Abstract
Food and drugs are both crucial for maintaining health. However, drug- nutrient (food) interactions may lead to treatment failure, adverse effects or complications related to specific nutrient deficiency. Drug-nutrient interaction (DNI) is defined as physical, chemical, physiologic or pathophysiologic relationships between a drug and a nutrient. The mechanism of DNI includes incompatibility in vitro and kinetic (absorption, distribution, metabolism, excretion) or dynamic (additive, synergistic or antagonist effects) interactions iv vivo. Understanding the mechanism and active ingredients of DNI can help us to manage and even prevent DNI in advance. Due to the diversity of food and the difficulty to quantify specific nutrient in ordinary diet, it is hard to predict the incidence and severity of DNI. The medical team should pay more attention to the high risk patients, such as polypharmacy, old age, patients with underlying malnutrition or intestinal dysfunction, multiple comobidities and receiving enteral or parenteral nutrition, who easily suffer from severe adverse events of DNI. By getting detail diet and drug histories of those patients, we might early detect significant DNI and take actions to avoid harmful consequence.
