REPORT SUMMARY
UNDERWATER INSPECTION
The substructure units inspected at Bridge No. 07569, Piers 2, 3, and 4, were in good condition with no defects of structural significance observed. The channel bottom around the substructure units consisted of firm material, which was well established and appeared stable. The west river bank exhibited vertically eroded slopes. Timber debris has accumulated at the Piers 2 and 3.
(A) The channel bottom material around the substructure units consisted of sand, gravel, and 2 to 4 foot diameter cobbles with up to 1 inch of probe rod penetration. (B) Timber debris, 2 foot diameter and smaller was observed around the upstream nose and along both faces of Pier 2, extending from the channel bottom to 10 feet above the waterline. (C) Overall the concrete of the piers was in good condition and only exhibited random vertical cracks.(D) Vertical bank erosion was observed along the river bank, consisting of up to 15 feet high vertical cut embankments. (E) Timber debris, 3 feet diameter and smaller was observed along the east face of Pier 3 from the channel bottom to 10 foot above the waterline and 30 feet upstream and downstream of the fascias.

The superstructure consists of a five spans of multiple prestressed concrete beams supporting a reinforced concrete deck. The superstructure is supported by two reinforced concrete abutments and four reinforced concrete piers. The abutment footings are founded on steel H-piles, while the pier footings are founded on 48 inch diameter caissons. The piers are numbered 1 through 4 starting from the west end of the bridge.

The routine underwater inspection of Bridge 07569 (CSAH 90 over Blue Earth River) was completed on December 2, 2016. The underwater inspection was conducted from shore. The inspection was conducted by a team consisting of a PE-Diver with a valid MnDOT Team leader certification, a backup diver and a dive tender. The inspection utilized commercial dive equipment and techniques (SSA and/or SCUBA) in accordance with OSHA regulations. Profiles were taken along the upstream and downstream faces of the bridge and around the periphery of substructure units to determine the presence, location and area of scour.The bridge elements inspected consisted of 3 concrete pier walls. According to the bridge inventory or design drawings, Piers 2through 4 were founded on piles. Inspection procedures followed FHWA guidance and the MnDOT Bridge and Structure Inspection Program Manual with channel bottom probing to search for bottom of foundations. The routine underwater inspection frequency is recommended to remain at 60 months based on those findings and risk factors. Also, inspection procedures should continue to follow the above approach and standard guidance with 100% Level I and 10% Level II intensity efforts.
