kwaliteit door middel van verbinding
- Regionale implementatie van samenwerkingsafspraken
- Komen tot gezamenlijke educatie en richtlijnen
- Transmuraal multidisciplinair overleg bij complexe patiënten

- Landelijke aanpak: Samenwerking op gebied van klinisch wetenschappelijk onderzoek, betere registratie en (transmurale) afspraken/richtlijnen

80 jaar, weduwe, woont zelfstandig.
VG: Hypertensie, permanent AF, TIA, gonartrose, presbyacusis.
Sample size: 2.500 (1250 per arm): 1250 switchen naar NOAC (geen voorkeur);
1250 continueren VKA.
Follow-up: 1 jaar.
Nauwe samenwerking met trombosediensten in werven/includeren van patiënten.
DUTCH-AF Registry (Project 848050006)
- DUTCH-AF Registry – Prospective evaluation of dosing and adherence to anticoagulant treatment and the risk for bleeding in atrial fibrillation
- Start 01-01-2018, N=6000 AF-patients, cardiologist, local thrombosis service and
general practitioner
Subsidie aanvraag VWS/ZonMw
- 2016 VWS/ZonMW stelt 1,5 mln Euro ter beschikking, mits;
- er zowel een onderzoekproject als een registratieproject komen in samenwerking met de 1e lijn
- na 4 jaar de onderzoeksvragen beantwoord zijn
- na 4 jaar de registratie gecontinueerd wordt
Registratiedoelen DUTCH-AF
- Binnen een periode van 2-3 jaar een minimale dataset registreren van 6000 AF-patiënten in NL
- Follow-up gegevens verzamelen om hiermee de (eerste) onderzoeksvragen te kunnen beantwoorden
- De basis leggen voor een voortdurende (> 4 jaar) nationale AF-registratie* en deze onderbrengen in de Nederlandse HartRegistratie (NHR)
* Met als doelen evaluatie van kwaliteit van zorg en een basis voor toekomstig wetenschappelijk onderzoek
Eerste onderzoeksdoelen DUTCH-AF
- DUTCH-AF Registry – Prospective evaluation of dosing and adherence to anticoagulant treatment and the risk for bleeding in atrial fibrillation
- Objectives: Investigate how daily use (dosing and adherence) of anticoagulants in
patients with AF is related to ischemic stroke, major and clinically relevant non-major bleeding, and (cardiovascular) death
Determine which patient, doctor and practice characteristics are related to improper use /non-adherence and develop a tool to predict therapy non-adherence and persistence
Validate and refine existing bleeding models for NOAC and VKA users.

Objective: to evaluate whether rivaroxaban is superior to ASA in reducing the risk of recurrent stroke and systemic embolism in patients with a recent ESUS
NAVIGATE ESUS is Designed to Examine the
Value of Reduced Dose Rivaroxaban in ESUS
Total study duration: ~3 years; mean treatment duration per patient: ~2 years. Patients will be recruited in 31 countries at over 450 sites worldwide
