Preparing for hospice care Sometimes patients succumb to a disease, in spite of the

best treatments and support. When the time comes for a family to confront a loved one’s declining health, hospice care can be a dignified way to make that person’s final days as comforting as possible.

Hospice is a type of care designed to make the final moments of a person’s life as pain-free and manageable as possible. It can take place in a medical facility, but very often hospice care occurs at home or wherever the ill person feels most comfortable.

Hospice care is usually the last step when all other options have been exhausted. Making the decision to move a loved one to hospice care can be an emotionally wrenching time. Decisions such as these may be better made at a time in a person’s life when he or she is not sick. That is why living wills that spell out details for end-of-life care can be quite valuable. Decisions are made with a level head and not wrought with emotions. Such living wills also can take the pressure off of family members who may not feel comfortable making such decisions on their own or in concert with relatives.

Hospice is a type of care and a philosophy that focuses on the palliative care of terminally ill patients. Rather than providing medication to try to treat the illness, medicine is offered to make a person more comfortable and remove any pain. Psychological therapy may also be offered to help the person come to terms with the end of life. Although hospice is a concept that has gradually evolved since the 11th century, the principles of modern hospice care can trace their roots to the 1950s and Dame Cicely Saunders, a nurse and social worker who is known as the creator of the hospice movement.

If hospice care is outlined in a dying person’s living will or expressed wishes, there are some guidelines that can be followed by the sick individual.

• Visit various medical care centers to see if away-from- home options provide the care and environment you desire. Be sure to establish what type of hospice care you prefer. This may

include care within a nursing home or hospital, or in the comfort of your own home.

• Sign a medical durable power of attorney. This signed document gives authority to an adult age 18 or older, who then has the right to make necessary medical and healthcare decisions for you in the event you become incapacitated.

• Clearly indicate your wishes regarding resuscitation. A signed document may alert emergency healthcare personnel or others of your wishes not to be resuscitated. This document must be signed by you and a doctor and witnessed by others.

• Decide on the duration of care. Hospice care generally lasts six months. This enables a long stretch of palliative care. Should you live beyond the time period, hospice care can continue. Studies have shown that patients who receive hospice care for at least 30 to 60 days gain greater benefit than those who are placed in hospice only in their final days.

• Investigate financial options for hospice. Medicare and Medicaid provide hospice coverage in 44 states. Many private insurance plans have a hospice benefit as well. Understand how payment is made to figure out if private funding will be necessary.

Not all hospice programs are the same. Certain core models of care are followed by many hospice facilities, but options vary. Hospice can provide comfort in a person’s final days and help sufferers better enjoy life and time spent with loved ones in these last days.

Ontario County 4-H Members at National Poultry Event

Two Ontario County 4-H members recently went to the 2015 National Poultry Conference in Louisville as part of the New York State 4-H Poultry program. Gracie Nicol, daughter of Nic and Amy Nicol of Victor, and Richard Lawson, son of Carolyn Lawson and Richard Lawson Jr. of Clifton Springs, gained the recognition to attend after competitions at the New York State Fair in September.

Gracie was a member of the Avian Bowl team for New York State which placed 5th at the National competition. As a member of this team, Gracie had to show her knowledge of the poultry breeds and challenges for the industry in the team completion of questions. Locally, Gracie is a member of the Sonshine Kids 4-H Club.

Richard, a member of the Castle Clovers 4-H Club, competed in the National Turkey Barbeque contest when in Kentucky and placed 3rd. He prepared turkey on the charcoal grill using his selection of maple barbeque sauce. During the event, he had to consider proper meat handling skills and food safety procedures. Richard also had to do an oral presentation on the turkey industry as part of the competition.

Other preliminary contests and activities for these events occurred during the Ontario County Fair. Thanks goes to Dudley Poultry of Middlesex for supplying the meat for the local and state fair barbeque events.

Can Heart Surgery Cause Hot Flashes? DEAR DR. ROACH: I have had hot flashes, similar to those experienced by post-menopausal women, ever since my coronary bypass in 1990. I can be sitting quietly in a chair reading or at my computer, and suddenly I find myself dripping with perspiration. I also have had panic attacks, with increased heart rate and other physical symptoms. A few years ago, a 24-hour EKG showed heart- rhythm irregularities and that my heart stopped briefly while asleep.

If these idiosyncrasies of my autonomic nervous system are a consequence of my time on the heart-lung machine for the bypass, I’m not complaining -- just curious. You said that studies have been done on almost everything. Is this one? -- W.S.B.

ANSWER: I found that a lot of people complain of similar symptoms. I also found that indeed, the autonomic nervous system (the part of the nervous system that regulates temperature, sweat, blood pressure and many other critical aspects of body function without our being consciously aware of it) can be adversely affected by cardiac surgery. Some authors have suggested that the autonomic nervous system changes are one mechanism by which people are more likely to have depression after cardiac surgery. I found several possible explanations why the ANS changes with bypass surgery, but no clear consensus.

I would want to be sure that there are no other causes of these symptoms. It sounds like you have had an extensive evaluation, but I certainly would be concerned about a sudden fast heart rhythm, such as atrial fibrillation.

DEAR DR. ROACH: I suffered a long time with GERD. I was diagnosed with Barrett’s esophagus, a precancerous disease. My doctor recommended a procedure called a Nissen fundoplication. Since the procedure, I have not had heartburn again. I would do it again, after what I went through. I get an endoscopy every two years to keep an eye on my disease. -- G.D.B.

ANSWER: Barrett’s esophagus is a complication of longstanding reflux. It is diagnosed when the lining of the esophagus starts to look like the lining of the stomach. Barrett’s esophagus can progress to cancer of the esophagus. About 0.2 percent of those with Barrett’s per year will develop cancer, which sounds low, but if you live with the condition for 25 years, that’s about a 5 percent risk.

Surgery for reflux disease is done only rarely. However, it is very effective (85 percent to 90 percent). The most common reason to consider surgery is inadequate relief from medications and lifestyle changes. However, severe erosions from acid in the esophagus, inability to take medications, a stricture (partial closure) of the esophagus and Barrett’s esophagus are all reasons to consider surgery. Surgery appears to reduce the risk of cancer more than other treatments.

29th I was in Denny’s in

Batavia alone having

breakfast. I am a widow of

20 years plus and dine alone

me a beautiful family were

twin daughters. They left

before I did; they glanced

over and gave me a nice

smile. After they left the

waitress came over and told

me they had paid for my

they are local or just passing

Cat overpopulation is not a cat problem, it’s a people problem. People caused it, people can fix it. All cats deserve a home and love; they should not be living wild outside. Please do not get a cat or kitten if you cannot have it spayed or neutered or keep it in at all times, and give it a lifetime home. Aid financially or help if you can those trying to trap, neuter & release feral cats. If you can, give a shelter cat or two a good home.

Veterinarians, please make the cost to euthanize an animal as low as you possibly can. Many animals suffer needlessly as their owners cannot afford the high cost. Do it for the animals.

While driving yesterday, I had two cars pull out in front of me! When I looked in my rear view mirror both times there were no cars behind me. Yes, you “made it” but only because I braked. I could understand if I had 4 or more cars behind me but to wait an extra 30 seconds or so to be safe, why not?

A neighbor up the hill from us lets their stupid dog bark incessantly, even late into the evening (10/ 11pm). If you are not going to take care of a pet properly, then DON’T HAVE ONE! We can hear it inside

our home, even on the opposite side of the house. My wife even wears ear plugs to bed and she can STILL hear it. If it keeps up I will contact the authorities. We are up early for work and we need to sleep!

Just a reminder from Driver’s Education class, when vehicles turn right at an intersection onto a 4- lane road, stay in the right lane, then put on your turning signal to merge to the Left lane. Those turning left onto a 4-lane road, stay in the left lane, and then put your turning signal on to merge to the right lane. As a quick reminder, when turning, stay in the closest lane, then merge into the other lane making sure lane is clear & your turning signal is on. Driver’s Ed should NEVER have been cut from school budgets. If you drive, learn the rules of the road & law enforcement of the rules as well.

The Town of Bristol is currently reviewing the need to update the Town Comprehensive Plan. The formation of a steering committee is needed to make recommenda- tions to the Town Board for pertinent changes.

Residents who are interested in serving on the committee are asked to submit a cover letter and a brief resume of their background to the Bristol Town Clerk no later than Feb. 19th, 2016. Letters of interest will be reviewed by the Town Board & selected for consideration.

The process may require a 2-3 year volunteer service commitment with regularly scheduled meetings. Input and review of future growth along with anticipated needs in the community will be prioritized and formatted. The Committee will be comprised of Town residents only.

Jackie was predeceased by her hus-band, Thomas and a son Peter. Sur-vived by three children, Beth (Dan)Conversi-Clark of Honeoye, Jennifer(John) Delgrand of Livonia, and Mat-thew Gilmore of Elmer, NJ. Two stepsons, David Garrity of Colorado, andDaniel (Dee) Garrity of North Caro-lina. Six grandchildren, Ryan, Joshua,Erin, Hannah, Madelyn and Adam.And two great grandchildren, Ben andLanden. She is also survived by a dearcousin Virginia Holden of Freeport,Maine and a very special friend, GerryRuflin of Ferris Hills, Canandaigua andher beloved dog, Opal.

She had a full life with many interests.She was an avid bridge player andloved cards, games, and puzzles, toread, knit and do needlework. Shewas an amazing cook and loved toenter ta in . Her ch i ldren andgrandchildren will always remembermany happy times spent vacationing ather home in Limington, Maine.

notice that the headstones are largely a mix of marble and granite. Because marble lends itself to elegant carvings and design that often mimic classical sculpture, it was the material of choice for gravestones throughout much of the 19th century. However, because it is composed of dense crystalline or granular metamorphic limestone, marble is a relatively soft stone that is prone to erosion. Because of this, there was a widespread switch to granite during the following century and up to today. This igneous rock composed primarily of quartz, feldspar, and some mica forms as magma cools, making it more likely to endure the test of time in the cemetery.
