Critical Incident Technique C.I.T.
• Randomly Select Sample Group
• Pre-Interview Meeting

• Interviewer is best to be unknown to the interviewees
C.I.T. Sample Questions
• Have you ever seen anyone use a power tools without the proper guarding or Personal Protective Equipment? Please tell me about it.
• Have you ever seen anyone slip on something left on the floor, walkway or stairs? Please tell me about it.
C.I.T. Sample Questions
• Have you ever seen anyone place themselves in a hazardous position? Please tell me about it.
• Please tell me about any “close calls” or accidents that you have been involved in or witnessed in the past 5 years. Please don’t reveal any names of the participants.
• Are there other incidents/situations that you would like to talk about?
Incident Imaging
• Simple but effective tool
• Advantage - You don’t have to “wait” for something to happen to perform an analysis of risk factors.
• Provides better information than “near miss reporting” if done with staff.
