Visa application form. Visa application form filled out in detail(block letters) and signed by the applicant. Itinerary. Copy of round trip tickets or confirmed itinerary. Yellow Fever Vaccination. Copy of International Certificate of Vaccination for Yellow Fever. Personal Invitation. If staying with friends or family, a letter of invitation from the host in Burkina Faso, NOTARIZED by the appropriate city hall in Burkina Faso can be used instead of the hotel reservation. Letter of Invitation. An official letter of invitation approved by the Ministry of Foreign Affairs in Burkina Faso. Tourist Voucher. Copy of tourist voucher issued by tourist company or hotel in Burkina Faso approved by the Ministry of Foreign Affairs.
