IV. Health Information

I, the undersigned, certify that the bird(s) described above meet(s)

the following requirements:

a) I have on this day inspected the bird(s) for export and found it/them

to be free from clinical signs of infectious or contagious disease

including Newcastle Disease, Avian Influenza, West Nile Disease,

Avian Chlamydiosis, Fowl Typhoid (Salmonella gallinarum) and Pullorum

Disease (Salmonella pullorum);

b) the region of origin is free of any restrictions imposed due to an

outbreak of velogenic Newcastle Disease or notifiable avian

influenza;

c) in so far as can be determined, no case of Avian Chlamydiosis has

been confirmed at the premises of origin at paragraph II. b) above;

at the premises of origin at paragraph II (b) above, nor within a

radius of 25 kms therefore, during the 30 days prior to export.

c) in so far as can be determined, no outbreak of Ornithosis or

Psittacosis has been confirmed at the premises of origin at paragraph

II (b) above during the 3 months prior to export;

d) to the best of my knowledge and based on a written declaration from

the owner/exporter the birds for export were hatched in and have

never left the United Kingdom;

e) either* (i) the birds were kept in quarantine since hatching,

or* (ii) the birds were kept in quarantine for twenty-one

(21) days prior to shipment,

And (in either case) during the quarantine period there were no

clinical signs of disease including Newcastle Disease, Avian

Influenza, West Nile Disease, Avian Chlamydiosis, Fowl Typhoid

(Salmonella gallinarum) and Pullorum Disease (Salmonella pullorum);

f)* (applies in the case of psittacines only; (delete in all other cases)

the birds were kept under veterinary supervision for forty-five (45)

days prior to shipment and were treated against Avian Chlamydiosis

using an authorised doxycycline product according to the

manufacturer’s instructions;

g) 7 – 14 days prior to export, the birds were subjected to a test for

avian influenza with negative results;

h) no case of West Nile virus infection has been confirmed in the area

of origin of the birds for export;

i) the birds have not been vaccinated against Avian Influenza.
