should be appropriate for the degree of severity of the illness. If you are doing well enough with your current treatment, I wouldn’t recommend trying a treatment with a higher risk of serious side effects. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. Hemlock Writer Weaves Tales of History and Folklore

eclectic-his father’s military motorcycle experiences, a stuffed bear, a stranded snapping turtle-yet each was rich with Ira’s descriptive language and folksy tone. Soon he was venturing into more imaginative territory, with a series of stories based upon Hemlock folklore and some of its more colorful residents, like “Uncle Jimmy.”

At the urging of his editor and friends, Ira decided it was time to publish his collection of stories. True Stories

and Tall Tales , printed by Penny Lane Printing in Avon, includes 23 of Ira’s entertaining tales. Copies will soon be available at local book stores and on Amazon. If you’d like to purchase a signed copy, contact Ira at [email protected].

Ira Briggs is a character. A mile-a-minute talker with a fascination for the past, Ira’s been collecting bits and pieces of local history for just about his entire life.

Ira grew up on the fringes of the Hemlock Fairgrounds, a veritable paradise for an adventurous boy. His father Larry had an auto repair/welding business which thrived on the trade generated by the famed Demolition Derby. Those experiences sparked Ira’s interest in anything with wheels - from machinery to motorcycles, antique cars to Atterbury Trucks. Larry also drilled wells and taught his son the trade.

His mother Dorothy passed on her gift of gab and love of antiques. Ira recalls the day decades ago when, at her urging they rescued the 100 year-old dilapidated wooden Hemlock Fair ticket booth from destruction. Today it resides at Ira’s “Hemlock House,” spruced up with a fresh coat of paint and a fancy sign.

School did not come easy for Ira, but he later gained an appreciation for reading. He recalls his father once giving him the Readers’ Digest and a copy of Sea Biscuit to help pass the time while drilling a well.

Ira later enrolled in a Creative Writing course at MCC. His professor, Maria Kendig, inspired him to write two articles for the Livonia Gazette about his experiences with the movie making industry. Once, inspired by a story idea while at a well site and frantic to record it before he forgot, Ira grabbed a roll of drywall tape and a pencil. He still has those notes.

In addition to well drilling, Ira works at Strong Hospital. He enjoys interacting with coworkers of diverse ages and nationalities, and is quick to share a laugh or a story. Ira understands the challenges of those who are new to America and still learning English, and is always happy to take the time to explain unfamiliar idioms and confusing customs. When a young work friend died suddenly, Ira composed a letter to his parents expressing the group’s sympathies and recalling memories of happier times.

To the couple who walk hand in hand on [local] Road. Please walk on walking lane and not in the road, and refusing to move for oncoming traffic. It’s unsafe for them and can cause a traffic accident when both lanes have vehicles passing.

We have quite a “rash” of identity thefts lately. It is upsetting when the victim is elderly. Our elderly citizens have worked hard all their life for all that they have. In most cases, they do not have much as they have had to give up quite a bit to receive health care and housing- let alone put food on the table. Most are on fixed incomes and every penny counts. Some people have difficulty reconciling their finances- which is why some thieves think they can get away with it. It sickens me that someone would take

advantage of a person who could be your or my grandparent. Why can’t those who commit such crimes actually get a job and support themselves? It is time we make an example out of those who commit such crimes and punish them to the fullest extent of the law.

Sick and Tired

I cannot figure out why since we have the computer link up that connects all area Doctors & Hospitals & other medical facilities to each other in our region, why we always have to fill out paperwork with the exact same questions on it at each medical care provider we go to. Why can’t things like past Operations/procedures/ history of illnesses/family health issues/things we do - do not suffer from & such be included with the patient’s current medical issue/conditions? Doing this would SAVE EVERYONE TIME and the office/facility MONEY and reduce paperwork, and help everyone all around and even be more accurate than someone who has problems remembering all those things. It would be a win/ win situation all the way around.

To the kind lady who found my cell phone on William Street in Palmyra, Friday, July 1st. After bike riding on the Canal Trail, I mistakenly left my cell phone on the truck running board. I realized once I arrived home what happened and called my phone. This nice lady returned my call. She had found my phone in the middle of the road. She agreed to drop it off at the Dollar Store on William Street. I drove back to Palmyra and retrieved my cell phone which had not been damaged or tampered with. My thanks for your kindness. I will certainly pass the favor on.

On Friday afternoon, an unknown woman, delivered buckets of fresh flowers that were going to be thrown out. Trader Joe’s gave them to her and she delivered them to Maplewood Estates in Fairport, NY. So many senior men and women were treated to beautiful bouquets for their rooms. I wish everyone could have seen the smiles! It sure lifted spirits for many of our residents. We just want to express our thanks to the unknown lady who took the time to do this and also Trader Joe’s for giving them to her.

To my neighbor and friend Ursala: On June 4th I had a very bad bike accident. Ursala came to my rescue although my injuries were not visible (I suffered a concussion, and facial fractures.) Medication mismanagement can be prevented

Medication management is an important issue for seniors and their families. Failure to properly manage medications can threaten the lives of seniors, highlighting the emphasis families must place on ensuring seniors take their medications in strict adherence to their physicians’ instructions.

Polypharmacy, or the taking of multiple medications for different conditions, can be a potential health hazard for the thousands of seniors who must manage health conditions with prescription drugs. Harmful drug interactions are a result of the confusion that can arise when seniors take multiple medications at the same time. The American Society of Health-System Pharmacists estimates that more than 34 percent of senior citizens are prescribed medications by more than one physician, and 72 percent use medications they were prescribed more than six months prior. Many people also have begun “pharmacy shopping” to save money. According to a study published in 2010 in American Nurse Today, 44 percent of men and 57 percent of women older than age 65 take five or more medications per week, with some taking as many as 10.

Medication confusion is one risk, but older adults also metabolize medications differently than young people. As a result, they may be more susceptible to overdose or other ill effects. Families looking to help seniors effectively manage their medications should consider the following tips.

· Keep a running list of medications. Maintain a list of

all medications being taken, noting both prescription and over-the-counter medications and any supplements and herbs. Provide a copy to any new doctors you visit and any new pharmacies you patronize.

· Use pill sorters. Medication sorters can keep medications organized and eliminate some of the personal error in medication management. Organizers have daily slots and may also differentiate between nighttime and daytime medications.

· Understand why each medication is being prescribed.

Ask your pharmacist and doctor to explain why each medication is prescribed. This information should be printed on the prescription label. Some drugs designed for one symptom may actually be used to treat other issues as well.

· Ask for help. Some seniors may benefit from friendly reminders from a loved one regarding when to take certain medications, especially if they need to take multiple doses throughout the day. Cognitively impaired seniors may require the services of a visiting nurse or another caregiver.

· Recognize that not all medications are right for

seniors. The American Geriatrics Society maintains a list of medications that older adults should avoid or take with extreme caution. Some drugs pose a high risk of side effects or interactions, while others are less effective. Discuss alternatives with your physician if you are prescribed one of these medications.

· Keep a medication diary. Record any side effects that occur and how the medications make you feel. Always bring up issues promptly with a doctor.

Proper medication management can help seniors avoid drug-related accidents or worse.
