ACCIDENTS
All ACCIDENTS regardless of how serious or minor they appear MUST BE
REPORTED IMMEDIATELY to the Safety Department at 800-543-7349. If the
Safety Department is not available when you call during office hours, tell Dispatch or the
Administrative Assistant that you have had an accident and you will be transferred to the
appropriate person. After hours, you may contact the Safety Department at (419) 348-
4355.
IF YOU ARE INVOLVED IN A TRAFFIC ACCIDENT OR HAVE CAUSED
DAMAGE TO SOMEONE ELSE’S PROPERTY, YOU MUST FOLLOW THESE
INSTRUCTIONS:
1. SECURE THE SCENE
STOP!
Shut off your vehicle.
Turn on your emergency flashers.
Put out your reflective triangles within 10 minutes.
See if anyone is injured.
2. NOTIFY THE AUTHORITIES
See if there is a fuel or haz-mat spill.
Call the police.
Call the Safety Department at Garner Trucking.
Don’t leave the scene.
3. DOCUMENT THE ACCIDENT
Be polite and courteous.
Obtain witness names, addresses and phone numbers.
Get the license number of any vehicles at the scene; they may be witnesses.
Don’t apologize or admit guilt.
Don’t talk to anyone except the police, a Garner official, and our insurance
investigator.
You can ask to see I.D.
Take pictures from the north, south, east, and west, and from varied distances
from the crash scene.
DO NOT take pictures of injured people.
Fill out your preliminary accident report.
OBTAIN INFORMATION FROM EVERYONE INVOLVED: name, driver’s
license number, address, phone, type of vehicle, license plate number.
All accidents and/or incidents, regardless of how minor, must be reported immediately
to the safety department. Failure to report accident/incident will result in termination
of employment.

Return the preliminary accident report and camera to the Safety Department, and
fill out a full accident report.
Pick up a replacement accident report and camera from the Safety or Human
Resource Departments.

ACCIDENT REVIEW BOARD (See Associate Handbook, Section 4, page 2, Disciplinary Rebuttal)
We have established an Accident Review Board for drivers who choose to appeal
preventable accident rulings. The Board consists of safety, operations, human
resources, operations, and driver representatives.
To appeal a ruling, please contact the Safety Department and the process will be
explained.

LOGS
1. If the assigned truck is equipped with an Electronic Log Unit, driver must
maintain current and legal status. If the assigned truck is not equipped with an
Electronic Log Unit, or it fails to function accurately, driver must maintain paper
logs daily and shall turn them in weekly with payroll. Failure to comply may
result in delays in payroll and disciplinary action up to and including termination.
2. If you are using paper logs, they must be turned in each week with your payroll
statement. Log dates must match the payroll statements. Example: If you turn in
a payroll statement for June 23rd through June 29th, then you must also turn in
completed logs for those days. Failure to turn in your logs weekly will result in
your paycheck(s) being held until your logs are received.
3. The cities listed on your logs must be the same cities that are listed on your
payroll statement.
4. In the event you are utilizing paper logs, You must complete a log for every day
whether you are on or off duty. You may list unlimited days on an OFF DUTY
log, however, you are only permitted to have one calendar month on the log at
one time.
5. A Pre-Trip and Post-Trip Inspection must be logged each day. If you run E-logs,
you must document daily post trip inspections on a Driver’s Vehicle Inspection
Report Booklet. The original white must be turned in with weekly payroll. The
yellow copy stays with vehicle.
6. PUCO and DOT Inspections, moving violation and drug/alcohol testing must be
logged as On Duty Not Driving.
7. Mileage must be shown for each day.
8. Every shipping number must be shown on the log, regardless of how long you had
the load. Example: If you pick up a load at 11:30 p.m., that shipping number still
needs to be on your log.
9. All receipts turned in must match the dates on the payroll statements or you won’t
be reimbursed. You will not be reimbursed for receipts that are turned in a week
late. The times printed on the receipts must match your log. If you smudge the
times or dates on receipts, that will be considered falsification. Log falsifications
will not be reimbursed.
The next two pages show examples of how the logs are to be completed. If you have
any questions regarding the logs, feel free to contact the Safety Director.

When drawing the line for the appropriate duty
status, draw the line in the middle of the row (the
white area). Also, for duty status changes (located
at the bottom of the grid) the line MUST be drawn
vertically on the bottom of the grid and go through
a white box below.

Write letters/numbers neatly within the white
boxes. Example:
Note: Be sure the numbers are written large
enough inside the boxes but do not extend outside.
The scanner only reads inside the boxes. For
example: if a “2” extends outside the bottom of
the box it could be mistaken for a “7”, etc.
No stray marks anywhere in a white area of the
log. The scanner will try to read whatever field
the stray mark is in. This will be a misread field
and will effect the accuracy of the log. In the
example below, the scanner will read a Co-Driver
Name when there is none. Also, if hour and 1/4
hour does not apply to your total time, leave those
boxes empty. The scanner will read numbers in
the “Total Hours” boxes when there should be
none.

Use a black pen when possible. Blue or red pens
may work if the line is dark enough. Pencils are
not recommended.
When removing the log from the book, be careful
not to tear the corners. The corners have
imprinted black boxes that are needed for
scanning purposes.
If a field does not have a value, leave the field
blank. Do not fill in the squares with zeros or
dashes. Also, DO NOT enter leading zeros in the
month and day fields.

LOG AUDITING
Garner Transportation Group is committed to operating in a safe and legal manner at
all times. Garner Transportation Group has established a zero-tolerance standard for log
violations. The company will audit driver's logs to ensure compliance with the Federal
Motor Carrier Safety Regulations, Part 395, as applicable to drivers of property-carrying
vehicles. Continual noncompliance with hours-of-service regulations will not be tolerated
at Garner Transportation Group.
Paper Log Auditing Procedures—for those drivers without working E-Logs
Garner Transportation Group’s paper log auditing procedures consist of four
steps.
1. First, all drivers are expected to submit accurate, true, neat, and legible
daily logs in a timely manner.
2. Second, daily logs will be forwarded to Garner Transportation Group’s
Safety Department for audit and review.
3. Third, drivers will be given feedback and/or corrective action will be
suggested based on results of log audits.
4. Corrective measures/actions instituted.
Log Submission
Garner Transportation Group requires all drivers to submit daily logs in a timely
manner.
Any corrections to the log before submission must be made by the driver only.
Auditing
After submission by drivers, logs will be forwarded to Garner Transportation
Group's Safety Department for auditing. Logs will be audited for form and
manner violations, hours-of-service violations, grid and recap violations, and log
falsification. All drivers’ logs will be retained by the company for a period of 6
months (Sec. 395.8(k)).
Electronic Log Auditing
For those trucks equipped with electronic logs, auditing will be completed daily. Drivers
must enter notes any time they show On Duty status. Examples appear as pop-up
messages on the qualcomm.
Violations such as hours-of-service violations, specifically violations of the 8, 11, 14,
and 60/70 hour rules, are serious violations. Hours-of-service abuse can cause fatigue,
which jeopardizes the safety of the driver and the general motoring public. Hours-of-
service, grid, and recap violations include:
8-Hour Rest Break: You must log a 30 minute rest break after 8 hours on duty.

Off Duty or Sleeper Berth or a combination of either qualifies as a Rest Break.
11-Hour Violation: After 10 consecutive hours off duty, a driver may not drive
more than 11 hours.
14-Hour Violation: A driver must not drive after the 14th consecutive hour after
coming on duty following 10 consecutive hours off duty.
Violation of 60/70 Hour Rule: Drivers may not drive after being on duty for 60
hours in any 7, or 70 hours in any 8 consecutive day period.
Falsification of a log shows a complete disregard of Garner Transportation Group policy
and federal regulations. This type of violation is looked at very seriously by the
company. Log falsification occurs when the times and locations on a log do not agree
with supporting documentation, such as scale tickets, toll receipts, loading and unloading
times as noted on shipping papers, etc.
Feedback Corrective Action
All violations will be brought to a driver's attention as soon as possible.
Safety Director will be responsible for reviewing the violation(s) with the driver. This
review will consist of an explanation of any violations and how to prevent reoccurrence.
The driver can request, or the driver's supervisor can recommend, hours-of-service and
logging remedial training at any time. Drivers in need of additional hours-of-service or
logging training will be referred to Garner Transportation Group's Safety Department for
scheduling.
Drivers with repeated or excessive log violations will be subject to the following
corrective action schedule. However, Garner Transportation Group reserves the right to
impose more stringent consequences based on circumstances and the severity of
violations. Situations will be judged on a case by case basis before the phased corrective
action described below is initiated.
Log Auditing Policy Corrective Action
Once a log violation has been discovered, the driver will be notified of the violation and
will be asked to come to the Safety Department for review of that violation. All
violations will remain on the driver’s record for a period of six (6) months.
If multiple violations occur within a six (6) month period, the disciplinary steps are as

Throughout all corrective action, the driver will receive continued counsel and training.
Garner staff will make every effort to help you document your logs properly; however,
there will a case by case basis consideration on the effectiveness of training. Any of the
disciplinary steps may be accelerated to suspension or termination.
DRIVER VEHICLE INSPECTION REPORTS (DVIR)
All drivers are to pick up a company DVIR sheet from the desk in the shop. You will be
required to sign and date this form, fill it out for the repairs that are needed, and place it
back in the mailbox. Then the mechanic will repair the unit, sign and date the DVIR sheet
and then the mechanic will place it in the driver’s door pouch. It is then your
responsibility to verify that the proper repairs were made on that vehicle, then sign off of
it at the bottom and place it in your payroll packet for that week.
If you get in another unit that is not yours, and there is a DVIR sheet in the driver’s
pouch, you will need to verify that proper repairs were made on that unit during your pre-
trip, and sign the DVIR sheet before leaving with that vehicle. You will also put that
DVIR sheet in your payroll packet for that week. Payroll will then give it to the safety
department for proper filing.
The finished DVIR sheet should have three (3) signatures on it when completed: the
driver’s original signature, the mechanic’s signature, and your signature at the bottom.
You also need to fill out your daily DVIR booklet during your Post-Trip Inspection.
Make certain that the same repairs and write-ups that you put on your DVIR sheet for the
shop are also on the DVIR booklet you keep in your truck. This is important for DOT
inspections and audits.
LOG RECAP OF HOURS OF SERVICE
All drivers who are on paper logs are required to keep a recap of their hours of service,
on the inside cover of their monthly logbook. This applies to all drivers that use a regular
logbook.
Failure to comply with this policy will be considered a breach of our company policy and
corrective action will be taken. The steps will include: verbal warning, written warning,
3 days off without pay, and possible termination.
Any driver could be asked to produce his or her monthly logbook for inspection, by any
company operations or safety personnel, at any time.

SPEED REDUCTION
It is the responsibility of each driver to regulate the speed of company-dispatched
vehicles to comply with state-posted speed limits, unless driving conditions dictate a
lower rate of travel.
Drivers who receive speeding tickets or moving violations must immediately report these
incidents to the Safety Director. The driving incidents will constitute a review of the said
driver’s motor vehicle driving record, performance evaluation, and the tractor’s engine
information (ECM data).
If a driver receives more than 1 speeding ticket or moving violation including but not
limited to: (seat belt, following too close, improper lane change) in a 12 month period,
the following shall occur:
The Safety Director and the Shop Supervisor shall examine both driver and
tractor engine information.
Driver will be placed on a three-month probationary period.
Online or in-house training required.
At the end of the three-month probationary period, the tractor engine shall be downloaded
again for review. Should a driver demonstrate unimproved statistical information, the
driver will be subject to further progressive disciplinary actions, up to and/or dismissal of
employment/termination.
All Garner tractors are governed. It is your responsibility to obey posted speed
limits.
IF AT ANY TIME YOU TAMPER WITH OR ALLOW SOMEONE ELSE TO
TAMPER WITH THE TOP SPEED OF A TRACTOR, YOU WILL BE
TERMINATED.
OVERLOAD FINE/VIOLATIONS
Garner Transportation Group will reimburse the driver for scale tickets. Please scale the
truck (especially when picking up a pre-loaded trailer!) NEVER ASSUME THAT THE
TRAILER HAS BEEN LOADED LEGALLY!
Given the fact that all scale tickets are reimbursed, THERE IS NO EXCUSE FOR AN
OVERLOAD! Therefore, our policy is as follows:
You pay all moving violation fines.

You pay the fines for a log book violation or expired physical card.
On a miss-load, if you loaded the trailer, you pay 25% of the fine. If you pick up
a pre-loaded trailer, it is your responsibility to weigh it, and make the gross and
axle weights legal.
Frost law or reduced weight roads. If you drive on a road that is covered by one
of these laws and you get a ticket, you will pay 100% of the fine.
On a gross overload or an axle overload, you will pay 100% of the fine if not
checked by scale.
COMPLIANCE SAFETY and ACCOUNTABILITY (CSA)
This chart is taken directly from the Compliance, Safety and Accountability (CSA) web
sight (published by the Federal Government). Please note in the chart there is a line
indicating the INTERVENTION THRESHOLD. In the sample 78.1% Rank is well over the
threshold for “Fatigued Driving”. That would put GTG in the INTERVENTIONS LIKELY
category for Carriers.
BASICs (Behavioral Analysis Safety Improvement Categories) data categories are: 1) Fatigued Driving, 2) Crash History, 3) Vehicle Maintenance, 4) Unsafe Driving, 5) Cargo Securement, 6) Driver Fitness, and 7) Controlled Substances and Alcohol. The Fatigued Driving category directly reflects Hours of Service problems.
Two of the major factors that calculate into our Driver score are:
1. Hours-Of-Service (8, 11, 14, & 70 Rule): Watch your hours and plan your
loads carefully
2. Log Book Not Current: If you are using paper logs. Your log books must
be current from your last duty point.
Major factors that calculate into our Vehicle score:
1. Brakes out of adjustment
2. Inoperative lights & airlines worn or rubbing

Required pre & post trip vehicle inspections will help minimize violations:
1. Record Post Trip in a DVIR booklet if your are using electronic logs
2. Record Post Trip on the back of your paper log if that applies
Remember, the DOT uses these scores to help them determine who may be a candidate
for their next audit or which company warrants inspections on the road.
Help us maintain a low BASIC score. We all will benefit.

CARGO AND LOAD SECUREMENT
It is the driver’s responsibility to make sure that the load (s)he is hauling is secure and
blocked properly for transport. That may mean the driver will have to block or secure the
load manually.
It is company policy to allow Garner drivers to block or secure the cargo as long as: THE
SHIPPER PROVIDES ALL NECESSARY BLOCKING MATERIAL NEEDED TO
SECURE THE LOAD PROPERLY, ALONG WITH TOOLS AND OR HAMMER AND
NAILS.
If the shipper cannot provide the necessary blocking and tools, the driver needs to call
dispatch and tell them. At this time, dispatch will call the shipper and politely tell them
of our policy and that we cannot haul the load. We will not compromise the carriers
responsibility for the drivers safety by hauling an unsecured load.
RIDER/PASSENGER REQUIREMENTS (See Associate Handbook, Section 4, page 7)
We at Garner Transportation Group realize the value of cooperating with our employees
to make everyone’s employment meaningful and profitable. Having a passenger program
has been an important part of that cooperation in the past and we have disciplined
ourselves by various methods. Our company feels that having a passenger program is our
way of showing confidence in the driver’s/employee’s ability to drive safely.
Our company proposes to use the following criteria to permit passengers in specific
situations, as follows:

Each driver/associate may have only one (1) passenger per calendar month for one (1)
trip. A “trip” is defined as leaving from the terminal and returning to the terminal,
without regard to the length of time between departure from the terminal and subsequent
arrival at the terminal.
Each driver/associate must purchase an additional insurance policy for each “trip” as
defined above. The cost is subject to change by the insurance company from year to
year. As of 2010, the policy is $25.00 and will be deducted from driver/associate payroll.
The driver/associate who is requesting a passenger permission slip must:
1. Be employed with our company a minimum of 6 months if experienced or 1
year if hired as trainee/driver.
2. Have a minimum of 1 year over-the-road experience.
3. Have 2 violations or less on their MVR.
4. Be in “good standing” with the company; i.e. deliveries on time, paperwork
correct & on time, and obey ALL company policies.
5. Sign and file the company release form with GTG, prior to each trip.
6. Never permit the passenger to perform any of the duties that are to be
performed by the driver/associate.
7. Wear their seat belt at all times.
The passenger must:
1. Be at least 18 years of age.
2. Be a member of the driver’s/associate’s immediate family.
3. Sign and file the company release form with GTG prior to each trip.
4. Follow the dress code of our company as set forth in the driver’s manual.
5. Understand that (s)he is a guest and not attempt to perform any of the duties
of a driver/associate.
6. All passengers will verify that they are in good health and if there are any
medical problems whatsoever, they will provide a medical release form from
their physician that certifies their ability to ride as a passenger.
7. Wear their seatbelt at all times.

No animals allowed:
1. At no times are animals of any kind (i.e. dogs, cats, etc.) allowed in a tractor.
***Failure to obtain authorization for a passenger and purchase insurance coverage will
result in immediate termination of employment.
NOTIFICATION OF TRAFFIC VIOLATION (See Associate Handbook, Section 4, page 6)
The Commercial Motor Vehicle Safety Act of 1986 requires that commercial drivers
notify their employer and the state that issued their license of all moving violations,
including those committed in a personal vehicle, for which the driver forfeited collateral
or was convicted, within 30 days after conviction.
Because of this Act, Garner Transportation Group requires that all drivers notify the
safety department of all moving violation convictions. This notification should include
those violations committed in a personal vehicle as well as in a commercial vehicle. This
information should be reported to the safety department as soon as possible, but no longer
than 30 days from date of conviction. A Notification of Traffic Violation Form should
also be completed and submitted, within 30 days, to the safety department.
