Violation of Contract FormViolation of Contract FormThis form must be submitted to The Office of Admissions if a student violates his or her contract by committing an action of voidance as noted on the GAP Participation Acceptance Form.

Submission of this form relieves the advisor of all duties associated to that particular student.

Course Schedule Conflict Course Schedule Conflict FormForm

A student must submit this form to his or her GAP departmental advisor if issues arise during the registration process and the student cannot enroll in a course required for that semester. Also, this form should be used if the student is dropped from a particular course section due to circumstances beyond both the student’s and the University’s control (I.e., cancelled section, time change).

The form must be approved by the GAP advisor and a copy should be placed in the student’s advising file.

This form is used to interpret students’ standardized test scores and assess GAP participation eligibility.

If a student’s score on a particular test section requires him or her to enroll in developmental coursework, then the student is automatically ineligible and the student’s status must be recoded in BANNER.

This form is to be used by the departmental GAP advisor during each semester’s advising session. Similar to University College’s procedure, the advisor keeps a copy of the form in the student’s folder. The student and the student’s major department receive copies as well.

FERPA Consent FormFERPA Consent FormIn accordance with The Family Educational Rights and Privacy Act (FERPA), this form must be completed at an advising session if anyone besides the student attends that advising session. The form serves as written release of education records to whomever (parent, spouse, friend, etc.) is in attendance.

NOTE: Once this form is submitted to the GAP advisor, the person(s) named on the form will be able to access that particular student’s educational records. Access to the student’s records must be revoked by the student in writing.
