Safety Moment Incident Name: Warehouse Overhead Door #2 Failure Incident Date: February 7, 2017 Time: 14:30:00 Location: Warehouse Description On Tuesday, February 7, 2017 a warehouse attendant was using the wall-mounted electric door opener to open overhead door #2 at the loading dock. During the process, the electric opener failed to stop the overhead door. The door continued opening which caused enough spring tension to break the cables that lift the door. As a result, the door was released and fell down to a closed position. Workers were in close proximity near the wall-mounted door opener. Leading up to the incident, overhead door #2 had been working inconsistently on the previous shift. The door would open three quarters of the way using the wall-mounted electric door opener and then required attendants to switch it to manual to open it the rest of the way. The door eventually became stuck open for a period of 12 hours and Mill Maintenance was called.
Effects on Environment/Health/Safety: None Immediate Corrective & Preventative Actions Taken: The overhead door was secured in the closed position. An overhead door contractor was commissioned and came to site Wednesday, February 8, 2017 to assess the door and make repairs. The contractor discovered that the limit switch failed which caused the cable to snap. The limit switch has been replaced and door is now working. An investigation was initiated.
