A student must submit this form to his or her GAP departmental advisor if issues arise during the registration process and the student cannot enroll in a course required for that semester. Also, this form should be used if the student is dropped from a particular course section due to circumstances beyond both the student’s and the University’s control (I.e., cancelled section, time change).

The form must be approved by the GAP advisor and a copy should be placed in the student’s advising file.

FERPA Consent FormFERPA Consent FormIn accordance with The Family Educational Rights and Privacy Act (FERPA), this form must be completed at an advising session if anyone besides the student attends that advising session. The form serves as written release of education records to whomever (parent, spouse, friend, etc.) is in attendance.

NOTE: Once this form is submitted to the GAP advisor, the person(s) named on the form will be able to access that particular student’s educational records. Access to the student’s records must be revoked by the student in writing.
