Prioritaskan Modus Kegagalan
Modus kegagalan harus dilakukan prioritas sesuai dengan prioritas tindakan.
Cara prioritas :1. Pareto chart2. Numerik : Cut off point
Nilai dibawah cutoff point tidak memerlukan tindakan segera kecuali tersedia waktu Nilai di atas cutoff point , harus dilakukan eksplorasi.
Jika Nilai RPN berada di bawah Cut off point tapi Nilai Severity nya tinggi (8 atau lebih / 4 atau lebih) maka perlu pendekatan kuantitastif & kualitatif : tetap di prioritaskan

Reducing CouplingAnother redesign strategy that deserves consideration is reduc-ing the force with which the process steps are coupled, or lesstightly coupling the process steps. One goal is to reduce thespeed of step sequencing so that when a variation occurs in onestep or link, it can be responded to before the next step or linkoccurs. Perhaps the number of steps can be reduced and theprocess simplified; perhaps the number of persons involved
and the number of handoffs can be reduced to decrease the riskof failure; perhaps time constraints can be loosened or analarm system can be introduced to increase detectability of fail-ure when it does occur. Sending e-mail and mailing letters in apostal box are tightly coupled processes. Once the sender hasclicked on the send button or dropped the letter in the mail-box, there is no way to undo the command or reclaim the let-ter. Perhaps an additional step could be added to each process.Another redesign strategy involves tightening time constraints tohelp decrease failures that result from boredom, inattentiveness,or fatigue. Perhaps the basic design of the process steps needs tobe examined and additional steps considered.
Case StudyConducting FMEA on the Opioid Prescribing Process at the University of Texas M.D. Anderson Cancer Center
One of the first changes that the group identified was in the order sets used to prescribe opioids, specificallyepidural and patient-controlled analgesia (PCA), which were very crowded with information, and were being inter-preted differently by physicians, nurses, and pharmacists. The order sets needed to be redesigned to make it eas-ier to read and complete, as well as to accommodate changes to the prescribing process.
Action items were identified to address each root cause, and then noted as being a short- or long-term plan. Seethe following table.
