Project provides a 24-hr hotline for frightened pregnant women or women contemplating
abandoning their children. They also offer support and education for those caring for
drug-exposed babies.
Prototypes Community Assessment
Center offers information on substance abuse and HIV/AIDS. They also offer referrals for other
social service needs and mental health assessments/referrals to treatment. Office hours are
Monday to Friday, 8 a.m. to 5 p.m., evening hours by appointment. Admission requirement is
men and women 18 years of age or more and their families. Free services. Serves San Gabriel
Valley.
PUENTE Learning Center
Adult education programs include English as a Second Language, job training, computer
applications and computer repair. Center also offers a youth tutoring program. Volunteer
opportunities available. Office hours are Monday to Friday, 8 a.m. to 5 p.m. Admission
requirement is individuals must meet eligibility to enroll. Free classes and materials. Registration
fee for some classes. Serves East Los Angeles County.
Saban Community Clinic — Beverly Health Center
Center offers a range of diagnostic and laboratory services, including X-ray, ultrasound,
computed tomography scans and mammography. Additionally offers treatment for obesity,
diabetes, asthma and chronic illnesses. The clinic provides various educational and outreach
programs including health education in weight loss, peer youth education, disease management,
weight loss, peer youth education and HIV testing/counseling. Office hours are Monday through
Thursday, 7:30 a.m. to 6 p.m., Friday 7:30 a.m. to 4:30 p.m. Serves Los Angeles County.
Salida Del Sol Adult Day Health Care
The center serves the region's neediest adults with basic services ranging from exercise classes to
medical services. The center is staffed with full-time or visiting nurses, therapists, dietitians and
other medical experts. Most of the seniors at the center (about 65 patients ranging from ages 24 to
102) require heavy intensive care. Workers at the day care regularly administer medication, feed
seniors, help them to the restroom, monitor blood sugar/pressure and report symptoms to the
patients' doctors. Office hours are Monday through Friday, 9 a.m. to 5 p.m. Serves Los Angeles
County.

Salvation Army/Burbank
Emergency housing for families when funds are available and emergency food assistance. Social
service hours are 9 a.m. to 3 p.m. Walk in or call. Office hours are Monday to Thursday
9 a.m. to 3 p.m. Admission requirement is the individual has a Social Security card, picture ID,
utility bill, rent receipt and proof of income. Free services. Serves residents of Burbank, Sun
Valley, Tujunga, Sunland and North Hollywood.
SEIU ULTCW
Union provides free referrals for in-home supportive services. Also helps low-income seniors and
people with disabilities remain in their homes. For more information, contact the health and
welfare office at 1-800 251-1777, and they will match needs and find a worker in your area.
Office hours are Monday to Friday, 9 a.m. to 5 p.m. Serves Los Angeles County.
Special Service for Groups — Benefits Assistance Clients Urban Project
Services offered include benefit assistance, emergency housing and vouchers (March through
August), bus tokens, employment preparation, and an education center. Volunteer opportunities
are available. Office hours are Monday to Friday, 9 a.m. to 5 p.m. Services by appointment only.
Free services. Serves Central Los Angeles.
Silver Lake Adult Day Health Care Center
The center provides seniors with transportation, meals, exercise, medication management,
physical and occupational therapy, and robust social programs. Office hours are Monday to
Friday, 8 a.m. to 2:30 p.m. Serves Los Angeles County.
Star Community Adult Day Health Care Center
The center provides nursing care, occupational therapy, physical therapy, meals and exercise.
Serves people with serious disabilities, brain injuries and chronic illnesses. Office hours are
Monday to Friday, 9 a.m. to 5 p.m. Serves Los Angeles County.
Safe Refuge
Coed residential substance abuse recovery program (90-day to one-year social model program),
outpatient, sober living, transitional and long-term services. Services for those living with
HIV/AIDS. Certified PC1000 Drug Diversion Program on site. Individual/group counseling in
anger management, alcohol/drug education, domestic violence, parenting and job training
services. State ID number: 190077RN. TTY 711. Coalition member. Office hours are Monday to
Friday 9 a.m. to 5 p.m. Admission requirement is adults 18 years of age or more with a desire to
stay clean and sober (and their children). Sliding fee scale. Low income and homeless may
qualify for some programs at no cost. Serves Los Angeles County.
Sunrise Community Counseling Center
Services offered include outpatient counseling, mental health/substance abuse counseling, a
domestic violence batterer’s program and a sex offender program. Also dual-diagnosis treatment,
family and child therapy, drug tests, victim support groups/counseling and a parenting program.

The LGBTQ Center offers counseling, treatment education, HIV/AIDS testing, support groups,

Monday to Friday, 10 a.m.to 9 p.m.; Saturday noon to 3 p.m. Admission requirement is
individual must have photo ID and income verification. Free services. Serves Long Beach.
The Midnight Mission
Hot meals Monday to Saturday, 6:30 a.m., 11:30 a.m. and 4:30 p.m. Free emergency beds
(separate room for families with children). Haircuts, showers (men and women) and weekly
clothing distribution with ID. Services include case management, life skills and court advocacy.
TB and HIV testing available. Limited medical, dental and vision care also available. Formal
live-in drug and alcohol rehab program for adult males. Alcoholics Anonymous and Cocaine
Anonymous meetings are available. Job referrals, a mobile kitchen, hygiene kits and family food
boxes available. Referrals for housing, substance abuse and other services. Monday to Friday 8
a.m. to 5 p.m.; 24-hour services. Admission requirement is individuals must be ambulatory.
Serves downtown, Skid Row, Los Angeles.
The Saban Community Clinic
Services include referrals, anonymous oral HIV testing with pre- and post-test counseling.

and Wednesday 8 a.m. to 8 p.m. Call to schedule an appointment. Serves Los Angeles County.
Watts Labor Community Action Committee
This location is one of the 21 Family Source Centers in Los Angeles County providing the
one-e-app system for financial assistance, case management and high-risk youth/young adult
employment/education services. Center also offers senior nutrition and transportation services for
residents of the city and county of Los Angeles. Additional services include child care, handy
worker program and social service programs. For transportation, call 1-323-563-4737. Office

services. Serves Whittier, La Mirada, Norwalk and nearby counties.
WISE & Healthy Aging
Nonprofit agency offering health care, psychotherapy, mental health and social services. Services
also include peer counseling, daily money management, friendly visiting, support groups and
health education screening. Day rehabilitation services available for older adults and their
caregivers. Office hours are Monday to Friday, 8 a.m. to 6 p.m. Accepts Medi-Cal, Medicare and
private insurance for some services. Some services free for qualified individuals. Sliding fee
scale. Serves primarily West Los Angeles and Santa Monica, but anyone is welcome.
Youth Outreach and Development
Youth Outreach and Development provides basic computer skills and Microsoft Word, Excel and
PowerPoint programs. These programs empower students to take charge of and be responsible for
their neighborhood, workplace, school and home. We also provide after school care, recreation,
training and skills for young African American and Latino athletes. Call 1-323-846-2500 to get
more information. Serves Los Angeles County Residents. Hours of Operation are Monday

YWCA/Wings Shelter
24-hr help line number with crisis intervention and referrals: 1-626-967-0658. Forty-five-day
shelter program for women and children who are victims of domestic violence available. Other
services include access to legal referrals, a restraining order clinic, support groups, community
education and substance abuse counseling. Other services include accompaniment to court, the
Department of Public Social Services and medical services. Office hours are Monday to Friday

Adult Day Health Care of Fresno and Clovis counties
Adult Day Health Care offers occupational and speech therapy as well as dietary and nursing
services. A registered nurse is present daily to evaluate and provide medical and nursing care.
Social work services are available to help participants locate needed services, and counseling is
provided to participants and their families. Serves individuals 18 years of age and older with a
physical or mental impairment. Hours of operation are Monday to Friday, 8 a.m. to 2 p.m.
Agency on Aging — Family Caregiver Support Program
The Agency on Aging offers services through their Family Caregiver Support Program. This
program provides caregivers with practical tools to alleviate stress and avoid burnout,
networking opportunities to decrease isolation and share concerns, and stopgap measures to
stabilize the provision of care during times of extreme difficulty. Services include support
groups, classes and case management. Serves Fresno and Madera counties; accepts Medi-Cal
members. Hours of operation are Monday to Friday, 8 a.m. to 4:30 p.m.
Alcoholics Anonymous (AA)
AA offers a self-help alcoholism recovery program utilizing the 12-step approach. Classes are
offered in English and Spanish at no cost individuals/families. Note, the center is only open for
meetings. Hours of operation are Monday through Friday from 10:30 a.m. to 4:15 p.m. and
Saturday from 9 a.m. to 1 p.m.
Adult Day Centers
OASIS and People Always Learn Something (PALS) Adult Day Centers specialize in Alzheimer
and dementia care — offering therapeutic activities for seniors, and allowing caregivers a
much-needed break. PALS is a program of OASIS for people 40 and older with mild cognitive
impairment in the early stages of dementia. OASIS and PALS provide a full day of stimulating,
recreational and therapeutic activities for people in the earliest to the most advanced stages of
dementia tailored to each participant's personal interests, needs, strengths, cognitive and physical
abilities to nurture self-esteem and dignity. Caregiver support is also provided. Medi-Cal will
not cover the cost of the programs, but financial assistance is available through private donations
and a grant through the Fresno-Madera Area on Aging. Offering attendance options to fit in
many budgets. Call 1-559-221-0396 for information about our full-day, half-day, and hourly
programs and share of cost of OASIS services. Serves Fresno County Residents. Hours of

Asian Pacific Islander Clinic
At the Asian Pacific Islander Clinic, case management teams provide comprehensive, culturally
and linguistically appropriate outpatient and community-based specialty mental health services
to transition individuals who meet medical necessity. Services are provided using a wellness and
recovery model and include assessments, clinical and rehabilitative services, case management,
peer support, and medication assistance. Hours of operation are Monday to Friday, 8 a.m. to 4
p.m.; walk-ins are accepted.
Behavioral Health — Adult Services
This program provides mental health services to low-income adults in order to help them with
their day-to-day lives. With locations in Bakersfield, Lamont, Delano, Frazier Park and Fresno,
psychiatrists, psychologists, clinicians and therapeutic support staff provide a wide range of
behavioral health care. Adult Behavioral Health Services provide psychiatric evaluations,
medication management, co-occurring capable and specialty mental health services and
comprehensive case management services. Serves Kern, Fresno, and Inyo County Residents.
Hours of Operation are Monday to Thursday 8 a.m. to 5 p.m.
Big Brothers Big Sisters of Central California
Big Brothers Big Sisters of Central California focuses on youth who are filled with promise but
whose life circumstances make it hard to achieve success. They connect children facing with a
volunteer mentor — one who is strong and enduring as well as professionally supported — for
1:1 relationships. They also provide recreational activities like sports. Hours of operation are
Monday to Friday, 8:30 a.m. to 5:30 p.m.
California MENTOR Family Home Agency (FHA)
The mission of California MENTOR is to offer adults innovative, quality services and supports
that lead to growth and independence regardless of the physical, intellectual or behavioral
challenges they face. In an effort to help people shape the direction of their own lives in
community-based settings, their philosophy emphasizes partnerships with those they serve as
well as their families, employees, mentors, payers and the communities in which they work.
Hours of operation are Monday to Friday, 9 a.m. to 5 p.m.
California Rural Legal Assistance Foundation
The California Rural Legal Assistance Foundation offers immigration services to marginalized
immigrant communities. Services include providing outreach and information on the
naturalization process as well as extensive community education, advocacy, training, and legal
and technical assistance. Serves rural California residents. Services are offered at no cost. Hours
of operation are Monday to Friday, 9 a.m. to 5 p.m.; appointments are required for assistance
regarding the naturalization process.

Central California Legal Services (CCLS)
The CCLS provides a range of services from advice to court representation for victims of abuse.
They assist with naturalization, legal permanent resident card renewals and U-Visa/VAWA
applications. CCLS employs attorneys, paralegals and support staff to provide civil legal
services at no cost. Other programs offered by CCLS include health services (advice and
representation for those having difficulty accessing health care services) and housing assistance
(representation for administrative hearings on Section 8 voucher terminations). Services are
offered at no cost. Hours of operation are Monday to Friday, 8:30 a.m. to 4:30 p.m.
Central Valley Regional Center (CVRC)
The CVRC offers advocacy, evaluation, assessment, information and referral, and coordination
of services to developmentally disabled children and adults. Serves Fresno, Merced,
Tulare/Kings, Madera and Mariposa counties.
Centro La Familia Advocacy Services Family Support Center
The Centro La Familia Advocacy Services Family Support Center provides a range of low-cost
immigration and naturalization services. They offer help through consultations and assistance
with family petitions, green card renewals and citizenship through naturalization. Serves remote
areas of Fresno County. Hours of operation are Monday to Friday, 8:30 a.m. to 4 p.m.
Clinica Sierra Vista Garland Health Center
The Clinica Sierra Vista Garland Health Center offers conventional HIV blood testing as well as
chlamydia, syphilis, gonorrhea and herpes testing. Serves Fresno County residents. Hours of
operation are Monday to Friday, 8 a.m. to 5 p.m.
Clubhouse Youth Program by Fresno American Indian Health Project
Clubhouse Youth Program mission is to enhance the growth and well-being of American Indian
Youth in Fresno, California, in a manner that is respectful to cultural and traditional values. The
Clubhouse is a no cost after school program for American Indian youth in the Fresno city limits.
Tutoring and educational activities. Summer youth camps ages 5 to 12 years. Offering peer
workshops that develop self-esteem, confidence, and job skills, prevention and awareness
activities that deal with substance/tobacco abuse and teen pregnancy prevention, and American
Indian cultural activities and field trips. Serves Fresno County Residents. Hours of Operation are
Monday to Friday, 8 a.m. to 5 p.m.
Comfort Keepers
Comfort Keepers provides in-home care that enriches lives and helps maintain the highest
possible level of independent living. Our Comfort Keepers staff members are carefully selected,
screened and trained to provide professional, compassionate care. We treat each of our clients

Encourage Tomorrow provides after-school and lunch-time youth development programs

Fresno Barrios Unidos is an organization that transforms communities by empowering youth and
families through advocacy, education and wellness. Programs offered include, El Joven Noble
(“The Noble Youth”) a youth development and support program and Teen Success, a program
that empowers teen mothers.
Fresno County Department of Public Health Community Health Division
The Fresno County Department of Public Health Community Health Division offers standard
hepatitis C and HIV testing, but they also offer rapid oral and blood HIV testing. Additionally,
hepatitis prevention/education, HIV test counseling, HIV/AIDS prevention education, partner
notification, and treatment information is provided. Hours for HIV testing are
Monday to Thursday from 8:30 a.m. to 11:30 a.m.
Fresno LGBT Community Center
The Fresno LGBT Community Center is open to the public. They offer access to information
and local and national LGBTQ resources including local publications such as News Link and the
CCA Newsletter. They offer reference materials relating to the LGBTQ civil rights movement as
well as printouts of recent court decisions affecting the LGBTQ community. Hours of operation
are Tuesday through Saturday from noon to 5 p.m.
Fresno Rescue Mission
The Fresno Rescue Mission offers an emergency family shelter that provides 14 to 30 days of
temporary shelter to families as well as food, clothing and personal care; referral to other
agencies that can assist them; and love/hope through spiritual support. To assist them in starting
over in an apartment, families are given food packages and other needed supplies when they
leave the shelter. Monday through Friday from 8 a.m. to 5 p.m.
Gay and Lesbian Center — Bakersfield
The Gay and Lesbian Center seeks to improve the well-being of the LGBTQ community with
cultural and educational programs and an extensive referral network. They offer community
building socials and other activities like picnics, dinner outings and camping trips. They also
offer a program that brings together lesbian, gay, bisexual, transgender and ally professionals

from Bakersfield and Kern counties together to celebrate and promote equality in the workplace
and 2030+Q-, a peer support and networking group for individuals under 40 years of age who

Guardian Angel Home Health Care-Rehabilitation Service
Guardian Angel Home Care provides quality home health services which meet the needs and
expectations of those injured, ill, disabled, or challenged. Guardian Angel Home Health Care is
comprised of multi-disciplinary professionals who have expertise in skilled nursing, physical
therapy, occupational therapy, speech therapy, medical social services, and home health aide
assistance. Serves California residents. Hours of operation are Monday to Friday,
8:00 a.m. to 5:00 p.m.
Heritage Adult Day Health Care
Heritage Adult Day Health Care offers therapeutic services to seniors. Transportation to and
from the facility is offered at no cost, and breakfast and lunch are provided. Patients typically
stay at the center for five to six hours. Nurses are on staff to monitor each patient’s health, social
workers provide 1:1 sessions, and nutritionists provide consultation. Every week, patients are
taken on field trips to various stores to purchase hygiene products. Serves Fresno County
residents; accepts Medi-Cal members. Hours of operation are 7:30 a.m. to 4:30 p.m.
Homeless Veterans Taskforce by The American Legion
To help our struggling homeless veterans, the Legion's Homeless Veterans Task Force, works to
ensure local services and resources are available to homeless veterans and their families. The
Task Force, which has chairpersons in each department, collaborates with government agencies,
homeless service providers and veterans service organizations to develop and implement
initiatives that will help homeless veterans. Additionally, Homeless Veterans Task Force
chairpersons can provide direct aid to veterans and their families who reach out to them. A
veteran who is homeless or facing possible homelessness can contact a chairperson, who will
provide guidance and assistance to the veteran and his or her family. Serves California
Residents. Hours of Operation are Monday to Friday, 8 a.m. to 5 p.m.
Kings View’s Blue Sky Wellness Center
The Kings View’s Blue Sky Wellness Center offers mental health services to adults by focusing
on key mental health recovery principles including dispensing hope, freedom of choice,
modeling self-determination and personal responsibility, and emphasizing that people can and
do heal from a mental health diagnosis. They offer self-help and support groups as well as a
variety of activities such as computer access, library time, a weekly clothes closet and a gym for

Legal Services for Immigrants by Mil Mujeres
Mil Mujeres providse legal services, community education and empowerment to low-income
Hispanic immigrants and their families in the United States. They offer immigration legal
services to the low-income individuals in the Latino community, specializing in immigration
benefits for survivors of violent crimes in the United States. Our main focus are survivors of
gender related crimes. However, we also provide services to other survivors. Offering help in the
following areas: U-visa (for victims of specific crimes), Violence Against Women Act, Deferred
Action for Childhood Arrivals, T-visa (for victims of trafficking), and Special Immigrant
Juvenile Status.
Madera Rescue Mission
The Madera Rescue Mission is committed to the needy, destitute and homeless. They not only

Marjaree Mason Center

Noami’s House
Noami’s House provides overnight shelter to homeless women 365 days a year. They also offer
services at no cost through the combined resources of the Collaborative including hot meals,
medical and dental care, a day shelter, laundry and clothing assistance, case management,
storage, transportation, health and personal living education, and information and referrals.
Assessments are offered Monday to Friday at 3:30 p.m. and Saturday and Sunday at 12:45 p.m.

Safe Houses — Fresno Emergency and Transitional Living Safe House
The Safe Houses program assists survivors of domestic violence by providing them with

Alameda County Public Health Department (ACPHD) Office of AIDS Administration
(OAA)
The mission of the OAA is to reduce the rates of new HIV infections, offer HIV testing and link
individuals who are HIV-positive with care. The OAA provides leadership, resources and
guidance in collaboration with community members and organizations to coordinate the delivery
of HIV/AIDS health services for the Alameda County Public Health Department. Serves

for teens), free food program for seniors, youth leadership development, dancing groups, sports,
on-the-job training, mental health case management and outreach, health access and outreach,

Behavioral and Community Services (BCS) by Terra Nova Counseling
Behavioral and Community Services provide a broad range of counseling services, BCS staff offer group counseling in anger management, parenting and co-parenting skills. BCS houses most
of the outpatient mental health services of the agency that are outside of the Children's Services. For clients seeking therapy or counseling, therapists meet with new clients to complete an intake assessment to determine a treatment plan. The length of services depends upon the specific
program or the needs of the client. BCS is able to accept some clients on a sliding scale, we are able to offer reduced fees based on your proof of income. Talk with the person who schedules your appointment about whether you qualify. Serves Sacramento County Residents. Hours of

Berkeley Free Clinic
The Berkeley Free Clinic offers services like acute medical care, tuberculosis testing, dental
services, and sexual transmitted infection (STI) and HIV testing. Accepts Medi-Cal members.
Hours of operation are Monday, Wednesday and Friday from 7 p.m. to 9 p.m. (same-day
appointments only); Saturday from 11 a.m. to 2 p.m. (drop-in, women and transinclusive clients
only); Saturday from 3 p.m. to 5 p.m. (drop-in and HIV testing only); and Sunday from 4 p.m. to
7 p.m. (drop-in, men's STI testing and HIV testing for all).
Bridge to Healthy Families by ACC Senior Services
Bridge to Healthy Families (BHF) is a program that seeks to improve access to comprehensive
support services for family caregivers and their care receivers. The program offers care
assessments and home safety checks, care consultations and caregiver support, support groups
and educational programs. Also offering information and referral for respite services, home
modification, medical alert, and more. Hours of operation are Monday-Friday 8 a.m.-5 p.m.

Catholic Charities of the East Bay — Immigration Project (Oakland)
Catholic Charities of the East Bay provides immigration and naturalization services and
programs for immigrants in the East Bay area. Services include green card application and
renewal assistance, family base petitions, Nicaraguan Adjustment and Central American Relief
Act form assistance, a violence against women program, adjustment of status assistance, and
help in completing other forms. There is a $40 fee for legal counseling appointments. Hours are
Monday through Friday from 9 a.m. to 5 p.m.; closed from 1 p.m. to 2 p.m.

Child Abuse Treatment Program (CHAT) by Child and Family Institute (CFI)
With funding awarded by the Federal Victims of Crime Act, Grant AT13101111, through the California Governor's Office of Emergency Services (Cal OES), CFI is able to offer
psychotherapy services to children and youth victims of crimes. CFI is able to serve children and youth victims who do not have access to such services due to a lack of resources, lack of insurance or Medi-Cal ineligibility. Treatment is offered to victims of physical, sexual, and
emotional abuse, parental substance abuse, child abduction, bullying and domestic violence. Serves El Dorado, Nevada, Placer, Sacramento, Sutter, Yolo, and Yuba County Residents.
Community-Based Adult Services by California Department of Aging
Community-Based Adult Services is a day health program for older adults and adults with certain chronic medical, cognitive or mental health conditions, or disabilities who are at risk of needing institutional care. This program used to be called Adult Day Health Care (ADHC). The primary objectives of the program are to restore or maintain optimal capacity for self-care to frail elderly persons or adults with disabilities and delay or prevent inappropriate or personally undesirable institutionalization. Services provided at the center include the following: professional nursing services; physical, occupational and speech therapies; mental health services; therapeutic activities; social services; personal care; hot meals and nutritional counseling; and transportation to and from the participants’ residence. Serves Sacramento County Residents. Hours of operation are Monday to Friday 8:00 a.m. to 5:00 p.m.
Community Resources for Independent Living (CRIL)
CRIL offers independent living services to those with disabilities. Services include housing
assistance, personal assistance referral, benefits advocacy, peer support and independent living
skills training. Serves Southern and Eastern Alameda county residents. Services are offered at no
charge. Hours of operation are Monday through Friday from 9 a.m. to 5 p.m.; closed for lunch
from 12:30 p.m. to 1:30 p.m.
Del Oro Caregiver Resource Center
The Del Oro Caregiver Resource Center is a nonprofit organization that serves individuals and
families who are providing care to brain-impaired adults and the frail elderly. Services include
information and referral, family consultation, family counseling, respite care, legal/financial
consultation, education and training, and support groups. Serves Alpine, Amador, Calaveras,
Colusa, El Dorado, Nevada, Placer, Sacramento, San Joaquin, Sierra, Sutter, Yolo and Yuba
counties. Most services are offered at no cost; some services require a small copayment. Hours
of operation are Monday to Friday from 8 a.m. to 4:30 p.m.
Emergency Housing-Family Shelter Program by Next Move
Family Shelter provides short-term emergency housing, life skills coaching, alcohol and drug recovery support and education, on-site SETA head start, laundry services, housing relocation assistance, case management for one year, assistance with clothing, shoes and housewares to homeless families up to thirty days. This program serves two parent families, single fathers, and sons over the age of 14. Serves Sacramento County Residents.

Emergency Youth Shelter by Wind Youth Services
Wind Youth Services provides supportive services and opportunities to youth experiencing homelessness as they pursue self-determined lives of stability and independence. Through our programs and services, Wind Youth Services provides homeless and at-risk youth between the ages of 12 and 24 with basic safety net and survival services and a safe place to stay. The Emergency Youth Shelter provides a temporary, low barrier, emergency overnight shelter for youth ages 12-17 (18 and in high school is okay). This LGBTQ-friendly, trans-affirming, pet-friendly emergency shelter is the only of its kind in the greater Sacramento area, and providing a safe, non-judgmental, and supportive community environment. Services include, hot meals, counseling, case management and more.
Fountain Recovery Drug and Alcohol Treatment
The Fountain Recovery Drug and Alcohol Treatment program offers help to individuals and
families affected by alcohol and drug addiction. The program is 12 step-oriented as well as
abstinence- and evidence-based and has a foundation of encouragement, love, practical guidance
and personal attention. Their small client-to-staff ratio allows for a client-centered approach that
offers individualized attention to meet each client’s specific needs. The program has four
different levels of care: residential treatment, day treatment, intensive outpatient treatment and a
sober living environment. Follow-up to treatment includes a free Aftercare Group for clients
who have successfully completed treatment. They accept most insurance. Hours are Monday
through Friday from 9 a.m. to 5 p.m.
Golden Rule Services
Golden Rule Services offers a variety of no cost, confidential HIV, STD and hepatitis C
education and prevention services that are accessible, affordable, and culturally and
linguistically appropriate. Services include rapid HIV and HCV testing, STD screening, needle
exchange, harm reduction services, HIV/STD risk reduction support groups, case management
services, individual peer-based risk reduction counseling, latex and polyurethane condom and
lubricant distribution, limited transportation to medical appointments, and information and

Health by Women’s Empowerment
Women’s Empowerment's mission is to educate and empower women who are homeless with the skills and confidence necessary to get a job, maintain a healthy lifestyle, and regain a home for themselves and their children. Health education and referrals include: wellness and recovery groups, onsite HIV and STI testing/counseling, mental health counseling and referrals, women’s health classes, counseling for mental wellness, yoga and fitness classes, and nutrition classes. Hong Fook Adult Day Health Care Center
The Hong Fook Adult Day Health Care Center provides individualized health care services to
frail adults. They enable individuals to remain in the community and prevent premature
institutionalization. Nursing services include health monitoring, medication assistance, personal
care, health education and liaison services with their personal physician. Rehabilitation services
include dietary consultation, nutritional education and daily hot meals. Recreational activities
include field trips, music programs, and birthday and holiday celebrations. Serves Alameda and
Oakland county residents who are 18 years of age or older; accepts Medi-Cal members. Hours of

Magnolia Women's Recovery Program, Inc.
Magnolia Women's Recovery Program, Inc. offers individual and group counseling for women
with substance abuse problems. They provide parenting classes, anger management, relapse
prevention and education classes (topics include Hepatitis C, HIV, domestic violence and
trauma). Serves Oakland County residents who are pregnant or have up to two children from
birth to 7 years of age; accepts Medi-Cal members. Program operates on a 24/7 schedule.
Mexican Consulate by Consulado de Mexico en Sacramento
The Mexican Consulate in Sacramento is appointed by the Mexican government to protect and promote the welfare and interests of Mexican immigrants living in the United States. The consulate offers a broad range of services including keeping the population up to date on relevant laws and regulation, providing a financial mailbox for Mexicans abroad that allows them to send and receive funds internationally, financial advice on problems with a financial institution in Mexico, assistance obtaining a Mexican passport. appropriate domestic violence intervention, community presentations and consultation, wish
lists, and volunteer opportunities. Serves Sacramento County women and children who are

Opening Doors
Opening Doors provides tools and support for survivors of human trafficking, refugees,
immigrants and other underserved residents. Services include refugee programs to support
refugees resettle and acclimate to a new country, a new workplace environment and new health
care and financial systems; a Prosperity Project to help residents improve their economic
situation by starting or expanding small businesses and to help them take control of their
personal finances by educating them about the U.S. financial system; a Survivors of Human
Trafficking program that provides case management to counsel victims of human trafficking;
and immigration legal services offering immigration services for nominal and affordable fees.

Rehabilitation Services Program by Woodside Healthcare Center
Rehabilitation Services Program provides post-hospital care to those recovering from strokes, orthopedic interventions including fractures and joint replacements, cardio-pulmonary conditions and more. Patients suffering from varying stages of Dementia and Alzheimer's Disease are given specialized care and recreational programs in a secure environment. Long-term care provides professional nursing and therapy for chronically ill or permanently disabled individuals. Residents at this level can often take care of many or all of their routine activities, yet typically require assistance with medical treatments, medication and maintenance therapy programs. RYSE Center (Richmond)
The RYSE Center offers youth programs and LGBTQ support. Programs include media, arts and
culture programs that promote community healing and social justice by facilitating young
people’s innovative creation of political, personal and expressive media and art; education and

management, academic counseling, transcript review, STEM programs, GED referrals and

Sacramento County In-Home Supportive Services (IHSS) Public Authority
The IHSS program helps pay for services to help residents remain safely in their home. Services
offered through IHSS include cleaning, meal preparation, laundry, grocery shopping, personal
care services (such as bowel and bladder care, bathing, grooming and paramedical services),
accompaniment to medical appointments and protective supervision for the mentally impaired.
To be eligible for IHSS, individuals must reside in California, have Medi-Cal eligibility
determination, live at home or an abode of his/her own choosing, and submit a completed Health
Care Certification form.
Sacramento Covered
Sacramento Covered is a community-based nonprofit organization with a focus on improving
the overall health of Sacramento county residents. Services connect individuals and families to
medical coverage, primary and preventative care, social services, and health care resources. The
main goal is to increase awareness and understanding of health care issues for uninsured and
underserved people; help the community overcome the challenges preventing them from getting
coverage; educate residents about their health care options; and offer in-person and
in-language assistance as well as provide onsite assistance for patients in local ER departments.

The Sacramento LGBT Community Center creates events, programs and pathways to services

The Sacramento County Mental Health Treatment Center is a locked, adult psychiatric health
facility providing acute inpatient services to indigent residents and Medi-Cal members. The
treatment center operates 24/7 and offers information regarding resources to individuals
experiencing a mental health crisis. Serves Sacramento County residents.
Second Chance
Second Chance offers an emergency shelter for individuals and families. The maximum stay is
two weeks for individuals and three weeks for families. Drug and alcohol programs are also

The treatment program is certified by the Commission on the Accreditation of Rehabilitation Facilities (CARF) and has helped countless patients achieve a healthier and more productive lifestyle. MedMark Treatment Centers takes a multi-modal approach to substance abuse treatment. Services include medication-assisted treatment, substance abuse counseling, physical evaluation, coordinated pregnancy treatment plans, relapse prevention and more. The Billy DeFrank LGBTQ Community Center offers no-cost rapid HIV testing. The center also
offers a CyberCenter which provides no cost use of internet-enabled computers; library/art
exhibits; support and social groups including for alcoholics anonymous, bipolar/depression
support groups, bisexuals and friends groups, crystal meth anonymous, Latino men's groups,
