การดื้อยาดวยกลไกบางชนิดอาจสงผลใหเกิดการดื้อตอยาชนิดอ่ืนๆดวยได เชน tigecycline ซ่ึงเปนยาในกลุม glycylcycline ที่ออกฤทธิ์กวางตานทั้งแบคทีเรียแกรมบวก แกรมลบ และมีฤทธิ์ตอ MDR- A. baumannii ดวย ในตางประเทศเริ่มมีการใชยานี้รักษา MDR- A. baumannii ในผูปวยปอดอักเสบ ซ่ึงพบวามีประสิทธิภาพที่ดี แตในประเทศไทยการใชยาชนิดนี้ยังไมเปนที่แพรหลาย ในขณะเดียวกันเริ่มมีรายงานการดื้อยา tigecycline ของเชื้อ A. baumannii ผานทาง efflux pump ชนิด RND(35) ดังนั้นหากมีการศึกษาในประเทศไทยวาเชื้อ A. baumannii มีการดื้อยาผานกลไกนี้ จําเปนตองมีการศึกษาฤทธิ์ของยานี้ตอเชื้อดังกลาวตอไปดวย เพ่ือใหไดขอสรุปที่ชัดเจนสําหรับบทบาทการใช tigecycline ในการรักษา MDR- A. baumannii
6. บทสรุป ในปจจุบันมีอัตราการการติดเชื้อ A. baumannii เพ่ิมสูงขึ้น A. baumannii เปนเชื้อสาเหตุของการติดเชื้อในโรงพยาบาลไดในหลายอวัยวะ เชื้อน้ีกอโรคไดบอยเนื่องจากมีความสามารถในการจับเซลลและสรางสารทําลายเนื้อเยื่อทําใหเชื้อเขาสูเซลลไดมากขึ้น และสามารถคงทนอยูในสภาพแวดลอมไดเปนระยะเวลานานดวย ปญหารุนแรงเกิดจากการที่เชื้อมีกลไกการดื้อยาหลายชนิด และยังไมมีขอสรุปที่ชัดเจนในการเลือกยาตานจุลชีพสําหรับรักษาการติดเชื้อ A. baumannii ที่ด้ือยา ดังนั้นเภสัชกรควรทําความเขาใจเกี่ยวกับการดื้อยาของเชื้อ A. baumannii เพ่ือใชในการอธิบายเหตุผลการดื้อยา รวมถึงขอมูลทางคลินิกในอนาคตที่อาจตองใชขอมูลเรื่องกลไกการดื้อยาในการตัดสินใจเพื่อการรักษา
