Welcome to Catholic Diocese of Richmond benefits guide! It’s time to consider electing a benefits package that meets your family’s health and financial needs, now and in the future. During this time, eligible employees will have a chance to enroll for benefits with a professional Benefits Counselor. This enrollment guide presents highlights of each of the benefit plans available to you. We hope you will use this information to make informed decisions that make the most sense foryou and your family. Please refer to the Premium Rate Sheet 2017-2018.

WHO IS ELIGIBLE?The Catholic Diocese of Richmond gives you the option of enrolling yourself and your eligible dependents in Medical, Dental, Vision, Life Insurance and Voluntary Critical Illness, Accident, Hospital Indemnity and Identity Theft Protection benefits. If you are a full-time employee working 30 or more hours a week, you are eligible for all enrollment options. If you work 20 or more hours a week, you are eligible for benefits as outlined. Eligible dependents include your spouse, child(ren) up to age 26, and unmarried child(ren) of any age who are incapable of supporting themselves due to a mental or physical disability. (Please note: Some benefits have different age limits for children.)

Eligible employees must enroll within 30 days from their date of hire. All benefits will become effective the first of the month following the date of hire, with the exception of Identity Theft, Hospital Indemnity, Critical Illness and Accident Insurance. These Voluntary Benefits will become effective the first of the month following enrollment. For questions about enrolling and eligibility, please speak to a Benefit Counselor.

HOW TO ENROLLBenefits Counselors can answer your questions and enroll you in benefits via telephone through the Enrollment Center. You can enroll by calling a Benefits Counselor directly the first two weeks of every month at 1-888-234-9479, Monday through Friday, 9 a.m. to 6 p.m. EST. You will also be able to enroll online at https://uenroll.net/richmonddiocese, which is available 24/7. To enroll in benefits, have your personal, dependent and beneficiary information ready. You will need full names, dates of birth, and Social Security Numbers of your dependent(s).

The Health Insurance Portability and Accountability Act of 1996 (HIPAA) has requirements about

privacy and protected health information. The Office of Human Resources has placed the privacy policy in compliance with HIPAA on our Extranet for your review.

Medical Benefits (Eligibility is 30 or more hrs/week)The Catholic Diocese of Richmond is offering three comprehensive high-quality medical plans through Anthem Healthkeepers to better assist you and your family. These options feature a network of doctors and specialists who have agreed to provide services at a discounted rate. You can use providers outside of the network, but you will pay more. We require verification of dependent status for anyone enrolling in Medical insurance. This will be collected at each location. To find out more information about pricing and coverage options, please speak to a Benefits Counselor or the Office of Human Resources.

Dental Benefits (Eligibility is 30 or more hrs/week)Dental coverage is important to your overall health and wellness. You can enroll in dental benefits through Anthem for yourself and your family. The plan is designed to help you maintain a healthy smile through regular preventive dental care and to address dental problems as they occur.

Vision Benefits (Eligibility is 30 or more hrs/week)Taking care of your eyes is also important! Blue View Vision through Anthem HealthKeepers is offered to you and your dependents through your medical insurance. The plan covers routine eye exams once every year, as well as discounts on eyewear through their Additional Savings Program. You can also choose to purchase supplemental vision coverage for yourself and your dependents through UNICARE for an additional cost.

Health Savings Account (Effective January 1, 2018)A Health Savings Account (HSA) allows you to set aside pre-tax money to pay for qualified health care expenses. It is only available to employees enrolled in the high deductible plan.

Here’s how it works:• Contribute and save on taxes. You make tax-free contributions to your account through payroll deductions, up to the yearly IRS limits ($3,450 for individual coverage and $6,900 for family coverage in 2018), plus an additional $1,000 if you are age 55 or older.• Use your HSA funds. You can use your HSA to pay for eligible health care expenses on a pre-tax basis.* Eligible expenses include deductibles, prescriptions and dental and vision costs incurred by you or your tax dependents. A full list of eligible expenses is available at www.irs.gov. If you pay out-of-pocket, you will be able to request reimbursement online for eligible expenses or set up an online bill payment.• Grow your HSA. If you don’t spend all of the money in this plan year, all available funds roll over year after year, and allow the account to grow over time.

*Any reference to taxes is at the federal level. State tax rules may vary.

Flexible Spending Account (Eligibility is 30 or more hrs/week) Effective January 1, 2018Flexible Spending Accounts (FSAs) allow you to set aside pre-tax dollars to pay for eligible out-of-pocket health care or dependent care expenses, therefore, reducing your taxable income. The Diocese offers two types of FSAs – a Health Care FSA and a Dependent Care FSA. To find out pricing information, speak to a Benefits Counselor or the Office of Human Resources.

Here’s how it works:1. Each year during Open Enrollment, you decide how much to set aside for health care and/or dependent care expenses.2. Your contributions are deducted from your paycheck on a pre-tax basis in equal installments throughout the plan year.3. As you incur health care or dependent care expenses, you can submit a claim form to be reimbursed from your account.

Basic Life and Accidental Death & Dismemberment Insurance (Eligibility is 20 or more hrs/week)Basic Life Insuance and AD&D offers financial protection to your loved ones if something were to happen to you. Catholic Diocese of Richmond provides Basic Life Insurance in the amount of $10,000 and AD&D coverage in the amount of $10,000. Your employer pays the full cost of this coverage. Supplemental insurance is available for employees, spouse/dependents but is employee paid.

Short Term Disability Insurance (Eligibility is 20 or more hrs/week)The Catholic Diocese of Richmond provides Short Term Disability through The Hartford to assist in the event an employee is out of work due to an injury or illness. If you are unable to work after 29 consecutive calendar days due to an eligible injury or illness, this benefit pays 60% of your weekly pay up to a maximum benefit of $1,000 per week, for a maximum of 22 weeks. Your employer pays the full cost of this coverage.

Long Term Disability Insurance (Eligibility is 20 or more hrs/week)Long Term Disability benefits through The Hartford provide a monthly source of income if you have an illness or injury that continues beyond the maximum Short Term Disability benefit period. The benefit provides you with 60% of your annual base pay to a monthly maximum of $6,000. Your employer pays the full cost of this coverage.

Voluntary BenefitsThe Catholic Diocese of Richmond offers you the opportunity to participate in Voluntary Benefits to help offset the out-of-pocket expenses that you might incur should an illness or accident occur. Protecting your financial future is just as important as taking care of your health. Voluntary Benefits provides you with access to a wide range of benefits to help protect your future and the future of your family.

Hospital Indemnity Insurance (Eligibility is 20 or more hrs/week)Hospital Indemnity Insurance provides financial protection if you or a covered family member require medical care in the hospital. The benefit amounts can be used to pay for medical plan deductibles and coinsurance until the out-of-pocket maximum kicks in. You may also have enough left over to pay for family member lodging and food while they stay near the hospital.

Critical Illness Insurance (Eligibility is 20 or more hrs/week)Critical Illness Insurance helps provide financial protection in the event of a covered serious illness. The plan pays a lump sum benefit that can be used anyway you choose, and benefits are paid in addition to any other coverage you have. Individually owned coverage means you can take your policy with you if you retire or leave the Diocese. Coverage options are also available for your spouse and children. Your Counselor or Human Resources can give you detailed information on the plan and rates during your enrollment session.

Accident Insurance (Eligibility is 20 or more hrs/week)Even when you live well, accidents happen. With Accident Insurance, cash benefits are paid directly to you if you suffer a covered injury. Covered injuries include fractures, burns, lacerations, ligament or tendon damage that requires surgery, eye injuries, and more. The benefit amount depends on the injury and the treatment received.

Identity Theft Protection (Eligibility is 20 or more hrs/week)ID Watchdog offers a comprehensive, proactive identity theft defense. It can help limit your chances of experiencing fraud and restore your identity if it does become compromised. It includes credit monitoring, an expert to help restore your identity, an identity theft insurance policy, and more.

Employee Assistance Program (All Employees) Employer paidThrough the Employee Assistance Program (EAP) provided by Anthem, you and eligible members have access to counseling for personal and work-related concerns. The assistance is completely confidential and is available 24 hours, seven days a week by calling 1-800-346-5484 or visiting www.anthemeap.com (enter CDoR Lay).
