Message from the IHMRI Chair Cast your mind back 10 years to 2008. What do you remember? The Global Financial Crisis, or the offcial apology to Indigenous Australians by Prime Minister Kevin Rudd? Perhaps it was NASA’s Phoenix Lander discovering ice on Mars?

In the midst of these headline events, an ambitious vision for health emerged in our region. The Illawarra Health and Medical Research Institute (IHMRI) was formed by the University of Wollongong (UOW) and the Illawarra Shoalhaven Local Health District (ISLHD) to help create a healthier Illawarra community, improve clinical practice, and build Wollongong’s reputation for research excellence.

IHMRI has grown at a rapid pace since its inception. New state-of-the-art facilities for research opened in 2010. The NSW Government recognised IHMRI as an independent Medical Research Institute in 2014, and in 2017, IHMRI established clinical trial facilities at Wollongong Hospital.

I am proud to be Chair of the Board and it is a privilege to contribute to IHMRI’s development. Working alongside my fellow Directors over the years has been very rewarding and I applaud their commitment to IHMRI.

I would like to thank Michael Bassingthwaighte AM, the highly respected former CEO of Lysaght Peoplecare, for his four years of service to the Board. Michael is now retired and we wish him all the best for the next phase of his life.

I am also very proud of our talented and dedicated researchers. Their collaborative and translational approach to research has enabled them to achieve outstanding results in publications, and continued success with grant applications, despite the high competition for funding.

Although IHMRI’s origins and achievements provide an inspirational foundation, we must not become complacent. IHMRI must employ a forward-thinking mind-set and invest in the right skills for the future. We must be ready to react to the changing nature of health and medical research in Australia, and the health needs of our community.

In this context, the Board has initiated a review of IHMRI’s governance structure, operations and leadership to ensure the organisation is prepared for the years ahead. A main goal of this review is to ensure that IHMRI has the right governance structure that will serve to improve national recognition and standing as an independent MRI. Our aim is to ensure that our researchers and support staff gain the true value and recognition for their efforts and contribution to IHMRI’s performance.

The drive and expertise of IHMRI’s Executive Management Team and operations staff will also help cement IHMRI’s future success. Throughout 2017-2018, they focussed on implementing the 2017-2021 Strategic Plan and the Research and Innovation Strategy 2017-2020. Their efforts have already created signifcant wins for IHMRI this year.

As IHMRI celebrates its 10th anniversary, it is important to refect that none of our achievements would be possible without the generous support of our founding partners, UOW and ISLHD.

It is remarkable what can happen in research when the brightest minds collaborate. Let’s work together to ensure the next decade is even more successful than the frst.

Message from the IHMRI Directors Researchers of the Illawarra Health and Medical Research Institute (IHMRI) are striving to fnd answers to our greatest health challenges such as cancer, diabetes, mental illness and dementia.

Every day, they question the boundaries of current knowledge and practices, working together to improve the health and wellbeing of people living in our region. Although our researchers work across diverse felds, they share the same commitment to translational research. They want to see knowledge and ideas converted into improved health care and new treatments as quickly as possible.

As a not-for-proft charity, IHMRI exists to support our researchers, whether they work in a laboratory, in a hospital ward or in private practice. Although it is very rewarding, health and medical research can be a challenging and competitive profession. That is why we provide a range of services to help our researchers. We want to provide a nurturing yet high-performing environment for research.

It is a dynamic time to be a part of IHMRI in the year of our 10th anniversary. Together with our dedicated operations staff, we are implementing the core pillars of our 2017-2021 Strategic Plan. As a result, we have expanded the services we provide to researchers, including grant writing assistance, commercialisation advice and media training. Our new personalised approach to support, which supplements our outstanding facilities, is producing remarkable results.

Due to improved research performance, IHMRI became eligible for increased Medical Research Support Program funding from the NSW Government. This funding for infrastructure will enable us to improve our services in specifc areas, including equipment, research development and professional training. We are also thrilled to see clinician researchers utilising our new clinical trial facilities at Wollongong Hospital to test new treatments for their patients.

We have also harnessed generous support from the community by

generating awareness of our researchers through the media and digital channels. We are very grateful for the generous donations we have received from the community this year, including the Saving Chloe Saxby campaign, the Illawarra Cancer Carers, Mr Richard Miller and the Great Illawarra Walk.

The frst ten years of IHMRI’s existence has been a journey of discovery. Not just breakthroughs in science and medicine, but discovering how we can assist in building the Illawarra’s reputation for health and medical research.

As a major participant in our region’s health and medical sector, we want to create a strong culture of high-quality research and innovation. To this end, our staff have also started to implement activities in our Research and Innovation Strategy 2017-2020. We are very excited by this new and pioneering approach to research and look forward to seeing it build our researchers’ skills in health analytics, clinical research and commercialisation.

As you will see when you peruse the pages of this report, 2017-18 has been a period of notable growth and achievement. We would like to thank the Board of Directors for their leadership, Chief Operating Offcer Kara Lamond for her stewardship of the operations staff, and to our partners—the University of Wollongong and the Illawarra Shoalhaven Local Health District—for their continued support.

Our multidisciplinary researchers work within and across three broad research themes.

Diagnostics and therapeutics

Researchers in this theme explore the fundamental mechanisms of disease. They use this knowledge to design,

test and evaluate new drugs as well as medical devices. They work together to increase treatment options and quality

of life for patients with complex medical conditions.

Mental health and the ageing brain

Researchers in this theme examine the mechanisms, causes, prevention and treatment of a wide range of

chronic and complex psychiatric illnesses, neurodegenerative diseases and neuro-developmental disorders. Together they seek to address the burden of mental

illness and neurodegenerative diseases in the region.

Chronic conditions and

lifestyle Researchers in this theme study the links

between disease and lifestyle risk factors, such as excess weight, poor diet, high blood pressure, drug addictions and physical inactivity. They work together

to fnd better treatments and prevention strategies to extend

Is mobile technology safe?

NHMRC Centre of Research Excellence dials up the facts You can’t see it or hear it, but it underpins so much of modern society. Radiofrequency (RF) electromagnetic energy is a type of radiation that supports modern communications. It enables us to talk on our mobile phones, watch television and listen to the radio.

Although these technologies have delivered substantial benefts to society, they have also raised concerns that frequent RF exposure may pose an unknown but pervasive health risk.

Established in 2012, the Australian Centre for Electromagnetic Bioeffects Research (ACEBR) is a National Health and Medical Research Council (NHMRC) Centre of Research Excellence that is investigating a range of possible health risks, including neurodegenerative disorders, sleep disturbances, and the impact on cellular processes in the body.

Led by Dr Sarah Loughran from the School of Psychology at the University of Wollongong (UOW), ACEBR is based at the Illawarra Health and Medical Research Institute (IHMRI), with other participating institutions including RMIT University,

Swinburne University of Technology, the University of Adelaide, the Victor Chang Cardiac Research Institute and SA Pathology.

Being part of the multi-disciplinary environment at IHMRI fosters collaboration with other scientists, including cancer researchers and experts in neurodegenerative diseases. The group also regularly use IHMRI’s clinical trial facilities, including two sleep laboratories, to conduct research into the effects of mobile phone use before sleep.

With over six billion mobile phone subscriptions world-wide, the results of ACEBR’s research is eagerly awaited by both industry, risk assessment agencies, such as the World Health Organisation, and the community.

A ‘diffcult’ infection

Fighting hospital gastro outbreaks When you go to hospital, you expect to get better. Unfortunately, recovery and healing is being jeopardised in hospitals around the world by an antibiotic-resistant gastro bug called Clostridium diffcile (C.diff).

C.diff attacks the gut resulting in diarrhoea and infammation of the colon. In severe cases, doctors must surgically remove parts of the colon. In 2011, C.diff was responsible for approximately ½ million infections and 29,000 deaths in the United States alone.

The spores are resistant to drying and the usual chemical cleaning agents, and can therefore remain in the environment for weeks or months.

Professor Stephen Pyne and Professor Paul Keller from UOW’s School of Chemistry are investigators on two Project Grants from the National Health and Medical Research Council (NHMRC) to study C.diff. They are exploring how C.diff spreads, and developing new drugs that both treat the infection and prevent its recurrence.

“We are developing new antibiotic drugs that mimic natural antibacterial peptides... We are also trying to make the drugs smaller and therefore easier and cheaper to synthesize.” Professor Stephen Pyne

They work with collaborators at the University of Western Australia (Professor Thomas Riley) and Monash University (Professor Dena Lyras).

Thanks to the efforts of these IHMRI researchers and their collaborators, health care providers have hope that insidious and costly infections like C.diff will one day be contained.

Can probiotics ease symptoms of schizophrenia? Pilot study in Shellharbour In Australia, approximately 1 in 100 people have or will develop schizophrenia during their lifetime and it is usually a life-long illness. This debilitating brain disorder ranks among the top 10 causes of disability in developed countries worldwide.

Current antipsychotic medications often do not alleviate negative symptoms such as memory loss, lack of attention and blunted expressions. In response, IHMRI researchers are investigating the link between bacteria in our stomach and the brain to identify new ways to improve cognitive function.

Dr Katrina Green, Distinguished Professor Xu-Feng Huang, Professor Nagesh Pai and Dr Jason Clark are conducting a pilot study at Shellharbour Mental Health Service, which

involves measuring the microbiota (or gut bugs) of people with schizophrenia using genome sequencing. The collaborative team will then compare tests of their cognitive function with their gut microbiota results.

There are around 1000 trillion microbial cells or bugs in the gastrointestinal system. Probiotics are mostly bacteria, which assist in the natural balance of microfora in the intestines.

“The results of the study will help us identify which gut microbes to target,” explains Professor Nagesh Pai.

An IHMRI Collaborative Project Grant funds this research. The researchers will use the results to support an application for NHMRC funding.

Boost for kidney disease research NHMRC Trip Fellowship for renal dietitian The Illawarra-Shoalhaven has one of the highest rates of chronic kidney disease in Australia, with 19.5 per cent of the region’s adult population showing signs of the disease.

In 2017, renal dietitian and PhD candidate, Kelly Lambert, become the frst clinician from the Illawarra Shoalhaven Local Health District to receive a TRIP (Translating Research into Practice) Fellowship from the National Health and Medical Research Council (NHMRC).

She will use the two-year Fellowship to develop a health literacy and diet program for people with chronic kidney disease.

“Dietary modifcation is a key part of the prevention and management of chronic kidney disease,” said Ms Lambert.

“The project itself follows on from my fndings of my PhD – namely that cognitive impairment and low health literacy are common in those with end stage kidney disease, and that this impacts how people learn and apply advice about managing their kidney disease.”

A pilot study of the program has already shown exciting results. Ms Lambert will roll out a larger study in 2019, with results expected by the end of the year.

As part of the professional development component of the Fellowship, Ms Lambert will complete a health economics degree and further training in Canada.

An Advanced Accredited Practising Dietitian in the Renal Unit of Wollongong Hospital,

Ms Lambert cares for patients with end-stage kidney disease who are on dialysis or who have received a kidney transplant.

In 2017, IHMRI recognised Ms Lambert's signifcant contributions in her feld of health care by awarding her the inaugural Dame Bridget Ogilvie Clinical Excellence Award, worth $3,000. These Awards aim to address the barriers to career progress faced by female researchers, particularly in regional areas.

Ms Lambert decided to use her award to help with the costs of visiting the Centre of Innovation for Complex Chronic Healthcare at the Edward Hines Veterans Affairs Hospital in Chicago. Her goal was to learn

from clinical researchers there and adapt their best practices into an Australian setting.

In addition to her clinical responsibilities, Ms Lambert has extensive teaching experience. She has supervised more than 50 research students, from the felds of nutrition, dietetics,

medical science and graduate medicine.

Her drive and determination to help patients led to an interesting technological collaboration. In 2016, she worked with xyris software and Clinical Professor Grahame J. Elder from the Garvan Institute to develop an iPhone app to help sufferers of chronic kidney disease manage their diets. The app allows patients to track their daily consumption of potassium, phosphate and sodium, as well as their calories.

IHMRI is proud of the support we have been able to provide Ms Lambert and look forward to seeing the results of her work from the TRIP Fellowship.

New standards for mental health nursing Professor Elizabeth Halcomb from the University of Wollongong’s (UOW) School of Nursing is the chief investigator on a project that will help improve mental health care in Australia.

Funded by the Australian College of Mental Health Nurses 2017-2018, the project will defne the primary health care roles of specialist Mental Health Nurses (Credentialed MHNS and MH Nurse Practitioners) and practice standards for nurses working in general practice settings.

This research will aid decision-making for regulators and policy makers. It will also assist in the development of a sustainable nursing workforce that is responsive to the mental health needs of the community across all care settings.

"It’s really important to make sure nurses are working to the full extent of the nursing scope of practice. If they’re not doing as much as they can, they’re less likely to be happy in their jobs and the patients aren’t going to get the best outcomes.” Professor Elizabeth Halcomb.

In 2018, Professor Halcomb's career received international attention when she was inducted into the Nurse Researcher Hall of Fame by the Sigma Theta Tau International Honor Society of Nursing. She was the only Australian to receive this recognition.

The accolade celebrates Professor Halcomb's world-class research on primary care nursing and its infuence on health care policy. It also recognises her role as a mentor to students and early career researchers.

A new hope for Chloe and Holly Great Illawarra Walk steps up funding for stem cell research The plight of Chloe Saxby and Holly Burns has touched the hearts of many people in the Illawarra and Shoalhaven. They both suffer from a rare genetic disorder, Vanishing White Matter Disease (VWM), a degenerative condition that affects the nervous system, causing problems with movement and mental functioning. Current treatments only alleviate the symptoms and there is no cure.

Associate Professor Justin Yerbury wanted to help. As an expert in neurodegenerative diseases, he believed that the Illawarra Health and Medical Research Institute (IHMRI) had the right expertise and facilities to make a difference.

His colleague, neuroscientist Dr Lezanne Ooi, is now leading a research project to identify personalised treatments for Chloe and Holly using the latest techniques in stem cell technology. Previously, the only research into VWM was happening overseas.

In March 2018, the Saxby and Burns families generously donated skin cell

samples in IHMRI’s clinical trial facilities. From these samples, Dr Ooi’s research team will grow a replica of VWM in a test tube so that FDA approved drugs can be tested. The researchers also hope that this process will provide new insights into VWM.

Government funding rarely covers research for diseases like VWM, so the Saxby and

Burns families have worked tirelessly to raise awareness and support for their daughters.

The community-minded spirit of the Illawarra-Shoalhaven has lead to signifcant donations. For example, organisers of the Great Illawarra Walk, Chris and Ruth Lovatt, raised over $100,000 for IHMRI's research into VWM.

IHMRI researchers and staff, led by Dr Lezanne Ooi, took part in the 100 km two-day walking event in March.

“This act of generosity by the Illawarra community will help fund the ongoing research into Vanishing White Matter Disease here at IHMRI,” stated IHMRI CEO and Executive Director, Professor David J. Adams.

Due to the event's success, the Great Illawarra Walk will also support IHMRI research into VMW in 2019.

IHMRI would also like to thank the Oak Flats branch of Bendigo Bank for their generous donation towards research for VWM.

Tackling our greatest health challenges

raised at each fundraising raffe goes to one lucky ticket holder who is drawn at random during the event. The remaining 50 percent goes directly towards the charity.

A commercial played during the matches on the big screen, as well as repeated announcements, assists with the fundraising and helps promote IHMRI to a large and diverse audience.

So far, the IHMRI Fundraising Dragons have raised just over $6,500 over three games in 2017-2018. IHMRI will use the funds raised to assist emerging and talented researchers. Go the Fundraising Dragons!

Members of the community seek more funding for GVHD research Graft Versus Host Disease (GVHD) is a complication of bone marrow or stem cell transplants. Neil Pennock knows frst-hand the devastating impact of the disease. His partner, Trace Richey, developed GVHD after a bone marrow transplant at St Vincent’s Hospital in 2015. This procedure was supposed to treat Mr Richey's blood disorder. Instead, he died of GVHD just 44 days after the surgery.

GVHD is the leading cause of non-relapse associated deaths in patients who receive donor stem cell transplants. Symptoms may include skin rash, intestinal problems and liver dysfunction.

John Morris is another member of the community who assists with GVHD research at IHMRI. Mr Morris' commitment to supporting cancer research began after he underwent treatment for Burkitt’s lymphoma. He spent four months in Wollongong Hospital in 2010 with GVHD.

Mr Morris' involvement with the GVHD research project includes assisting with grant applications, providing advice and feedback.

With bone marrow transplants increasing, both in Australia and around the world, incidents of GVHD are expected to rise. This direct collaboration between researchers and the people affected by the disease is vital to help raise awareness and opportunities for funding.

Nanostring machine for cancer research Illawarra Cancer Carers donation enables speedy gene analysis In 2017, the Illawarra Cancer Carers (ICC) donated $130,000 to IHMRI researcher Professor Marie Ranson to go towards the purchase of specialised diagnostic equipment for laboratories at the Illawarra Health and Medical Research Institute (IHMRI).

It’s a state-of-the-art tool that dramatically speeds up the process of gene function analysis.

For Professor Ranson this local support is invaluable for the continuation of her work, especially since competitive government funding does not always cover the high-costs associated with cancer research.

Since 1990, volunteers from The Illawarra Cancer Carers (ICC) have raised $3.6 million dollars for cancer research and care in our region. Over the past decade, IHMRI cancer researchers have received more than

$800,000 to support cancer research.

ICC also donated an additional $70K to go towards resources for a project to develop a novel small inhibitor of cancer, conducted by Research Fellow, Benjamin Buckley. They are also assisting Dr Kara Vine-Perrow as nominated Informed Consumers in a major grant application to the National Health and Medical Research Council for a pancreatic cancer project.

ICC volunteers fundraise year-round through stalls, raffes and events, including the annual banquet. Unlike other fundraising initiatives for cancer research, all money raised by the ICC is spent locally to support cancer patients in the Illawarra.

New electrophysiology facility Australian Research Council LIEF Grant for Syncropatch IHMRI is set to get a state-of-the-art robot that will enable researchers to do a year’s worth of work in just one day. The new electrophysiology facility is only the second of its kind in Australia and could lead to faster drug discoveries and treatments.

The patch-clamp technique allows scientists to measure the electrical activity in single living cells and their responses to different drugs. This powerful new technology will speed up the process of cell analysis and signifcantly reduce laboratory time for researchers.

David J. Adams, was instrumental in securing awarded to a partnership of scientists from Australian Research Council (ARC) funding IHMRI, the University of Sydney, the University under the Linkage Infrastructure, Equipment of New South Wales and the Victor Chang and Facilities (LIEF) scheme. Our region’s growing and diverse population, and incidence of chronic disease, makes it an ideal location for clinical trials.

In August 2017, IHMRI offcially opened a clinical research and trials unit (CRTU) at Wollongong Hospital to supplement existing CRTU facilities at the University of Wollongong.

The CRTU at Wollongong Hospital is operated by experienced clinical trials nurses and features patient examination rooms, blood collection facilities, a -20oC freezer and a secure pharmacy.

The CRTU is now being utilised by a range of clinician researchers to test new treatments, drugs and devices.

Infectious disease researchers, Clinical Professor Spiros Miyakis and Dr Omar Shum, are working on a study to determine the analgesic effcacy, safety and tolerability of a new topical treatment for patients with painful wounds.

Gastroenterologist Dr Thomas Lee is also working on fve studies testing the effcacy of new treatments for Crohn’s Disease and ulcerative colitis.

Scientifc Advisory Committee IHMRI Directors appointed a new Scientifc Advisory Committee (SAC) for 2017-2019.

The SAC provides advice on IHMRI’s research priorities and research strengths, as well as guidance on funding opportunities. The SAC also participates in reviews of research initiatives and provides a broad perspective of national and international trends in health and medical research.

The SAC consists of up to eight members, including IHMRI’s Executive Director and Clinical Director, and external members who serve for an initial term of three years.
