Benefits Enrollment Enrollment information mailed by Minnesota Management & Budget
(MMB) to your permanent/home address (Worksheet can be used as reference tool while enrolling online)

Effective Date of Coverage New hires: Insurance and flexible spending accounts take
effect after 35 calendar days from the date of hire in an eligible appointment. Examples:

Change in insurance eligibility status:• Insurance coverage is effective on the date of status change• Flexible spending accounts are effective on the date

Open Enrollment Open Enrollment period each fall (usually two

Option to Waive Health Coverage
New employees eligible for the full employer contribution may waive medical coverage by:
• Completing a Waiver of Medical Coverage form
• And by providing proof of other coverage to SEGIP’s office within the enrollment period

Cost Levels Deductible & copay depends on cost level. Cost levels are determined through state negotiation. Clinics and
carriers are placed in cost levels through negotiations. You are not receiving more or better service in one cost level than the other.
The two different deductible numbers depends on whether you have single or family coverage. For example, with cost level 2, your deductible will be $250 if you have single coverage and $500 if you have family coverage.
If you look at the copay on row C of the benefits schedule, you will see two different copay dollar amounts. Being newly eligible, you automatically get the lower copay. During open enrollment in the Fall, you will be asked to take a health assessment and agree to a coaching call. Doing so will let you keep the lower copay for the next calendar year.
More About Clinics Each family member may select a different
primary care clinic, but must be with same insurance carrier as employee. Keep an eye on cost levels if selecting different clinics
Each carrier has network of specialty care providers
Patient must have referral from primary care clinic for most services outside the primary clinic
