Instructions for Shipping1. Label the specimen(s) with the patient’s name, date and time of collection.
2. Complete the Requisition Form. Please be sure to fill in all fields of information,
including the patient’s demographics and billing information.
3. Verify that the appropriate specimen requirements have been met (sample type,
volume, collection tube type).
4. Place specimens and completed requisition form in Shipping Kit.
5. For courier service: Place Shipping Kit into Biohazard Bag and seal. Call GenPath
at 800 627 1479 for courier pickup.
6. For FedEx service: Place Shipping Kit in FedEx Diagnostic Kit and seal. Complete
and attach FedEx Airbill. Mark for Saturday delivery if shipping on Friday. Call
FedEx for pickup at 800 463 3339 and call GenPath Client Services Department
with Airbill Number at 800 627 1479.
