What is an Accident

An unplanned incident or event that causes or may cause injury , lose or damage.

No one comes to work expecting to have an accident let alone leave bits of themselves at an A&E Hospital or worse

Page 5: A Part 1 Safety Induction

Accidents

Come in all sizes

At any time

Unexpected

Tend to be very personal

Invariably painful

Occasionally Body changing

No one wants them to happen

MOST ACCIDENTS FOLLOW

THIS PATTERN– Lack of management control, permitting

– Basic causes (personal & job factors), leading

to

– Immediate causes (substandard practices or

conditions), which are the direct cause of

– The accident, which results in

– Loss (negligible, minor, serious or

catastrophic

Page 8: A Part 1 Safety Induction

Caused by

•poor safety culture ???

•Lack of supervision/Training ???

•Poor systems of work ???

•He still has one good hand

Page 9: A Part 1 Safety Induction

Caused by

•Not using PPE provided

•‗Too vain to wear goggles‘ !!

Not so vain now !!!

HEALTH SAFETY AND WELFARE LAWS

ARE NEEDED TO PROTECT EMPLOYERS,

EMPLOYEES, PUBLIC AND THE

ENVIRONMENT FROM

•UNSCRUPULOUS EMPLOYERS

• STUPID PEOPLE

Page 12: A Part 1 Safety Induction

Health & Safety at Work Act

This act of parliament makes it a criminal offence not to comply with the duties it imposes.

Failure to comply could lead to prosecution in the courts with heavy fines and/or imprisonment.

Ignorance is no excuse.

Legal Standards

―Reasonably Practicable‖ or ―SFARP‖

Implies a weighing up of the risk against the cost (in terms of time, money or trouble) in preventing or controlling a risk

NOT AN EXCUSE TO DO NOTHING

― BEST PRACTICAL MEANS‖

SOME LAWS ARE ABSOLUTE i.e. you must do it (unless prevented by ‗State of Technological advances‘)

Page 14: A Part 1 Safety Induction

Section 2. GOAL SETTING

Section 7

Duty of Employees at Work

It shall be the duty of every employee whilst at work:-

MoD Safety Handbook JSP 375

Contains:

General procedures for

Emergencies, First aid &

Fire.

Information on particular

hazards.

Safe systems of work

And much more..........

Page 18: A Part 1 Safety Induction

Manual Handling The transporting or supporting of loads by hand or by

bodily force i.e. Lifting, pushing and pulling

Most accidents are caused by manual handling incidents.

Page 19: A Part 1 Safety Induction

EMPLOYERS SHOULD

AVOID the need for hazardous manual handling, as far as is reasonably practicable

ASSESS the risk of injury from any hazardous manual handling that can‘t be avoided; and

REDUCE the risk of injury from hazardous manual handling, as far as is reasonably practicable

Page 20: A Part 1 Safety Induction

Risk Assessments Management Duty

Risk assessments are the most important and effective recent safety legislation.

Before undertaking any task,

1. Identify the hazards and

2. Assess the risks.

3. Implement the measures required to remove or minimise the risk before starting work.

4. significant findings must be recorded

5. Revise and review

Page 21: A Part 1 Safety Induction

The important thing is to identify the risk of injury and point the way to practical improvements

LEAVING IT TO SOMEONE ELSE IS NEGLIGENCE

Page 22: A Part 1 Safety Induction

Employees have duties too.

They should:

Follow appropriate systems of work laid down for their safety;

Make proper use of equipment provided for their safety;

Co-operate with their employer on health and safety matters;

Inform the employer if they identify hazardous systems of work, activities or conditions;

Take care to ensure that their activities do not put others at risk.

―Well this certainly buggers up our plans to conquer the

Universe‖

Poor Communication……...

Page 24: A Part 1 Safety Induction

Supervision, Training &

InstructionMAKING SURE PEOPLE KNOW WHAT

IS EXPECTED OF THEM

Don‘t just hope people understand

Don‘t just pin up notices

Don‘t rely on Emails

Don‘t leave it till someone gets injured

Page 25: A Part 1 Safety Induction

Safety Signs

Blue signs are compulsory.

Failure to comply not only puts you at risk,

but means you have broken the law.

Page 26: A Part 1 Safety Induction

Safety Signs

Yellow signs are warnings.

Failure to take notice not only puts you at

risk, but means you could be breaking the

law if you fail to take precautions.

Page 27: A Part 1 Safety Induction

Safety Signs

Green signs are safety guidance.

First aid , emergency exits

Keep areas tidy.

Many accidents are caused by untidy Workplaces.

Trailing leads, spills not cleaned up, cluttered benches blocked passages and gangways.

Always keep your work area tidy.

DO SOMETHING ABOUT IT

Page 31: A Part 1 Safety Induction

Incident and accident reporting

Report all accidents, incidents and unsafe practices, even if no injury was caused.

This will allow action to be taken to prevent it happening to someone else and may even save a life.

Primarily poor cleaning schedules and inspections led to a build up of debris

Little or no Emergency planning or procedures

Failures of equipment

COMPLACENCY

Page 41: A Part 1 Safety Induction
Page 42: A Part 1 Safety Induction

The highest radiation doses were received by emergency workers and on-site personnel. In time more than 600 000 people were registered as emergency and recovery workers, over 5 million people received relatively low whole-body doses of radiation....... The mitigation measures taken by the authorities, including evacuation of people from the most contaminated areas, substantially reduced radiation exposures and the radiation-related health impacts of the accident.

Page 43: A Part 1 Safety Induction

It is impossible to assess reliably or with any precision, numbers of fatal cancers caused by radiation exposure due to the Chernobyl‘accident’— or indeed the impact of the

stress and anxiety induced by the ‘accident’and the response to it. A Human tragedy................. WHO
