検出率で最も高値であったのはIb 型の16.4% (111株),
次いでIII型の13.4% (91株),Ia型の12.7% (86株),V型
が12.0% (81株),II型の6.1% (41株),IV型が1.6% (11株)
であった。NT型を除いた各血清型において,GBSの平
均保菌率 (11.8%) を超えていたのはIa型,Ib型,III型お
よびV型であった。年次毎に各血清型の検出率は増
減していたが,Ia型,II型,III型およびV型は増加傾
向,Ib型,IV型およびNT型は減少傾向を示した。
Figure 2-3に40歳代におけるGBS保菌率および血清型
検出率を示す。40歳代のGBS保菌率は,2006〜2009年
に増加傾向,2010年以降は増減しながら減少傾向を示
した。なお,平均保菌率は10.8%であった。NT型の
41.5% (39株) を除いた血清型の平均検出率で最も高値
であったのはIb 型の19.1% (18株),次いでIII型の11.7%
(11株),Ia型の9.6% (9株) とV型の9.6% (9株),II型の
5.3% (5株),IV型の3.2% (3株) であった。NT型を除い
た各血清型において,GBSの平均保菌率 (10.8%) を超
えていたのはIb型とIII型であった。年次毎に各血清型
の検出率は増減していたが,Ia型,II型およびV型は増
加傾向,Ib型,III型,IV型およびNT型は減少傾向を示
した。
Table 3に各世代における2006年のGBS保菌率および
各年のGBS保菌率との有意差検定を示す。GBS保菌率
の統計処理では,20歳代は2016年,30歳代は2008年,
2010年および2014年以降に有意な増加を認めた。40歳
代は2012年に有意な減少を認めた。
Table 4に各世代における2006年の血清型検出率およ
び各年の血清型検出率との有意差検定を示す。血清型
検出率の統計処理では,20歳代は2012年,2015年およ
び2017年のIa型,2010年のV型に有意な増加,2017年
のIb型に有意な減少を認めた。30歳代は2015年のIb
型,2007年,2010年,2014年および2017年以降のIII
型,2016年のV型に有意な増加,2007年のIa型,2008
年,2014年および2016年以降のNT型に有意な減少を認
めた。Vi GBS による分離培養検査を開始した2015年
以降に有意に増加した血清型は,20歳代のIa型,30歳
代のIb型,III型およびV型,有意に減少した血清型は
20歳代のIb型,30歳代のNT型であった。なお,20〜30
歳代の他の血清型と40歳代のすべての血清型で有意な
増減は認めなかった。
考 察
GBS感染症は,新生児や乳児期の細菌感染症として
注意を要する重篤な疾患である。妊婦では絨毛膜羊膜
炎により前期破水や早産の原因となり,分娩後には子
宮内膜症や子宮筋層炎の要因となる8。妊婦のGBS保菌
率は3.2〜19.0%の範囲であり9-11,年間出生数1,000人あ

たり新生児1.8人がGBS感染症の発症リスクを有する12。
わが国の妊婦の膣内におけるGBS保菌率は10〜20%程
度とされており,母体に抗菌薬の予防投与をしなかっ
た場合は約50%,予防投与した場合は約10%が新生児
に垂直伝播する。そして,GBS保菌妊婦から出生した
新生児の1〜2%がGBS感染症に罹患する13。近年,小児
の細菌性髄膜炎におけるGBSの割合は,Streptococcus
pneumoniaeやHaemophilus influenzae b型のワクチンが定
期接種となった影響により,相対的に増加を示してい
るとの報告がある14。
本検討において検体数や菌株数が最も少なかったの
は,40歳代の検体数を除くと各世代とも2012年であっ
た。特に,全ての世代で菌株数が低値を示した。本現
象について2010年から2014年の分娩数を調査した結
果,2010年は1,038件,2011年は933件,2012年は920
件,2013年は971件,2014年は1,016件であり,2012年
の分娩数が最も低値であった。したがって,検体数の
低値は分娩数の低下が要因の一つと考えられたが,菌
株数の低値については原因の特定はできなかった。
全世代のGBS保菌率は2006〜2018年にかけて増加傾
向を示し,統計処理の結果では2008〜2010年に有意な
増加,2012年に有意な減少を認めた。しかし,2014年
以降では有意な増加を示した。なお,平均保菌率は
11.3%であったが,本邦における妊婦のGBS保菌率 (10
〜20%) と比較すると低値であった。本要因としては,
対象検体の検体採取部位が腟口部から肛門周囲のみの
擦過であったこと,GBSの検出方法が直接法による分
離培養検査であったことなどが考えられる。検体採取
部位に関する検討では,Kovavisarachら15は腟または肛
門直腸の単独採取での検出率 (それぞれ13 .44%,
10.31%) に比べ,腟および肛門直腸からの採取による
検出率 (18.12%) が有意に高いことを報告している。ま
た,産婦人科診療ガイドライン−産科編2017年のガイ
ドライン7では,腟口部から肛門周囲および肛門内を擦
過する検体採取法を推奨している。通常,当院では妊
婦全例に対して腟口部から肛門周囲にかけての検体採
取を行っている。しかし,肛門内までの擦過が行われ
ていなかったか,徹底されていなかった可能性が考え
られた。したがって,検体採取部位を腟口部から肛門
周囲および肛門内までに変更する必要性がある。ま
た,BAによる分離培養検査は非溶血性GBSを検出でき
ない。非溶血性GBSは1〜5%存在することから16-18,そ
の対策法としてDNAプローブ法やPCRなどの代替方
法19-21が開発された。しかし,高価で操作に熟練を要す
る欠点がある。Vi GBS による分離培養検査は,非溶
血性GBSの検出に優れているが検出感度はBAとほぼ同
等である。Takayama 22らは,GBS選択増菌培地と開発
した免疫クロマトグラフィーを用いることにより,少
量のGBSや非溶血性GBSを短時間で検出可能であり,
その検出率は直接法 (BA,Vi GBS) 以上であったこと
を報告している。したがって,GBSの検出方法につい
て検討する必要性がある。
各世代のGBS保菌率に関しては,2006〜2018年にか
けて20歳代と30歳代に増加傾向,40歳代では減少傾向
を示した。なお,平均保菌率は30歳代の11.8%が最も
高値で,次いで40歳代の10.8%,20歳代の9.9%であっ
た。GBS保菌率の統計処理では20歳代は2016年,30歳
代は2008年,2010年および2014年以降に有意な増加,
40歳代は2012年に有意な減少を認めた。30歳代の保菌
率が最も高値で有意な増加を認めた要因として,最も
多い妊婦数の他に過去の妊娠中にGBSが検出された妊
婦は,以後の妊娠時にもGBSを検出する割合が高い要
因が考えられた23。さらに,GBSは腸管内常在菌であ
り持続的な保菌状態の原因が示唆されたが,本現象に
ついては更なる検討が必要と考えられる。
NT型を除いた血清型において,GBSの平均保菌率を
超えていたのは40歳以上を除く世代でIa型,Ib型,III
型およびV型であり,40歳以上ではIb型とIII型であっ
た。全世代の血清型の検出率で増加傾向を示したのは
Ia型,II型,III型およびV型,減少傾向を認めたのはIb
型,IV型およびNT型であった。そのうち,Vi GBS に
よる分離培養検査を開始した2015年以降に有意に増加
した血清型はIII型およびV型,有意に減少した血清型
はIV型およびNT型であった。
さらに,各世代の血清型の検出率で増加傾向を認め
たのはIa型,II型およびV型,減少傾向を示したのはIb
型,IV型およびNT型であった。なお,III型では30歳代
のみに増加傾向を認めた。そのうち,Vi GBS による
分離培養検査を開始した2015年以降に有意に増加した
血清型は,20歳代のIa型,30歳代のIb型,III型およびV
型,有意に減少した血清型は20歳代のIb型,30歳代の
NT型であった。
Edwardsら24は,EODで最も多かった血清型はIa型 (約
30%) とIII型 (30%),次いでV型 (20%),II型 (約15%),
LODで最も多かった血清型はIII型 (約60%),次いでIa型
(約25%),V型 (約15%) であったと報告している。さら
に,保科25,26らは,本邦でGBS感染症の予防対策を検討
し始めた1980年代〜1990 年代初期の血清型に関して
は,IV型 (当時はNT6型) とVIII型 (当時はJM9型) で過
半数を示した。しかし,1997年,2002年,2007年およ
び2012年の血清型分布の推移を調査した結果,Ia型,
Ib型,III型およびV型は増加傾向,IV型とVIII型は減少
傾向を呈し,経年的に血清型分布が変化を示していた
と報告している。なお,調査期間内のGBS保菌率はほ
ぼ横ばいであったが,血清型分布が変化を示した要因
については不明であるとされている。
今回,全世代と各世代で増加傾向,ならびに減少傾
向を認めた血清型に関しては,Edwardsら24や保科25,26ら
の報告とほぼ同様の結果であり,特に2015年以降はIa

型 (20歳代),Ib型 (30歳代),III型 (全世代,30歳代) お
よびV型 (全世代,30歳代) で有意な増加を認めた。し
かし,20歳代のIb型に有意な減少を認めたことの要因
に関しては不明である。
一般的に,GBS感染症の原因菌として多い血清型は
Ia型,Ib型およびIII型である2-4。小児IGBS感染症のう
ち,敗血症や髄膜炎に由来した血清型はEODではIa型
が最も多く,次いでIII型,Ib型,LODではIII型が最も
多く,次にIa型,Ib型との報告がある3。
今回の検討では,GBS血清型の検出率は全ての世代
でIa型,Ib型およびIII型の3血清型が上位を示し,20〜
30歳代のIa型,Ib型,III型およびV型で有意な増加を認
めた。以上より,新生児や乳幼児のIGBSの原因とし
て,今後はこれらの血清型の動向に注視する必要性が
考えられた。

Objective: Neonates and infants who develop Group B hemolytic streptococcal (GBS) infections have a highrate of death and sequelae.Methods: Clinical specimens and strains from pregnant women with GBS infections for culture tests weregrouped under all age groups or classified into three age groups (those in their 20s, 30s, and 40s) by year, andthe annual transition in GBS colonization and serotype detection rates in each age group were investigated.Furthermore, based on GBS colonization and capsular serotype (serotype) detection rates in 2006, we analyzedtransitions in these after 2007.Results: In all age groups, Types Ia, Ib, and III were the top three serotypes, followed by Types V, II, and IV.Statistical processing of GBS colonization rates in all age groups showed a significant increase in 2008−2010and after 2014 and a significant decrease in 2012. Statistical analysis of serotype detection rates showed asignificant increase in Type III in 2014 and 2017 and in Type V in 2014 and 2016. In contrast, a significantdecrease was observed in Type II in 2009 and in Type NT in 2014 and 2016−2018. In the statisticalprocessing of GBS colonization rates in each age group, patients in their 20s showed a significant increase in2016; those in their 30s showed an increase in 2008, 2010, 2014, and later; and those in their 40s showed asignificant decrease in 2012. In the statistical processing of serotype detection rates, patients in their 20s, asignificant increase was observed in Type Ia in 2012, 2015, 2017 and Type V in 2010 but 2017 in Type Ibshowed a significant decrease. For patients in their 30s, a significant increase was observed in Type Ib in 2015,Type III in 2007, 2010, 2014, 2017−2018 and Type V in 2016; however, 2007 in Type Ia and 2008, 2014,2016−2018 in Type NT showed a significant decrease.Conclusions: In all age groups, the detection rates of GBS serotypes were highest for Types Ia, Ib, and III.After statistical processing, a significant increase was shown for Type III in 2014 and 2017 and for Type V in2014 and 2016 in all age groups. Patients in their 20s showed a significant increase in Type Ia in 2012, 2015,2017 and in Type V in 2010. Patients in their 30s showed a significant increase in Type Ib in 2015 and TypeIII in 2007, 2010, 2014 and 2017−2018 and in Type V in 2016. From the above, it is considered necessaryto pay attention to the trends in Types Ia, Ib, III, and V, the major serotypes of causative bacteria in GBSinfections.
