Confirming your conversation with Bill Murfree, we shall add the following service to the above listed contract:
Utility Service Co., Inc. agrees to provideall labor, equipment, and materials needed to complete thefollowing:
SCOPE OF WORK FOR ROOF RAFTER REMOVAL & REPLACEMENT:
• Remove & Replace 19 Roof Rafters, individually(one-at-a-time).• The new spoke rafters and shall match existing material size, dimensions, and thickness.
Includes:
Modifying sidewall to accommodate lift stagingand restoring sidewall upon completionRepairs &Modifications at center top-plate, and rafter attachment points at "knuckle" ring-girder perimeter.Incidental roof-plate and ring-girderspot-patching {as needed, under 20 l.f.).
The maintenance fee for the above addition scope of work will be invoiced per the schedule below:
ADDENDUM TO ADD ADDITIONAL ROOF RAFTER REMOVAL AND REPLACEMENT
Should the Village of Malta, IL elect to cancel this addendum and/or the Original contract then the currentbalance of the Additional Fees shall be due and payable within thirty (30) days of the notice to cancel. Thepayment of the current balance of annual fees shall be governed bythe terms of the Original contract.Any andall other aspects of the Original contract not addressed in this addendum shall remain unmodified and infullforce and effect.

AN ORDINANCE DETERMINING THE PREVAILING WAGE RATES AS OF JUNE2017 FOR CONSTRUCTION OF PUBLIC WORKS PROJECTS UNDER THE
JURISDICTION OF THE VILLAGE OF MALTA
WHEREAS, the State of Illinois has enacted the Prevailing Wage Act, 820ILCS 130/0.01et seq. (hereinafter the "Act"); and
WHEREAS, Section 9 of the Act, 820 ILCS 130/9, requires that, for effectuation of thepurposes of theAct, each public body, during themonth of June of each calendar year, investigateand ascertain the prevailing rate of wages within its locality, to publicly post such determinationor keep it available for inspection by interested parties, andto filea certified copythereofno laterJuly 15th of each calendar year with the Illinois Secretary of State and the Illinois Department ofLabor; and
WHEREAS, the Village of Malta ("Village"), as a "public body" under Section 2 of theAct, 820 ILCS 130/2, is required by the Act to investigate and ascertain the prevailing rate ofwages asdefined in theActforlaborers, mechanics and other workers within its locality employedin performing construction of public works for the Village; and
WHEREAS, the Village is located in DeKalb County, and therefore, in accordance withthe definitionof "locality" in Section2 of the Act, the prevailingwages to be paid in the Villagemust be as determined in regard to those areas of the Village in DeKalb County; and
WHEREAS,the President and Board ofTrustees of the Village has reviewed the scheduleof wage ratefor laborers, mechanics, or otherworkers set out in Exhibit "A" for DeKalb County,and has determined it to be the prevailing rate wages for localities within the Village in DeKalbCounty during the month ofJune 2017,
NOW, THEREAFTERBE IT ORDAINEDby the President and Board ofTrustees of theVillage ofMalta, DeKalb County, Illinois,as follows:
Section 1. The recitals set forth above are incorporated herein and made a part hereof.
Section 2. To the extent and as required by the Prevailing Wage Act, the generalprevailing rate of wages in this locality for laborers, mechanics, or otherworkers engaged in the construction ofpublic works, under the jurisdictionof the Village is hereby ascertained to be the same as the prevailingrate ofwages for construction work in the DeKalbCountyarea (the prevailingrateto be determined by the locality of the work performed) as determined bythe Department of Labor of the State of Illinois, a copy of thedeterminations being attached hereto as Exhibit "A" for the DeKalb Countyarea, which is incorporated herein and attached herein by reference. Thedefinition of any terms appearing in this Ordinance, which are also used inthe aforesaid Act, should be the same as in said Act

Nothing herein shall be construed to apply said general prevailing rate ofwages as herein ascertained to any work or employment except publicworks of the Village to the extent required by the aforesaid Act.
The Village Comptroller/Administrator shall publicly post or keepavailable for inspection by any interested party in the main office of theVillage, this determinationof such prevailing rate ofwages.
The Village Comptroller/Administrator shall mail a copy of thisdetermination to any employer and to any association of employers and toanypersonor association of employees whohave filed, or filednamesandaddressesrequestingcopiesofany determinationstatingthe particularratesand the particular class of workers whose wages will be affected by suchrates.
The Village shall promptly file, not later than July 15, 2017, a certifiedcopy ofthis Ordinance with both the Secretaryof State IndexDivisionandthe Departmentof Labor of the State of Illinois at the following addresses:

This Ordinance shall be in full force and effect upon its passage andapproval as required by law.

This schedule contains the prevailing wage rates required to be paid for work performed on or after Monday, June 5,2017 on public worksprojects in this County. Pursuant to 820ILCS130/4, public bodies in this County that have active public works projects are responsible fornotifying all contractors and subcontractors working on those public works projects of the change (if any) to rates that were previously ineffect. The failure of a public body to provide such notice does not relieve contractors or subcontractors of their obligations under thePrevailing Wage Act, including the duty to pay the relevant prevailing wage in effect at the time work subject to the Act is performed.
