Our Region Our footprint covers an area of 123,175 square

kilometres and extends from Croydon, Forsyth and Georgetown in the south to the Torres Strait

Islands off the Northern Peninsula. It includes Cairns, the Atherton Tableland and Cape York

Peninsula.

In 2012 the region’s population was approximately 266,290. This population includes 47,133 (17.7 per cent) Aboriginal and Torres Strait Islander people

and 31,954 (12 per cent) people aged 65 years and over.

The Board is accountable to members and stakeholders for HRe’s overall performance and responsible for steering the growth and development of the company.

Ian Hook (Chairperson) Ian is a graduate of Murdoch University with a Master’s in Business and has completed four programs at Harvard Business School in Performance Management and Quantifiable Outcome Management within the Not for Profit Sector. He was for five years the Chief Executive Officer (CEO) of Tropical Medical Training and has a very good understanding of health policy and health services in northern and remote Australia. He is a former National CEO of Arthritis Australia, an advisor to the Western Australia Health Minister in Health Planning and a former Director of Mission Australia and Foodbank. Ian was Chair of the HRe Finance, Audit and Risk Management Committee and Nomination Committee and also a member of the HRe Governance Committee.

Deb Hancock (Deputy Chairperson) Deb Hancock was the Chief Executive Officer (CEO) of the Cairns Chamber of Commerce for almost five years up until August 2017. Deb has now accepted the position of CEO at Scitech in Western Australia. She also works with a number of community groups and boards. In addition to this philanthropic work, Deb is a highly motivated business professional, and business owner, with senior management experience in both the private and public sectors. Deb has proven ability to drive outcomes and engage effectively with a range of government, business, industry, and community stakeholders; along with experience working in a range of industries and sectors, such as mining and resources, local and state government, financial services and NFPs. Deb has a Bachelor Commerce/Bachelor Business; is a Certified Practicing Project Manager, has a Diploma of Project Management and a Diploma of Public Relations, Promotions and Publicity. Deb is Chair of Governance Committee.

Gail Rodgers – until February 2017 Gail has clinical experience as a Speech Language Pathologist in various parts of Australia and the United Kingdom. She specialises in working with teenagers and adults with communication and/or swallowing challenges and facilitates Vocal Boot Camps for people of all ages who use their voice within their profession of personal leisure time. Gail was rewarded for her contribution to health in the Far North at the recent Cairns Business Woman’s Club awards as the recipient of the Small Business Owner award 2014. Combining these skills with her membership with Woman On Board’s, the Australian Institute of Company Directors and her Vice-Presidency of FNQAHA Gail has a strong voice on multiple levels for the community members in the Far North.

Joanne Leamy Nomination body: FNQ Community and Practice Nurse Association (CAPNA) Joanne is a Registered Nurse, with 21 years’ experience in the sexual health and HIV sector. Prior to this she had 13 years’ practice nurse experience. Joanne has an MA (Soc.Sci) and is Clinical Coordinator of the Men’s, Woman’s and Sexual Health Program with Torres and Cape HHS. Joanne is President of Queensland AIDS Council, Secretary of Australian Federation of AIDS Organisations and President of FNQ Community and Practice Nurse Association. Joanne was the Chair of the HRE Governance Committee and now Chair of the Finance, Audit and Risk Management Committee.

Julianne Boneham – since February 2017 An ex-registered nurse, Julianne is a business owner and has been Chairperson of the Wuchopperen Health Service and representative on the Northern Aboriginal and Torres Strait Islander Health Alliance Ltd. Julianne brings to the board strong community links and networks with Aboriginal and Torres Strait Islander health organisations within the FNQ footprint, strong business management and communication skills, strategic planning, experience in health, leadership skills and strong Board Governance practice.

HRe people have strong industry connection, cultural intelligence and a capacity to deliver new and innovative solutions.

Ingrid Steed - Chief Executive Officer Ingrid is a senior registered nurse and specialises in health service leadership. Her experience includes leading health programs for over 25 years in many regional areas of Queensland most recently in the Community and Primary Health Sector. Ingrid holds a Bachelor Degree in Nursing majoring in Primary Health Care and has a certification as a mediator with the institute of Arbitrators and Mediators Australia. Additionally, Ingrid holds memberships with the Australian College of Nurses and the Australian Institute of Company Directors. As the Chief Executive Officer of Health Reimagined, Ingrid applies her expertise to ensure effective cross functional project teams, coordination with external key stakeholders and clients and ensuring programs are meeting key performance requirements. Ingrid was previously a Non-Executive Director of the Far North Queensland Medicare Local Board where she also held the position as Chair of the Finance, Audit and Risk Management Committee.

Alan Evans - Finance and Administration Manager Alan has over 40 years of experience in company accounting, financial control, compliance, audit and corporate services. Alan holds Certified Practice Accounting qualifications and has held senior accounting roles in the public and not-for-profit sectors. He has held senior accounting, corporate service and company secretary roles in the public, not-for-profit and private sectors in Victoria, New South Wales, Queensland and Papua New Guinea. Industries included aviation, engineering, manufacturing and primary health care. Alan is a member of the Finance, Audit and Risk Management Committee.

Jason Trim - Business and Operations Manager Jason has over 20 years’ experience in sales, marketing, business development, management, strategic commercial partnerships, sponsorships, innovation and networking specialises in B2B and B2C predominantly in the Queensland market. Jason has a Certificate of Business and has several qualifications in leadership and management with the Australian Institute of Management. Jason brings a wealth of knowledge and experience from the private sector having lead and worked in teams in various industries including pumps and irrigation, fasteners, professional sport, tourism, marketing and advertising and labour hire and recruitment. Jason also operates a consulting form Barking Gecko Consulting.

Barbara Somerville - Strategic Support Officer Barbara is a registered nurse with years of experience. Barbara has held various senior roles within the health industry and brings with her a wealth of knowledge and experience. Barbara is responsible for quality management, accreditation and policy and procedure development at Health Reimagined. Barbara is also responsible for the coordination of two programs: Aged Care and Allied Health and is an integral addition to the Access to Allied Psychological Services (ATAPS) team, overseeing the triage of patient referrals.

HRe has been successful in obtaining a Contract Extension from the Northern Queensland Primary Health Network (NQPHN) for Access to Allied Psychological Services (ATAPS) to 31 December 2017. A further predicted extension until 31 March 2018 is likely, when the new Mental Health Stepped Care Services is expected to commence. HRe was successful in its Expression of Interest (EOI) with the NQPHN for the Mental Health Stepped Care Services and is actively engaged in interactive procurement workshops. This includes selection to participate in response to tender for the intake triage and clinical governance component of the NQPHN preferred Model. HRe will respond to undertake this role for the whole of the NQPHN footprint from Mackay to Torres Straits. HRe will be offered to complete the registration to participate in the Mental Health Stepped Care as a service delivery agent and contracts will be awarded for 18 months. HRe partnered with James Cook University, Selectability and North West and Remote Health for the Mental Health Stepped Care Services EOI which is testimony to the organisation’s standing and reputation. HRe was successful in an EOI to the NQPHN be a consulting agency for Health Care Homes Readiness Program for General Practice. HRe was successful against a tender in the Healthy Active NQ Grants for the NQPHN. $15K was awarded and will see 45 representatives from sporting clubs trained in Mental Health First Aid. HRe was proactive in the training arena and saw stakeholders embrace; Specialised Psychological Dialectal Behaviour Training, Mental Health First Aid, Suicide Assist Skills Training and more generalised Human Resource Training for General Practice and Allied Health Practice. The NQPHN also asked HRe to expand this training on their behalf in Townsville and Mackay to similar cohorts. The sessions were a great success. We undertook six stakeholder engagement meetings with our mental health providers which saw approximately 20-25 on each occasion come together to network and discuss program specific matters. We commenced and embedded new relationships as we went in as partners on a joint tender to the NQPHN for the Integrated Team Care (ITC) Program. This was successful and has seen us partner with Northern Australia Primary Health Limited (NAPHL) in delivering Indigenous support within the Cairns and Tablelands regions. Additionally, the relationships with private Allied Health Providers was strengthened with the networking events to launch Allied Health Workforce initiatives in the aged care sector. Service delivery in this cohort was delivered in the Indigenous space to Mutkin Aged Care facility at Yarrabah. We further supported this sector with training pertaining to the Aged Care Funding Instrument (ACFI) and was attended by eight Aged Care organisations. HRe was successful in being selected to rollout a Queensland Government Initiative, “My Health for Life” which saw our Allied Health Partners engaged in chronic disease group education and support.

HRe was invited by NQPHN to participate as a Platinum sponsor of the myPHN Conference which was held at the Pullman Reef Casino, from 8-9 July 2017. The theme was ‘Transforming healthcare together’ and it focused on connecting health providers to deliver better health outcomes through general practice, Aboriginal and Torres Strait Islander medical services, allied health, pharmacy, dental, and hospital and health services. As Platinum Sponsors, HRe were provided with trade display booths, marketing by way of print, satchel inserts which were taken home by each visiting delegate, corporate banner display, digital advertising, social media acknowledgment, verbal recognition, access to the delegate list, and the opportunity to present during the conference opening. HRe Deputy Director, Deb Hancock did the honour and delivered an exceptional opening speech which was very well received by all the delegates.

HRe is currently tendering for the National Disability Insurance Scheme, to perform the role as Local Area Coordinators for the region. HRe is concurrently undertaking an education package with the Royal Australian College of General Practitioners (RACGP) for Mental Health Skills Training (MHST) which is clinical training that General Practitioners must do each triennium if they want to claim a higher fee item number when completing a mental health care plan. Currently there is no MHST that addresses the Stepped Care model as a topic. With each PHN and their respective commissioned bodies launching the Stepped Care Model it provides an excellent vehicle for orientation, education, engagement and ticking the boxes for reporting. As part of the Stepped Care EOI, the PHN has stated that the successful organisation must:

Increase the capacity of General Practitioners (GPs) to identify and support people with mental health issues Increase the capacity of the primary healthcare workforce and other workers in education, emergency, and welfare sector to support people with mental health issues Develop and support the primary health care mental health workforce

Access to Allied Psychological Services (ATAPS) Cairns & Hinterland | Cape & Torres Regions The ATAPS Program has continued to meet and surpass key performance indicators of contract milestones. In June 2017 the program received and processed record numbers of referrals. A further highlight of the year has been the establishment of a strong relationship and collaboration with Acute Care Team (ACT) from Cairns and Hinterland Hospital and Health Service, to review and discuss client cases resulting in improved continuity of services for clients presenting through the Acute Services. HRe clinical staff attended daily meetings of the ACT to facilitate this collaborative effort, creating a better understanding between the services and improved outcomes for clients. To better service the increasing referrals, more mental health professionals were sub-contracted across the Northern Queensland Primary Health Network (NQPHN) footprint resulting in total of 66 providers offering services in locations from Thursday Island to Innisfail and west to Georgetown and Croydon. The increase in services to the Cape and Torres region was another pinnacle in service expansion. During the year HRe engaged regularly with Providers, NQPHN and the ACT Team through quarterly provider breakfast meetings that were well attended and received as a forum to exchange ideas and receive updates on the program. HRe statistics 2015-16 versus 2016-17

• 122% Increase in Contracted Mental Health Professionals to deliver services (currently have 66 Contracted Mental Health Professionals);

• 35% Increase in Referral Pathways/Partners developed; • 48% Increase in Referrals Received from Referral Partners; • 62% Increase in Occasions of Services delivered; • 77 locations Mental Health Services being delivered; and • Increased delivery via telephone and video to rural & remote areas.

HRe continues to use the HealthE Platform, developed in 2015 to support the delivery of its ATAPS Program. It provides a streamlined connection with a central database which can be accessed in house and remotely. The cloud based electronic platform enables processing of the patient referral from receipt of referral through to triage and allocation of client to provider thus creating a central processing point for the administration of the program. This platform has improved the time it takes for clients to receive access to services, in addition to widening the scope for access to services in diverse locations, through utilisation of a new model allowing for dispersed geographical placement of sub contracted staff. The benefits to HRe are many, however the stand out has been the efficiency of the processing of applications, access for the clinical triaging process and the reduction in staffing administering the program.

The new system has enabled the model to also be delivered as a full contract model of service which has enabled the service to expand to more remote areas. The effectiveness made in program savings have enabled a wider reach, by reinvesting in service delivery. HRe have achieved significant improvement in service delivery of Mental Health Services through innovation, partnerships, delivery, up to the second data and education. Indigenous Health and Wellbeing - Integrated Team Care (ITC) HRe facilitate and deliver the Integrated Team Care (ITC) Program for Cairns, Tablelands, Douglas, Croydon and Etheridge in partnership with Northern Australia Primary Health Limited (NAPHL). The aim of the ITC Program is to improve access to mainstream primary care and contribute to Closing the Gap in life expectancy by improving services available to Indigenous Australians. HRe have successfully developed referral pathways and partnerships within the footprint to promote the program and educate and inform community members. HRe have developed a strong relationship with Kuranda community and Kuranda medical to deliver services within the region. Care Coordination Supplementary Services (CCSS) Program (part of the ITC Program) The CCSS program supports Aboriginal and Torres Strait Islander people who have a chronic disease to look after their health. Clients in this program have a Care Coordinator who will work with them, their doctor and other community services to help them access the resources they need such as arranging appointments, transport to and from, assistance to understand and follow care plans, payment of ‘gap’ fees not covered by Medicare and hire or purchase of equipment. HRe employ two indigenous health workers to deliver the program. Indigenous Outreach Worker (IOW) Program (part of the ITC Program) The program assists Aboriginal and Torres Strait Islander peoples to access doctors, specialist appointments, access cheaper or free medications and health services. The IOW acts as a link or resource between health services and the Aboriginal and Torres Strait Islander peoples to assist in breaking down any barriers. HRe employ three IOW’s to deliver the program. Allied Health Services HRe have developed an Allied Health Services Program to provide a booking and one invoice service for Aged Care Facilities and General Practices. The program contracts Allied Health Professionals within the Cairns Region to deliver health services. The one phone call to book various allied health services and the one invoice for all services provided at the end of month model is an option for Aged Care Facilities and General Practices to utilise as it takes away the hassle of dealing with individual operators in which multiple bookings to individual operators are to be made and multiple invoices to be paid at end of month.

My Health for Life (MH4L) HRe was successful in the application to become a contracted MH4L Program Facilitator Organisation. The MH4L Program is a Queensland Government funded program facilitated by a health alliance which includes Diabetes Queensland, the Heart Foundation, the Stroke Foundation, the Queensland Primary Health Networks, the Ethnic Communities Council of Queensland, the Queensland Aboriginal and Islander Health Council, Queensland University of Technology and Ernst and Young. MH4L is a Queensland integrated risk assessment and behaviour modification program that aims to assist participants to reduce their risk of developing chronic conditions such as type 2 diabetes, heart disease and stroke. The MH4L Program offers eligible community members, diagnosed with chronic disease, six sessions with a health professional providing personalised coaching and support over a six-month period. The program focusses on teaching community members the skills to get started with and active and healthy lifestyle and works in partnership with participants along the six-month journey rather than providing information only. HRe engages contractors and facilitators to deliver the MH4L Program within the Cairns region, delivering 86 sessions to 21 participants. National Disability Insurance Scheme (NDIS) The NDIS will support a better life for hundreds of thousands of Australians with a significant and permanent disability, and their families and carers. The NDIS will mean peace of mind for every Australian – for anyone who has, or might acquire, a disability. The priority of the NDIS is to ensure people with disability continue to get the support they need. The changes that are required to existing disability support systems are significant. Arrangements are being made to ensure the scheme can be introduced gradually, ensuring a smooth transition for people with a disability and support providers. HRe has registered as a Provider for NDIS Services to best place ourselves in the upcoming roll-out of the program in 2018. HRe has submitted a tender for the NDIS Partners in the Community Grant Round Three – Local Area Coordination Services, and are awaiting the results. Education & Training Mental Health First Aid Training HRe have commenced delivering Mental Health First Aid Training to businesses, community members and organisations within the Cairns Region. Mental First Aid Training courses run over two days and provide training to community members over the age of 18 years to assist and help someone developing a mental health problem or in a mental health crisis. The first aid is given until appropriate professional treatment is received or until the crisis resolves. The training is found to be effective in improving mental health literacy, reducing stigma and increasing helping behaviours.

The Mental Health First Aid Program was developed in 2001 by Betty Kitchener and Professor Tony Jorm. It was then auspiced as a training and research program at Orygen Youth Health Research Centre, University of Melbourne from mid-2005. Since mid-2012, it has operated as a non-profit health promotion charity called Mental Health First Aid Australia. Health Reimagined contracts Mental Health First Aid Facilitators to deliver training (Standard, Youth and Aboriginal and Torres Strait Islander) to the Cairns Region with the focus to deliver Queensland wide utilising the contractor model in place. Human Resources Training for Health Professional (General Practice Managers, Small Business Owners, Aged Care Facility Managers) HRe in partnership with local based HR Dynamics developed a 2 Day Human Resources Training Package to assist and provide information and training to General Practice Managers, Small Business Owners, Aged Care Facilities in all areas of Human Resources. The package included the option of further one-on-one, in-house human resources training with attendees focusing on the individual business. The facilitation and delivery of the 2 Day Human Resources Training Package in Cairns was a success and HRe was approached by the NQPHN to deliver the same training package to professionals in Townsville and Mackay, provided by Federal Government funding. Positive feedback on the training package was received and HRe will look at future training packages to deliver. Dialectical Behaviour Therapy (DBT) Training HRe in partnership with Expressive Therapy Clinic, facilitated and delivered a 2 day specialised and intensive Dialectical Behaviour Therapy Training course in Cairns for Mental Health Professionals in the Cairns Region. DBT Skills are extremely useful for people assisting in management of impulsive behaviours, mood changes, out of control emotions, anxiety and depression and urges to use substances, food, gambling etc. to avoid or get rid of uncomfortable or intense emotions. HRe identified a need within the Cairns Region for such training and will continue to deliver similar courses to upskill mental health professionals to improve the overall mental health and wellbeing of the community. Applied Suicide Intervention Skills Training HRe in partnership with LivingWorks, facilitated and delivered a 2 day specialised Applied Suicide Intervention Skill Training course in Cairns for Mental Health Professionals in the Cairns Region. Participants learn to apply a suicide intervention model and recognised the signs when someone may be at risk of suicide. HRe identified the need within the Cairns Region for such training and will continue to deliver similar courses to upskill mental health professionals to improve the overall mental health and wellbeing of the community. Development of an Education & Training page on Health Reimagined Website HRe identified the need for a ‘One stop shop’ of Professional Development options for Health Professionals and community members within the Northern Queensland footprint. HRe will continue to develop and promote training opportunities through the engagement of stakeholders with goal of upskilling enabling the improvement in the health and wellbeing of the community. Mental Health First Aid Resources E-Mental Health Resources in Your Practice (online learning) Telephone Cognitive Behaviour Therapy Training (online learning) Professional Development options – Australian Counselling Association

Health Care Homes Readiness Program HRe were successful in a tender submission to the NQPHN for the Health Care Homes Readiness Program to provide readiness activities for general practice in preparation for the introduction of the system by the Federal Government to replace the current chronic disease items MBS model in 2018. Active Healthy NQ – Mental Health First Aid Training Program

HRe were successful in obtaining Federal funding from the NQPHN to facilitate and deliver a HRe program initiative. The Active Healthy NQ Grants Program aims to improve health outcomes and minimise risk of chronic disease in the Northern Queensland community. The grants program provides funding for health promotion and illness prevention initiatives which aim to change community attitudes and behaviour and create environments that support people to make healthier choices. The Active Healthy NQ – Mental Health First Aid Training Program, as developed by HRe, will upskill 45 community members from 45 different organised sport, recreational, active and healthy organisations within the Cairns local government area. The program will contribute to the improved mental health and wellbeing in the local area and will roll-out from November 2017 to March 2018.

Queensland Woman’s Week – Mental Health First Aid Training Program

HRe have completed a submission to Young Women’s Christian Association (YWCA) Queensland and Department of Communities, Child Safety and Disability Services for state funding to facilitate and deliver Mental Health First Aid Training to 8 community members employed and delivering vital services in the areas of domestic violence and women’s shelters during within the Cairns Local Government Area during Queensland Women’s Week 2018 (3-11 March 2018). The program will contribute to the improved mental health and wellbeing of the Cairns Local Government Area.

Accreditation

The organisation has continued to follow the structured cycle of continuous improvement embedded in the quality and continuous improvement framework. The various components of the framework include – information management, governance structure, suggestions and feedback, document management systems, internal reviews, information, communications and technology, and performance. Policies, Procedures, Induction and Training To facilitate a sound knowledge among staff of the reviewed policies and procedures, in-service training has been provided at the weekly staff meetings to reinforce where to locate them. Presentations have been conducted on employee code of conduct, privacy, risk management, quality, and phone etiquette. All quality management exercises such as this are recorded in the Quality Management Database, as recommended by the certification surveyor.

Governance

A comprehensive review of all policies and procedures was undertaken over a six-month period by assessing which existing complied and were required to meet the International Standard of Quality Management Systems. HRe subscribed to the Standards Performance and Pathways on-line systems to direct this review. HRe document management and control system, SharePoint, underwent a complete review and was improved to reflect the focus of HRe and to ensure that it was user friendly for ease of staff access. Following a review and approval process, HRe policies and procedures were uploaded into the newly structured SharePoint system. Assessment body, Best Practice Certification was engaged to carry out a desk assessment of all HRe policies and procedures and an on-site survey assessment was conducted over two days in September. HRe systems passed the audit with the surveyor strongly recommending that we be accredited. HRe received a three-year accreditation for ISO 9001:2015 Quality Management Systems.
