Application Due by. . .The American Kennel Club must receive this application and corresponding judging panel at least 18 weeks prior to the closing date of the Show/Trial. (Chapter 2, Sections 4 & 9 and Chapter 4, Section 1 of the Rules Applying to Dog Shows.)
Submit a separate application for each event. A separate application form is required for Agility (See www.akc.org, Keyword Agility Application), Tracking (see www.akc.org, Keyword Tracking Application), and Rally (See www.akc.org, Keyword Rally Application).

SWEEPSTAKES OR FUTURITIES (IF APPLICABLE)
• Clubs extending invitations to prospective Sweepstakes/Futurity judges must inform them that if they are approved, neither they nor any person residing in the same household with them shall exhibit or act as agent or handler in the regular competition of the show and dogs owned wholly or in part by them or any member of their household shall be ineligible to be entered in any competition of the show.
• Judge information (Name, Address and Judge’s Number, if available) should be submitted on the Specialty Club Judges Panel form. Foreign judges must be submitted 6 months before closing date of event with a list of the breeds to be judged.
• All dogs participating in Puppy Sweepstakes and/or Futurity must be at least 6 months of age.
• In Sweepstakes and Futurities, no more than 35% of entry money may be retained for expenses. Information on division of money as prizes, as well as any other prizes to be awarded, must be provided to the Superintendent/Event Secretary for inclusion in the premium list.
You must return the completed forms for the events you are holding. Specifically, page one must be returned with all applications along with page 2 and/or page 3 and/or page 4 and/or page 5. Note: All applications require the signature of an officer or event chairperson of the club. Any changes after initial event approval must be submitted in writing, again signed by a club officer or event chairperson, to the AKC for approval before a new event approval will be issued.
