� LMRA – Last Minute Risk Analysis/Assessment(requirement from SCC)

� Creation of an order from the notification
� Print out of the order and handover to the craftsmen

3. Fill out five fields, take a photo and sent it off
Paper-based process
1. Near-accident, e.g. tripping over a threshold
2. Return to the workshop or office
3. Look for report form on the sharepoint
4. Fill out the form and sign it
5. Submit it to supervisors
6. Once it has been returned, forward it to those responsible for HSEQ
7. HSEQ representative calls back in order to get a better understanding of the situation
8. File the documented near accident on the sharepoint
