ADDENDUM NO. 3 ITB-ENG-09-04 Royal Palms Waterway Pedestrian Connection ISSUE DATE: May 19, 2009 BID DATE: May 26, 2009 @ 2:00pm SUBJECT: Miscellaneous Answers to Questions INTENT: This addendum is issued prior to the date bids are due in order to incorporate the following clarifications, additions, omissions, deletions, or changes into the Contract Documents. Except as hereinafter specified, the work shall be in accordance with the drawings and specifications. Bidders are to use the changed quantities of the items listed on this addendum in their proposals, and it shall become a part of the Contract Documents when construction is executed. Bidders are reminded that this addendum must be noted on the “Official City Acknowledgment Form” when they submit their proposal. A signed copy of the addendum must also be included with the bid submittal. 1. The limits for the sidewalk along Royal Palms Parkway (deductive bid item) are stated on Sheet 4 of 14, Note #9. The width and length of the proposed sidewalk is 10 feet and 550 linear feet respectively. 2. There is no need for an office trailer as indicated in Section 0900, Subsection 3 of the specifications. 3. Note #20 on Sheet 2 of 14 indicates that it is the contractor’s responsibility to obtain all necessary permits that may be needed for the job. These permits include but are not necessarily limited to a City issued Right of Way Permit ($35 Fee), a Flagler County Right of Way Permit (Estimated Fee of $600) and NPDES Permit. 4. The Prefabricated Bridge is included in the project pursuant to note 1 on Sheet 14 of 14. This note describes the nature of construction activities, which indicate the scope of the proposed project; “THE PROJECT CONSISTS OF APPROXIMATLY 630 LINER FEET OF IMPROVEMENTS ALONG THE EASTERN SIDE OF THE BELLE TERRE PARKWAY (120’ OF WHICH IS THE PRE- FRABRICATED BRIDGE) AND APPROXIMATLY 550 LINER FEET OF IMPROVEMENTS ALONG THE SOUTHERN SIDE OF ROYAL PALM PARKWAY.” In addition, for additional information on the bridge, bidders should review Division 13 – Special Construction in the specifications. 5. Design information / calculations for the Pedestrian Bridge Foundation Design are attached. 6. The bollard indicated on Sheet 12 of 14 shall be BC13 Solar ‘Nova’ Bollard / Column Light (www.solarilluminations.com) or an approved equal.
