Dear Parents/Guardians of ASD Students:

Transition refers to activities meant to prepare students for adult life. At ASD, it is our aim to help students achieve their personal and professional goals through career exploration classes, transition and academic assessments, work experience, and other experiences that match their interests, abilities, skills and needs. The transition process will guide your student from self- awareness to career awareness to career exploration and ultimately, to career decision making.

In high school, students begin to develop a plan, in collaboration with their families and educational staff, for transitioning into post-secondary education/ training or employment. This may include developing post-secondary education and employment goals, getting work experience while still in school, and setting up links with adult service providers such as a vocational rehabilitation agency.

You are your student’s connection to programs and services when they are under the age of 18. After the age of 18, many students may still need assistance negotiating service delivery and post-secondary agencies. Your involvement in this process is essential to your student’s success in the future.

Transition Planning AssessmentsTransition Planning assessments are part of an ongoing process of gathering information that will help students, educators, and family members make informed decisions about life, during and after high school.

The purpose of a transition assessment is to identify a student’s strengths, preferences, interests, and needs. The results will assist in the development of realistic goals and objectives. Specific transition assessments will vary from student to student depending on need and ability. Please refer to the Transition Assessment information that is included in this guide.

During the sophomore, junior and senior year, your student may be recommended to take the ACT (American College Test) at a cost of $39.00-$42.00. The fee and a signed permission form are required for students to take the exam. ACT scores measure the student’s current level of educational development and skills that are essential for admission to college. Generally, the minimum score for college admission to Gallaudet University and NTID is 14 or 15, but a score above 15 is preferable.

Transition Planning Agency InformationDuring your student’s sophomore year, you should contact your state’s Vocational Rehabilitation Service agency. Vocational Rehabilitation can assist you and your student to plan for life after high school, with services such as post-secondary education options, employment support, assistive devices and job training. Vocational Rehabilitation counselors work closely with you and your student to determine if you are eligible for services.

Eligibility is determined by a review of records, assessments and evaluations. Establishing the need for services is a critical decision and the final decision is made by your state agency. Be advised, adult services are not automatic. You must start this process early and be persistent in working with the system.

Please refer to the Transition Planning Resource List for your state that is included.With your commitment, guidance, and support, your student will be given the educational foundation and tools needed to confidently transition into the adult world. Transition Assessments

The Career Code Interest Inventory

Career Codes are based on the Holland Code Career Theory that represents a set of six personality types/vocational interests formulated by Dr. John Holland (Realistic, Investigative, Artistic, Social, Enterprising and Conventional). The Career Codes/Holland Codes are referred to by their first letters: RIASEC and graphically represented in a hexagon shape.

To complete the Career Code Interest Inventory, students are asked to examine a list of activities and check any that are enjoyable or interesting. Afterward, the list is tallied to yield the top three interests areas (RIASEC)—their personal Career Code. This Career Code correlates to a group of possible career options based on their interests and personality.

Visual Enderle-Severson Transition Rating Scale

The Visual Enderle-Severson Transition Rating Scale is an informal, criterion-referenced transition assessment designed for students with mild disabilities. It provides scores in the areas of Employment, Recreation and Leisure, Home Living, Community Transportation/Participation and Post-Secondary Education. Student strengths and areas of concern are highlighted for each area.To complete the Visual Enderle-Severson Transition Rating Scale, students are asked to view pictures of Employment Options, Recreation and Leisure Activities, Home and Living Arrangements, Employment Possibilities, Community Transportation/Participation Options and Post-Secondary Education Interests and check any that would apply to their future goals. The results identify the student’s present level of performance with respect to their long range goals and expectations for employment and adult living.

The Holland Career Interest Inventory (Holland Codes)

Holland Codes represent a set of six personality types/vocational interests formulated by Dr. John Holland (Realistic, Investigative, Artistic, Social, Enterprising and Conventional). He believed that any person could be described as having interests associated with each of the six types in a descending order of preference. It is usually the top three most dominant codes that are used for vocational interest information. The Holland Codes are referred to by their first letters: RIASEC and graphically represented in a hexagon shape. The shorter the distance between their corners on the hexagon, the more closely they are related.

The six personality and work environment types are described as follows: Realistic – practical, physical, hands-on, tool-oriented Investigative – analytical, intellectual, scientific, explorative Artistic – creative, original, independent, chaotic Social – cooperative, supporting, helping, healing/nurturing Enterprising – competitive environment, leadership, persuading Conventional – detail-oriented, organizing, clerical

Transition Self-Assessment Checklist

The Transition Self-Assessment Checklist is a written self-assessment that encourages students to evaluate and synthesize important information they will need when considering a future career.

PICS (Picture Interest Career Survey)

PICS is an occupational interest assessment for students with limited reading ability or special needs. Students identify occupational/career interests by using pictures of people at work rather than text-based items.

PICS has 36 sets of 3 pictures and students are asked to choose which of the three portrayed occupations seems most interesting. Based on the number and kind of pictures selected, PICS creates a student profile which then leads directly to career information and potential job matches.

PICS uses two of the most widely accepted occupational coding systems:

RIASEC (Holland Codes) and “people, data, things, ideas”.

The ACT: What Do My Scores Mean?

The ACT is a curriculum-based, nationally normed assessment program for 11th and 12th graders. It measures current level of educational development in English, Mathematics, Reading and Science. Knowledge and skills in these areas are generally essential for admission to college and are considered important for success in college studies. The ACT also includes a Career Interest Inventory component.

Your Composite Score and each Test Score (English, Mathematics, Reading and Science) range from 1 (low) to 36 (high). The Composite Score is the average of your four test scores.National Ranks are also listed. Your rank indicates the approximate percentages of recent hearing high school graduates in the U.S. and your state who took the ACT and received scores that are the same as or lower than yours. (Example: a rank of 50 means that 50% of students received scores that are the same as or lower than your score.)

You can use the ranks to get a sense of your strengths and weaknesses in the four areas represented by the test scores and in the seven specific areas represented by the sub scores. A high rank in the content area may suggest a good chance of success in related college majors and careers. A low rank may indicate that you need to develop your skills more in that area. ACT test scores; along with information about high school grades, courses, class rank, current school and community activities, planned college activities and work experience help colleges identify applicants who can benefit most from their programs.

DMHAS:

The Department of Mental Health and Addiction Services (DMHAS) promotes and administers comprehensive, recovery-oriented services in the areas of mental health treatment and substance abuse prevention and treatment throughout Connecticut.

While the Department's prevention services serve all Connecticut citizens, its mandate is to serve adults (over 18 years of age) with psychiatric or substance use disorders, or both, who lack the financial means to obtain such services on their own. DMHAS also provides collaborative programs for individuals with special needs, such as persons with HIV/AIDS infection, people in the criminal justice system, those with problem gambling disorders, substance abusing pregnant women, persons with traumatic brain injury or hearing impairment, those with co-occurring substance abuse and mental illness, and special populations transitioning out of the Department of Children and Families.

NEW YORK MEDICAID HEALTH HOME

Health HomesWhat is a Health Home? A ‘Health Home’ is NOT building or place. A Health Home is a group of providers in your community who work together to provide your medical care. These Providers include doctors, therapists, counselors, care managers, legal advocates, vocational supports, and many others. The goal of the Health Home model is to make sure that you receive the necessary medical care.

How do these “Health Homes” even know who I am? You are identified by Medicaid and assigned to a Health Home based on the medical services that you have received already (like hospital visits or overnight stays).

What is a care manager?A care manager (also called “care coordinator”) is a person who works with you almost like a personal assistant would. A care manager can:

Talk with you about your goals and needs Help you find the services you need to stay healthy Listen to your feedback about how you feel you are doing Help you change your services if you are not happy Help to make sure that your providers are working together

If this is based off of my Medicaid…Is this mandatory?No. The Health Home program is completely voluntary and paid for by Medicaid been created to help you stay well and happy. You will not lose your Medicaid insurance if you are not interested in the Health Home program.

What if I join, and then decide I don’t like it?If you don’t like the Health Home, you can stop the services. Your Medicaid will not be turned off because of that.

Who can join a health Home? You must be 18 years of age or older. You must be eligible for and receiving Medicaid. And have 2 chronic medical conditions; including mental health issues

How can I find out more, for me or someone in my family?Talk to your discharge planner.
